## Key takeaways

- The Asia Pacific hypercholesterolemia drugs market is worth USD 5.84 billion in 2025 and USD 10.57 billion in 2035, a 6.11% revenue CAGR.
- About 112.4 million treated patient-years at USD 51.98 each form the 2025 base; volume grows 4.30% a year and price 1.74%.
- Statins lead at 62.0%; bempedoic acid and other oral agents grow fastest at 17.5% a year.
- China spends 34.0% of value; India grows fastest at 9.8%; Viatris leads with about 7.4%.
- China's 2025 NRDL cut an inclisiran year from about 75 to 21 statin years, opening hospital budgets to injectables.

Novartis sold USD 506 million of Leqvio (inclisiran) in the third quarter of 2026, up 63% at constant currencies, helped by "continued uptake in China following inclusion in the NRDL in January", its [results of 27 October 2026](https://www.sec.gov/Archives/edgar/data/0001114448/000111444826000014/a261027-99_2.htm) state, after China cut the price of a dose from CNY 9,988 to CNY 2,790. The Asia Pacific hypercholesterolemia drugs market covers prescription medicines that lower low-density lipoprotein cholesterol (LDL-C) in Asia Pacific: statins, ezetimibe and fixed-dose combinations, proprotein convertase subtilisin/kexin type 9 (PCSK9) monoclonal antibodies, small interfering RNA (siRNA) therapy, and bempedoic acid and other oral agents. Douglas Insights sizes it at USD 5.84 billion in 2025, about 112.4 million treated patient-years at an average net USD 51.98 each, and at USD 10.57 billion in 2035, a 6.11% compound annual growth rate (CAGR). The study is the Asia Pacific row of our worldwide [Hypercholesterolemia Drugs Market](https://www.douglasinsights.com/hypercholesterolemia-drugs-market/) model, sits beside our [U.S. Hypercholesterolemia Drugs Market](https://www.douglasinsights.com/u-s-hypercholesterolemia-drugs-market/) and [Asia Pacific Pharmaceutical Drug Market](https://www.douglasinsights.com/asia-pacific-pharmaceutical-drug-market/) reports inside our [pharmaceuticals coverage](https://www.douglasinsights.com/industry/healthcare/pharmaceuticals/), and every figure follows the Douglas Insights [research methodology](https://www.douglasinsights.com/research-methodology/).

## Which drug class carries the most hypercholesterolemia drugs value: statins, ezetimibe combinations or PCSK9 therapies?

Statins carry the most, 62.0% of 2025 regional hypercholesterolemia drugs value or USD 3.62 billion, Douglas Insights estimates. Bempedoic acid and other oral agents grow fastest, at 17.5% a year to USD 615.4 million by 2035.

| Drug class | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Statins | 62.0% | USD 3.62 billion | 3.2% | USD 4.96 billion |
| Ezetimibe and fixed-dose combinations | 21.5% | USD 1.26 billion | 6.2% | USD 2.29 billion |
| PCSK9 monoclonal antibodies | 10.8% | USD 631.0 million | 10.0% | USD 1.64 billion |
| Small interfering RNA (siRNA) therapy | 3.6% | USD 210.3 million | 15.5% | USD 888.6 million |
| Bempedoic acid and other oral agents | 2.1% | USD 122.7 million | 17.5% | USD 615.4 million |

Statins lead because about 96.7 million of the region's 112.4 million treated patient-years start on one, at an average net USD 37.50 a year, Douglas Insights estimates. Their 62.0% share sits above the 57.6% worldwide figure in our global model because PCSK9 and siRNA use is still far lower per patient in China, India and Southeast Asia than in North America. At 3.2% a year statins grow slowest, as volume-based procurement keeps cutting their price.

Ezetimibe and fixed-dose combinations, USD 1.26 billion, hold 21.5%, well above the world share, because South Korean and Japanese doctors prescribe statin-ezetimibe tablets early; they grow 6.2% a year. PCSK9 monoclonal antibodies, USD 631.0 million, grow 10.0% as Chinese reimbursement opens access at lower prices. Small interfering RNA therapy, USD 210.3 million, grows 15.5% to USD 888.6 million on twice-yearly inclisiran. Bempedoic acid and other oral agents, USD 122.7 million, grow 17.5% as Japan launches bempedoic acid and oral PCSK9 pills follow later in the decade. Class values for 2035 sum to USD 10.40 billion, 1.66% below the USD 10.57 billion headline.

