### AstraZeneca and Jiangsu Hengrui in China

AstraZeneca is the largest single seller of medicines in the region. Its [full-year 2025 results of 10 February 2026](https://www.astrazeneca.com/content/dam/az/PDF/2025/Q4-FY/Full-year-Q4-2025-results-announcement.pdf) show China revenue of USD 6.65 billion, up 4%, inside total revenue of USD 58.74 billion. Douglas Insights estimates that equals 1.52% of the 2025 Asia Pacific pharmaceutical drug market.

Jiangsu Hengrui is the domestic champion that the 7 December 2025 NRDL update favours most. Its [2025 annual report](https://www.hkexnews.hk/listedco/listconews/sehk/2026/0326/2026032600033.pdf) on the Hong Kong exchange shows revenue of RMB (renminbi) 31.63 billion, up 13.0%, with innovative drug sales of RMB 16.34 billion, up 26.1% and 58.3% of drug sales. Excluding RMB 3.39 billion of licensing income, Douglas Insights values Hengrui drug sales at USD 3.93 billion.

### Daiichi Sankyo, Chugai, Astellas and Takeda in Japan

Japan's leaders are export-led. Daiichi Sankyo reported FY2025 revenue of JPY 2,123.0 billion, up 12.6%, with its Japan Business Unit at JPY 485.8 billion, up only 1.9%, in its [results of 11 May 2026](https://www.daiichisankyo.com/files/investors/library/quarterly_result/2025/FY2025Q4_Financial_Results_E.pdf). Chugai booked JPY 1,257.9 billion of 2025 revenue and JPY 472.4 billion of domestic sales, and flagged National Health Insurance (NHI) price revisions and biosimilar entry against Avastin, in its [29 January 2026 release](https://www.chugai-pharm.co.jp/english/news/detail/20260129170004_1224.html). Astellas reported FY2025 revenue of JPY 2,139.2 billion with Japan sales of JPY 289.0 billion, up 8.2%, in its [27 April 2026 results](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/4q2025_en.pdf). Takeda posted FY2025 revenue of JPY 4,505.7 billion, down 1.7%, in its [13 May 2026 release](https://www.takeda.com/newsroom/newsreleases/2026/takeda-reports-fourth-quarter-fy2025-results/), a group total that Douglas Insights does not split by country.

### Sun Pharma, BeOne, Celltrion and Samsung Biologics

Sun Pharma ranks first in India, with FY26 India formulation sales of INR 192,904 million, up 14%, and an 8.4% share on the Pharmarack moving annual total (MAT) to March 2026, in its [22 May 2026 release](https://sunpharma.com/wp-content/uploads/2026/05/Press-Release-Sun-Pharma-Q4FY26-Financial-Result.pdf). BeOne Medicines, formerly BeiGene, reported 2025 revenue of about USD 5.3 billion, up 40.2%, including USD 3.9 billion from the Bruton's tyrosine kinase (BTK) inhibitor Brukinsa, in its [2025 Form 10-K](https://www.sec.gov/Archives/edgar/data/1651308/000162828026011946/bgne-20251231.htm). Korea's Celltrion reported record 2025 revenue of KRW 4.16 trillion, up 17%, in its [5 February 2026 release](https://www.celltrion.com/en-us/company/media-center/press-release/4458), and Samsung Biologics booked KRW 4.56 trillion, up 30.3%, as a contract development and manufacturing organization (CDMO), per its [21 January 2026 results](https://samsungbiologics.com/media/company-news/samsung-biologics-reports-fourth-quarter-and-fiscal-year-2025-financial-results). Both sell mostly abroad.

## Which Asia Pacific pharmaceutical drug segment leads: branded innovative medicines, generics, biologics or OTC?

Branded innovative small molecules lead the Asia Pacific pharmaceutical drug market with 37.4% of 2025 value, worth USD 163.36 billion, while biosimilars grow fastest at 14.86% a year. Generics move 71.6% of units but earn only 31.8% of value.

| Segment | Share 2025 | Value 2025 | Growth 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Branded innovative small molecules | 37.4% | USD 163.36 billion | 6.62% | USD 310.13 billion |
| Generics and branded generics | 31.8% | USD 138.90 billion | 4.93% | USD 224.75 billion |
| Innovative biologics | 16.9% | USD 73.82 billion | 10.37% | USD 198.01 billion |
| Over-the-counter (OTC) medicines | 11.2% | USD 48.92 billion | 7.28% | USD 98.81 billion |
| Biosimilars | 2.7% | USD 11.79 billion | 14.86% | USD 47.13 billion |

Branded innovative small molecules earn USD 163.36 billion because Japan pays patented prices for most of its USD 79.30 billion market and China's NRDL now reimburses targeted oral oncology drugs. Generics and branded generics hold USD 138.90 billion; Indian branded generics keep prices above Chinese volume-based procurement (VBP) tenders, which explains a value share far below their unit share.