## How do hypercholesterolemia drugs split by route, channel and patient type?

Oral tablets take about 85.6% of 2025 hypercholesterolemia drugs value in the region, Douglas Insights estimates, and subcutaneous injections 14.4%. Hospital pharmacies dispense about 52.0%, because Chinese and Japanese patients collect most prescriptions at hospital outpatient desks.

Oral tablets, about USD 5.00 billion, combine statins, ezetimibe products and bempedoic acid; injections, about USD 841.3 million, combine PCSK9 antibodies and inclisiran. The injectable share rises to about 23% by 2035, a Douglas Insights estimate, as reimbursed antibodies and siRNA reach more secondary prevention patients after heart attacks and strokes.

By channel, hospital pharmacies dispense about 52.0%, retail pharmacies 41.0% and online pharmacies 7.0%, all Douglas Insights estimates; online sales grow fastest in China and India, where repeat statin prescriptions move to platform pharmacies. By patient type, primary non-familial hypercholesterolemia accounts for about 90.5% of value, heterozygous familial hypercholesterolemia (HeFH) 8.6% and homozygous familial hypercholesterolemia (HoFH) 0.9%, Douglas Insights estimates; familial patients carry a far higher spend per patient-year because they need antibodies or siRNA on top of statins.

## What drives hypercholesterolemia drugs growth: reimbursement, untreated patients, combination therapy or new mechanisms?

Treated patient-years in the region rise 4.30% a year in our base case, from about 112.4 million in 2025 to 171.2 million in 2035. Four drivers add 5.40 gross points; restraints remove 1.10.

### Chinese reimbursement of new lipid drugs: 1.90 points

Chinese reimbursement is the largest driver, worth 1.90 points. The National Healthcare Security Administration (NHSA) added 114 drugs to the 2025 national reimbursement drug list (NRDL), effective 1 January 2026, [China Daily reported on 7 December 2025](https://www.chinadaily.com.cn/a/202512/07/WS6934f436a310d6866eb2d43c.html). Inclisiran fell from CNY 9,988 to CNY 2,790 a dose and Innovent's tafolecimab from CNY 1,388 to CNY 300 or less, and six of China's seven approved PCSK9 drugs are now listed, [The Paper reported in April 2026](https://m.thepaper.cn/newsDetail_forward_33068133). Junshi's ongericimab entered the list for 1 January 2026 to 31 December 2027, its [exchange filing of 8 December 2025](https://static.cninfo.com.cn/finalpage/2025-12-08/1224856650.PDF) shows.

### Untreated patients in China and India: 1.30 points

Untreated patients add 1.30 points. Dyslipidaemia affected 38.1% of Chinese adults aged 18 or older in 2020 to 2022 surveillance of 275,961 people, with mean LDL-C of 110.7 mg/dL, a [Biomedical and Environmental Sciences review of 9 September 2025](https://www.besjournal.com/cn/article/doi/10.3967/bes2025.099) states. In India the ICMR-INDIAB study found dyslipidaemia in 81.2% of 113,043 adults, its [Lancet Diabetes and Endocrinology paper of July 2023](https://researcher.manipal.edu/en/publications/metabolic-non-communicable-disease-health-report-of-india-the-icm/) shows. Douglas Insights estimates that only about 1 in 6 Indian adults with high LDL-C takes a statin, so each point of treatment gain adds about 2.4 million patient-years.

### Combination therapy in Korea and Japan: 1.20 points

Combination therapy adds 1.20 points. Hanmi's statin-ezetimibe tablet Rosuzet sold KRW 227.9 billion in 2025 outpatient prescriptions, up 8.4% and the largest product in that market, while its pitavastatin combination Livalozet grew 25.4% to KRW 117.0 billion, UBIST data show, [Daily Pharm reported on 14 January 2026](https://www.dailypharm.com/user/news/334760). Japan's health ministry approved Otsuka's NEXLETOL (bempedoic acid) for hypercholesterolemia and familial hypercholesterolemia, Esperion said on [19 September 2025](https://www.esperion.com/news-releases/news-release-details/esperion-partner-otsuka-receives-regulatory-approval-market), and Otsuka launched it in late 2025.

### New mechanisms and listings: 1.00 point

New mechanisms add the last 1.00 point. Australia's Pharmaceutical Benefits Scheme (PBS) shows an inclisiran listing effective 1 March 2025, its [medicine status page](https://pbs.gov.au/medicinestatus/document/1246) states, and CSL Seqirus filed bempedoic acid in Australia in July 2025 with approval expected in the fourth quarter of 2026, Esperion's [results of 10 March 2026](https://www.sec.gov/Archives/edgar/data/1434868/000162828026016230/a991q42025earningsrelease.htm) show. Merck's oral PCSK9 inhibitor Lipfendra won US approval on [16 July 2026](https://www.merck.com/news/mercks-lipfendra-enlicitide-is-the-first-and-only-once-daily-oral-pcsk9-inhibitor-approved-by-the-u-s-fda-to-reduce-ldl-c-in-adults-with-hypercholesterolemia/) after 56% LDL-C cuts against placebo; no Asian filing has been announced, so Douglas Insights adds it from 2029. Summed, 1.90 + 1.30 + 1.20 + 1.00 = 5.40 points; after 1.10 points of restraints the 4.30% volume leg times the 1.74% price leg gives the 6.11% revenue rate.

## What holds back hypercholesterolemia drugs: poor adherence, payer limits or generic price cuts?

Three restraints remove 1.10 points a year from patient-year growth: poor adherence takes 0.50 points, payer limits on new drugs 0.35 points and out-of-pocket costs in uninsured markets 0.25 points, Douglas Insights estimates.

Poor adherence is the sharpest drag. Statins lower risk only while taken, and Douglas Insights estimates that about 40% of new Chinese and Indian statin starters stop within a year, which removes 0.50 points. Twice-yearly inclisiran partly answers this, which is one reason Novartis's Leqvio sales reached USD 1.20 billion in 2025, up 57% at constant currencies, its [annual results of 4 February 2026](https://www.sec.gov/Archives/edgar/data/1114448/000111444826000007/a260204-99_2.htm) show.

Payer limits take 0.35 points. NRDL listing usually restricts new lipid drugs to patients who miss LDL-C targets on statins, and hospitals cap spending per prescription, so a listed drug still reaches only a share of eligible patients. Out-of-pocket costs take the final 0.25 points: most Indian and Southeast Asian patients pay for medicines themselves, which keeps injectables to a small group. Generic price cuts weigh on the price leg rather than volume; China's 11th national volume-based procurement (VBP) round put 55 drugs out for bid, including lipid-lowering medicines, with 272 firms winning and new prices reaching patients from February 2026, [China Daily reported on 29 October 2025](https://cn.chinadaily.com.cn/a/202510/29/WS6901ac09a310c4deea5eecfe.html).

## Where in Asia Pacific does hypercholesterolemia drugs spending grow fastest: China, India or Japan?

China buys the most, about USD 1.99 billion or 34.0% of 2025 value. India grows fastest, at 9.8% a year to USD 1.34 billion by 2035, as statin use spreads from cities to district hospitals.

| Country or sub-region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| China | USD 1.99 billion | 34.0% | USD 2.13 billion | 7.4% | USD 4.06 billion |
| Japan | USD 1.43 billion | 24.5% | USD 1.47 billion | 2.4% | USD 1.81 billion |
| South Korea | USD 584.2 million | 10.0% | USD 616.9 million | 5.6% | USD 1.01 billion |
| Southeast Asia | USD 555.0 million | 9.5% | USD 600.5 million | 8.2% | USD 1.22 billion |
| India | USD 525.8 million | 9.0% | USD 577.3 million | 9.8% | USD 1.34 billion |
| Australia and New Zealand | USD 438.2 million | 7.5% | USD 457.5 million | 4.4% | USD 674.0 million |
| Rest of Asia Pacific | USD 321.3 million | 5.5% | USD 340.6 million | 6.0% | USD 575.4 million |

China leads on volume: about 47.5 million treated patient-years at an average net USD 41.80, a Douglas Insights estimate, after procurement made generic atorvastatin and rosuvastatin among the cheapest in the world. Its value grows 7.4% a year to USD 4.06 billion because reimbursed antibodies and inclisiran add spend per patient faster than VBP removes it. Japan, USD 1.43 billion, spends about USD 100.80 per patient-year on about 14.2 million patient-years, but grows only 2.4% because its 1 April 2026 price revision cut drug prices 4.02% and patient numbers are flat.