Innovative biologics are worth USD 73.82 billion and grow 10.37% a year to USD 198.01 billion, as antibodies and antibody-drug conjugates (ADCs) from Daiichi Sankyo, Chugai and Hengrui move into reimbursement. OTC medicines add USD 48.92 billion, mostly analgesics and cold remedies sold in Japanese, Chinese and ASEAN pharmacies. Biosimilars are the smallest slice at USD 11.79 billion but rise to USD 47.13 billion; Celltrion and Samsung Bioepis supply the antibody copies. Japan's Ministry of Health, Labour and Welfare (MHLW) estimates that biosimilars saved JPY 119.9 billion a year, inside JPY 1.24 trillion of savings from generic substitution, in its [fiscal 2026 price revision summary](https://www.mhlw.go.jp/content/12400000/001676427.pdf).

## Why is demand for Asia Pacific pharmaceutical drugs rising 2.83% a year in units?

Asia Pacific pharmaceutical drug volume grows 2.83% a year to 2035, from 1,872.6 billion to 2,475.4 billion standard units, and five forces explain all of it. Chinese chronic disease adds the most, 1.08 points; Indian access adds 0.71 points.

### Chinese ageing and chronic disease: 1.08 points

China supplies 1.08 points of the 2.83-point volume leg. Hypertension, diabetes and dyslipidaemia patients take daily tablets for decades, and VBP tenders have cut the cost of those tablets so far that hospitals dispense more of them. The 11th VBP round announced on 15 July 2025 covered 55 generic molecules, fixed contracted volumes at 60% to 80% of hospital demand and dropped the lowest-bid price anchor, according to the [Chinese government portal](https://www.gov.cn/lianbo/bumen/202507/content_7032255.htm). Douglas Insights estimates China dispensed 902.4 billion standard units in 2025.

### Indian access and Jan Aushadhi: 0.71 points

India adds 0.71 points. The Department of Pharmaceuticals counts 17,990 Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP) generic medicine stores open on 31 December 2025, against a target of 25,000 by 31 March 2027, in its [Annual Report 2025-26](https://pharma-dept.gov.in/sites/default/files/Annual%20Report%202025-26_0.pdf). The same report puts industry turnover growth at 13.07% in FY2024-25.

### ASEAN universal coverage: 0.49 points

ASEAN adds 0.49 points. Indonesia, the Philippines and Vietnam are widening public insurance, so prescriptions for diabetes, hypertension and tuberculosis shift from untreated to treated. Douglas Insights estimates ASEAN dispensed 244.7 billion units in 2025.

### NRDL listings and biologics: 0.55 points

China's NRDL update adds 0.33 points. The 2025 list carries 3,253 drugs after 114 additions and 29 removals, effective 1 January 2026, per the [Guangzhou government notice](https://www.gz.gov.cn/zwfw/zxfw/ylfw/content/post_10582536.html); Douglas Insights assumes a negotiated listing triples a new drug's hospital volume within two years. Biologics, including glucagon-like peptide-1 (GLP-1) drugs for diabetes and obesity and insulin pens, add the final 0.22 points.

The five contributions, 1.08 + 0.71 + 0.49 + 0.33 + 0.22, sum to the 2.83% volume leg. Price adds 4.29% a year, mostly mix: each unit that moves from a VBP generic at USD 0.104 to an innovative biologic at USD 12.72 lifts the regional average. Douglas Insights calculates that mix adds 5.47 points to price, while Japanese NHI cuts remove 0.41, Chinese VBP 0.62 and Indian price ceilings 0.15.

## What holds back Asia Pacific pharmaceutical drug volumes in Japan and Chinese hospitals?

Three restraints remove 0.64 points from Asia Pacific pharmaceutical drug volume growth, which would otherwise run at 3.47% a year. Japan's shrinking population, Chinese hospital payment caps and antibiotic stewardship each subtract a measured slice.

Japan removes 0.29 points: the region's second-largest pharmaceutical drug market, at USD 79.30 billion, has a falling population, so Douglas Insights holds Japanese units close to flat through 2035.

Chinese hospital payment caps remove 0.21 points. Diagnosis-related group (DRG) and diagnosis-intervention packet (DIP) payment gives hospitals a fixed fee per case, so they trim drugs per admission. Antibiotic stewardship removes 0.14 points, as China, India and Australia cut broad-spectrum anti-infective prescribing.