India, USD 525.8 million, grows fastest at 9.8% from about 25.0 million patient-years at roughly USD 21.00 each; its National Pharmaceutical Pricing Authority (NPPA) fixed atorvastatin-ezetimibe 10/10 mg at Rs 19.86 a tablet, [Medical Dialogues reported on 4 August 2025](https://medicaldialogues.in/news/industry/pharma/37-essential-drugs-including-paracetamol-atorvastatin-to-get-cheaper-see-full-list-152956). South Korea, USD 584.2 million, grows 5.6% on combination tablets. Southeast Asia, USD 555.0 million, grows 8.2% as Indonesia, Vietnam and the Philippines widen insured access. Australia and New Zealand, USD 438.2 million, grow 4.4% on PBS-listed inclisiran and antibodies, and the Rest of Asia Pacific, led by Taiwan and Hong Kong, grows 6.0%. Country values sum to USD 6.19 billion in 2026, 0.12% below the headline, and USD 10.69 billion in 2035, 1.09% above it.

## Who sells hypercholesterolemia drugs, and how concentrated is the field?

Viatris holds about 7.4% of 2025 regional hypercholesterolemia drugs value, a Douglas Insights estimate, on Lipitor in China, Japan and emerging Asia, ahead of Hanmi at 3.6% and Amgen at 2.3%. The top three hold about 13.3%.

| Company | Main products | Latest filed figure | Estimated share 2025 |
| --- | --- | --- | --- |
| Viatris | Lipitor (atorvastatin) | Greater China sales USD 702.6 million, Q3 2026 | 7.4% |
| Hanmi Pharmaceutical | Rosuzet, Livalozet | Rosuzet KRW 227.9 billion prescriptions, 2025 | 3.6% |
| Amgen | Repatha (evolocumab) | Repatha USD 1.09 billion, Q3 2026 | 2.3% |
| Novartis | Leqvio (inclisiran) | Leqvio USD 506 million, Q3 2026 | 1.9% |
| Innovent, Hengrui, Junshi and Akeso | Tafolecimab, recaticimab, ongericimab, ebronucimab | Innovent revenue RMB 13.0 billion, 2025 | 1.1% combined |
| Otsuka | NEXLETOL (bempedoic acid), Japan | USD 90 million milestone paid to Esperion | 0.1% |

Viatris's Greater China net sales rose 10% at constant currencies to USD 702.6 million in the quarter to September 2026 and global Lipitor sales were USD 451.9 million, its [results of 5 November 2026](https://www.sec.gov/Archives/edgar/data/0001792044/000179204426000053/exhibit991-3q26earningsrel.htm) show; we place about USD 432.3 million of 2025 regional lipid sales with the company. Amgen sold USD 1.09 billion of Repatha in the third quarter of 2026, up 37%, of which USD 419 million outside the United States, its [results of 3 November 2026](https://investors.amgen.com/news-releases/news-release-details/amgen-reports-third-quarter-2026-financial-results) show. Hanmi's KRW 344.9 billion of combined Rosuzet and Livalozet prescriptions, about USD 242.9 million at KRW 1,420 to the dollar, make it the leader in South Korea.

Chinese antibody makers are small but growing. Innovent lifted 2025 revenue 38.4% to RMB 13.0 billion and called its PCSK9 drug SINTBILO the first China-developed PCSK9 inhibitor on the NRDL, its [results of 26 March 2026](https://www.biospace.com/press-releases/innovent-announces-2025-annual-results-and-business-updates) state. Hengrui's recaticimab was approved by the National Medical Products Administration (NMPA), its [notice of 11 June 2025](https://english.nmpa.gov.cn/2025-06/11/c_1101534.htm) shows, and Akeso licensed China commercial rights to its ebronucimab to JumpCan in February 2026 for RMB 80 million upfront, its [annual results](https://www1.hkexnews.hk/listedco/listconews/sehk/2026/0326/2026032602886.pdf) show. Generic makers in India, China and Japan share most of the remaining 83.6%.