## Where in Asia Pacific is pharmaceutical drug spending growing fastest: China, India or ASEAN?

China is the largest Asia Pacific pharmaceutical drug country at USD 208.60 billion, 47.76% of 2025 value, while India grows fastest at 10.64% a year. ASEAN is the wildcard, adding USD 59.05 billion by 2035 as public insurance widens.

| Country or sub-region | Value 2025 | Share 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| China | USD 208.60 billion | 47.76% | 8.43% | USD 468.61 billion |
| Japan | USD 79.30 billion | 18.15% | 1.12% | USD 88.64 billion |
| ASEAN | USD 43.60 billion | 9.98% | 8.94% | USD 102.65 billion |
| Rest of Asia Pacific | USD 35.90 billion | 8.22% | 7.79% | USD 76.00 billion |
| India | USD 26.90 billion | 6.16% | 10.64% | USD 73.94 billion |
| South Korea | USD 22.70 billion | 5.20% | 5.38% | USD 38.34 billion |
| Australia | USD 19.80 billion | 4.53% | 4.47% | USD 30.66 billion |

China grows 8.43% a year to USD 468.61 billion, because NRDL listings pull innovative drugs into hospitals faster than VBP cuts generic prices. Japan grows only 1.12% a year to USD 88.64 billion; the [MHLW](https://www.mhlw.go.jp/content/12400000/001676427.pdf) cut drug prices by 4.02% on a drug-spending basis from 1 April 2026.

India more than doubles from USD 26.90 billion to USD 73.94 billion at 10.64% a year, the fastest pace, from a low per-person spend and the 13.07% turnover growth the [Department of Pharmaceuticals](https://pharma-dept.gov.in/sites/default/files/Annual%20Report%202025-26_0.pdf) recorded for FY2024-25. ASEAN grows 8.94% to USD 102.65 billion. South Korea reaches USD 38.34 billion at 5.38%, and Australia USD 30.66 billion at 4.47%. The rest of Asia Pacific, led by Taiwan, Pakistan, Bangladesh and New Zealand, grows 7.79% to USD 76.00 billion.

In the data table the row labelled Global holds the Asia Pacific total, USD 436.80 billion in 2025, not world medicine sales; the world view sits in the [Pharmaceutical Drug Market](https://www.douglasinsights.com/pharmaceutical-drug-market/) study.

## How much does a unit of Asia Pacific pharmaceutical drug cost, from a VBP generic to a biologic dose?

An Asia Pacific pharmaceutical drug unit averages USD 0.233 in 2025, inside a band from about USD 0.01 for a Chinese VBP tablet to more than USD 2,000 for an oncology antibody vial. Molecule age and reimbursement status set the price, not the country alone.

| Segment | Units 2025 | Average per unit | Realised price band | Basis |
| --- | --- | --- | --- | --- |
| Generics and branded generics | 1,340.8 billion | USD 0.104 | USD 0.01 to USD 0.30 | Douglas Insights estimate |
| OTC medicines | 428.8 billion | USD 0.114 | USD 0.04 to USD 0.40 | Douglas Insights estimate |
| Branded innovative small molecules | 95.5 billion | USD 1.71 | USD 0.50 to USD 60 | Douglas Insights estimate |
| Biosimilars | 1.7 billion | USD 7.00 | USD 3 to USD 250 | Douglas Insights estimate |
| Innovative biologics | 5.8 billion | USD 12.72 | USD 5 to USD 2,500 | Douglas Insights estimate |

Biologic averages look low because insulin pens and biosimilar insulins sold under Chinese tenders make up most biologic units. Australia gives a realised benchmark: the average dispensed price per Pharmaceutical Benefits Scheme (PBS) prescription rose from AUD 85.10 to AUD 91.62 in 2024-25, per the [PBS Expenditure and Prescriptions Report](https://www.pbs.gov.au/info/statistics/expenditure-prescriptions/expenditure-prescriptions-report-1-july-2024-30-june-2025). Douglas Insights projects the regional average unit price to rise 4.29% a year, to USD 0.355 by 2035.

## Which therapy areas and channels carry Asia Pacific pharmaceutical drug sales?

Cardiovascular and metabolic medicines carry 22.1% of 2025 Asia Pacific pharmaceutical drug value, about USD 96.53 billion, and oncology 19.4%, about USD 84.74 billion. Hospital pharmacies sell 57.6% of value, because Chinese hospitals still dispense most prescriptions.

| Therapy area | Share of 2025 value | Value 2025 |
| --- | --- | --- |
| Cardiovascular and metabolic | 22.1% | USD 96.53 billion |
| Oncology | 19.4% | USD 84.74 billion |
| Anti-infectives | 12.6% | USD 55.04 billion |
| Central nervous system | 10.3% | USD 44.99 billion |
| Immunology | 7.2% | USD 31.45 billion |
| Respiratory | 5.3% | USD 23.15 billion |

The six areas hold 76.9% of value; gastrointestinal, ophthalmic and haematology products such as CSL's plasma medicines make up the rest. CSL reported FY26 revenue of USD 15.80 billion and a net loss of USD 2.58 billion, citing a fall in the market value of albumin in China, in its [18 August 2026 results](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vppn8s55bwq.pdf).