## What does a treated patient-year cost, and how fast does the price rise?

A treated patient-year of hypercholesterolemia drugs costs about USD 51.98 net in the region in 2025, Douglas Insights estimates, 46.1% below the USD 96.40 world average, from USD 37.50 for statins to USD 780 for inclisiran in China.

The regional average sits below the world figure because generic statins dominate and Chinese procurement prices are low. A PCSK9 antibody year in China costs about USD 545 at CNY 300 a dose and 13 doses, and an inclisiran maintenance year about USD 780 at two doses of CNY 2,790, both at CNY 7.15 to the dollar, Douglas Insights estimates from the [NRDL prices](https://m.thepaper.cn/newsDetail_forward_33068133). The 1.74% price leg takes the average to about USD 52.88 in 2026 and USD 61.76 in 2035.

Two forces set the price leg. Mix shift to antibodies, siRNA and branded combinations adds about 2.90 points a year, a Douglas Insights estimate. Against that, price cuts take about 1.16 points back: Japan cut drug prices 4.02% on a drug-cost basis from 1 April 2026, the Ministry of Health, Labour and Welfare (MHLW) [revision notice](https://www.mhlw.go.jp/content/12400000/001676427.pdf) shows, and each Chinese VBP round resets generic statin and ezetimibe prices.

## What rules and policies govern hypercholesterolemia drugs, from reimbursement lists to price revisions?

Three layers of policy govern hypercholesterolemia drugs in the region: reimbursement listings such as China's NRDL from 1 January 2026, price controls such as Japan's 4.02% cut from 1 April 2026, and national drug approvals.

The reimbursement layer sets access. China's NRDL agreements run for two years, so the prices agreed in December 2025 hold to 31 December 2027, per [Junshi's filing](https://static.cninfo.com.cn/finalpage/2025-12-08/1224856650.PDF), and Australia's PBS and South Korea's national insurance list each new drug with criteria that limit use to high-risk patients. Douglas Insights treats the next NRDL renewal in late 2027 as the main pricing event of the forecast.

The price layer acts every year. Japan's 2026 revision cut prices 4.02% on a drug-cost basis and 0.86% on a medical-cost basis, per [MHLW](https://www.mhlw.go.jp/content/12400000/001676427.pdf); India's NPPA fixes ceiling and retail prices for scheduled formulations including statin combinations; and China's VBP rounds set generic prices through tenders with an anchor-price rule against extreme low bids, per [China Daily](https://cn.chinadaily.com.cn/a/202510/29/WS6901ac09a310c4deea5eecfe.html). The approval layer decides timing: Japan approved bempedoic acid on 19 September 2025, and Merck has not announced an Asian filing for its oral PCSK9 pill.

## What if reimbursement widens or prices fall faster: how large is the hypercholesterolemia drugs market in 2035?

The regional hypercholesterolemia drugs market ends 2035 between USD 8.59 billion and USD 12.77 billion across three cases, around a USD 10.57 billion base. Chinese reimbursement and Indian treatment rates set most of the spread.

| Case | Volume leg | Price leg | Revenue rate | 2035 value |
| --- | --- | --- | --- | --- |
| Slower | 3.00% | 0.90% | 3.93% | USD 8.59 billion |
| Base case | 4.30% | 1.74% | 6.11% | USD 10.57 billion |
| Faster | 5.50% | 2.50% | 8.14% | USD 12.77 billion |

In the slower case the 2027 NRDL renewal cuts antibody and siRNA prices again, Indian treatment rates stall and Japan keeps cutting prices by 4% a year, taking 1.30 points off volume and 0.84 points off price. In the faster case oral PCSK9 pills reach China and Japan by 2028 and Indian statin use doubles, adding 1.20 points to volume and 0.76 points to price. One extra point of yearly revenue growth adds about USD 1.04 billion to 2035 value, and one point less removes about USD 955.5 million, Douglas Insights estimates.

Company signals bracket the base case. Leqvio grew 63% and Repatha 37% in the third quarter of 2026, while Japan cut prices 4.02% and China ran its 11th procurement round, the sources above show. Our 6.11% sits between fast-growing branded injectables and a large generic base whose price keeps falling.

## Douglas Exclusive: the Reimbursed Dose Ledger

The Reimbursed Dose Ledger is a Douglas Insights model built from 10 inputs: 5 government or regulator documents and 5 company filings or releases. It compares what one treated year of each lipid therapy costs before and after reimbursement.

| Date | Input | What it moves |
| --- | --- | --- |
| 11 June 2025 | NMPA approves recaticimab | Antibody supply |
| 4 August 2025 | NPPA prices for statin combinations | Indian price |
| 19 September 2025 | Japan approves bempedoic acid | Oral agent volume |
| 29 October 2025 | 11th VBP round, 55 drugs | Generic price |
| 7 December 2025 | 2025 NRDL, 114 drugs added | Chinese access |
| 8 December 2025 | Ongericimab NRDL term to 2027 | Price lock |
| 4 February 2026 | Leqvio 2025 sales USD 1.20 billion | siRNA uptake |
| 5 March 2026 | Japan price revision, minus 4.02% | Japanese price |
| 27 October 2026 | Leqvio China uptake after NRDL | siRNA volume |
| 5 November 2026 | Viatris Greater China up 10% | Branded statin volume |

The finding, a Douglas Insights estimate: before the NRDL, an inclisiran maintenance year in China cost about USD 2,794, 75 times a USD 37.50 statin year; after listing it costs about USD 780, 21 times. That fall moves inclisiran inside the budget a Chinese hospital sets for secondary prevention, and we expect Chinese injectable patient-years to rise from about 0.6 million in 2025 to 3.1 million by 2035.

The ledger changes how to read the 2027 renewal. If China cuts injectable prices another 30% at renewal but volume doubles, regional injectable value still grows; if renewal adds stricter use criteria instead, the siRNA line slows to about 10% a year and the faster case falls away.

## How does the methodology turn 112.4 million patient-years into USD 5.84 billion?

Our methodology multiplies about 112.4 million treated patient-years in the region by an average net USD 51.98 to reach USD 5.84 billion in 2025, then grows patient-years 4.30% and price 1.74% a year.

The model covers 5 drug classes, 2 routes, 3 channels, 3 patient types and 7 countries or sub-regions, using 68 data points, 22 sourced and 46 modelled. Patient-years come from prevalence surveys, prescription data and company sales; prices come from reimbursement lists, price notices and net-to-list estimates, all Douglas Insights estimates. Arithmetic: 112.4 million x USD 51.98 = USD 5.84 billion; USD 5.84 billion x (1.0430 x 1.0174)^10 = USD 10.58 billion, within 0.05% of the USD 10.57 billion headline. Patient-years reach about 117.2 million in 2026 and 171.2 million in 2035.

Two cross-checks agree. First, USD 5.84 billion is the Asia Pacific row of our worldwide model, 25.4% of its USD 23.00 billion total, and our 6.11% rate equals that row's rate. Second, our Hanmi estimate of about USD 210 million sits 13.5% below its USD 242.9 million of prescription value, the usual gap between pharmacy and manufacturer prices. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our [editorial standards](https://www.douglasinsights.com/research-methodology/) explain.

## What counts in the hypercholesterolemia drugs study, and what is excluded?

The study counts prescription LDL-lowering medicines sold in Asia Pacific, valued at net manufacturer prices: statins, ezetimibe products, PCSK9 antibodies, siRNA and oral agents such as bempedoic acid. Fibrates, omega-3 products, supplements and lipid tests are excluded.

The study splits value by 5 drug classes, 2 routes, 3 channels, 3 patient types and 7 countries or sub-regions: China, Japan, South Korea, Southeast Asia, India, Australia and New Zealand and the Rest of Asia Pacific. Hospital and retail prescriptions both count; samples and trial supply do not. Worldwide figures appear in our [Hypercholesterolemia Drugs Market](https://www.douglasinsights.com/hypercholesterolemia-drugs-market/) report, lipid testing in our [Cholesterol Tests Market](https://www.douglasinsights.com/cholesterol-tests-market/) study and Indian medicine spending in our [India Pharmaceutical Drug Market](https://www.douglasinsights.com/india-pharmaceutical-drug-market/) report.