By channel, hospital pharmacy holds 57.6%, or USD 251.60 billion, retail pharmacy 35.1%, or USD 153.32 billion, and online pharmacy 7.3%, or USD 31.89 billion, on Douglas Insights estimates.

## Which rules set Asia Pacific pharmaceutical drug prices in Japan, China, India and Australia?

Four policy tracks set Asia Pacific pharmaceutical drug prices: Japan's NHI price revision, cut 4.02% on a drug-spending basis from 1 April 2026, China's NRDL and VBP, India's Production Linked Incentive (PLI) scheme and Australia's PBS.

Japan's MHLW notified the fiscal 2026 revision on 5 March 2026, a cut of 0.86% of medical expenses and 4.02% of drug spending, effective 1 April 2026, per its [revision summary](https://www.mhlw.go.jp/content/12400000/001676427.pdf). The same document records generics at 88.8% of volume and 68.7% of value in the 2025 price survey. A year earlier, the off-year revision for fiscal 2025, starting April 2025, covered about 9,320 of 17,440 items, 53%, and cut JPY 246.6 billion from the budget, per the [MHLW notice of 20 December 2024](https://www.mhlw.go.jp/content/12404000/001434754.pdf).

China's NHSA sets reimbursed prices through NRDL negotiation and sets generic prices through VBP tenders. The commercial health insurance innovative drug list of 19 drugs from 18 companies, effective 1 January 2026 under the [2025 notice](https://www.gz.gov.cn/zwfw/zxfw/ylfw/content/post_10582536.html), opens a second payer for high-priced therapies.

India's PLI scheme for pharmaceuticals carries an outlay of INR 15,000 crore from FY2020-21 to FY2028-29, with 55 selected applicants and cumulative sales of INR 3,16,797 crore by September 2025, above the INR 2,94,000 crore target, per the [Department of Pharmaceuticals](https://pharma-dept.gov.in/sites/default/files/Annual%20Report%202025-26_0.pdf). Australia's PBS listed 949 medicines across 5,164 brands at 30 June 2025, and government paid AUD 19.1 billion, 91.7% of medicine cost, per the [2024-25 PBS report](https://www.pbs.gov.au/info/statistics/expenditure-prescriptions/expenditure-prescriptions-report-1-july-2024-30-june-2025).

## What if China's NRDL listings speed up or stall: where does the Asia Pacific pharmaceutical drug forecast land in 2035?

The Asia Pacific pharmaceutical drug market ends 2035 between USD 726.02 billion and USD 1.04 trillion across our three cases, around a base of USD 878.84 billion. Chinese reimbursement of innovative drugs sets most of the spread.

| Case | Volume leg | Price leg | 2035 value |
| --- | --- | --- | --- |
| Slower | 1.94% | 3.21% | USD 726.02 billion |
| Base case | 2.83% | 4.29% | USD 878.84 billion |
| Faster | 3.58% | 5.27% | USD 1.04 trillion |

In the slower case, VBP extends to biologics, Japan keeps cutting prices every April and the commercial list launched on 1 January 2026 stays small. In the faster case, the 7 December 2025 [NRDL and commercial list](https://english.shanghai.gov.cn/en-InnovationPolicies/20251209/8ab2bcf5e66442dbbedaca79cc55a9d3.html), which admitted 88% of candidate drugs against 76% a year earlier, keeps widening, and India's PLI makers win domestic share. One extra point of yearly volume growth adds USD 89.30 billion to the 2035 value; one point less removes USD 81.82 billion.

Published forecasts for Asia Pacific medicines run from about 6.3% to 7.2% a year. Our 7.24% sits at the top edge, because we price units at manufacturer selling prices and let the shift to innovative biologics carry the price leg.

## Douglas Exclusive: the Asia Pacific Drug Price Reset Calendar

The Asia Pacific Drug Price Reset Calendar is a Douglas Insights model built from 6 sourced inputs: four government notices from Japan and China, the Indian Department of Pharmaceuticals annual report and one CSL filing. The calendar dates each pricing or reimbursement reset and tags its direction.

| Date | Reset | Effect on value |
| --- | --- | --- |
| 20 December 2024 | MHLW agrees fiscal 2025 off-year revision, JPY 246.6 billion | Down, Japan |
| April 2025 | Fiscal 2025 revision applies to 9,320 items | Down, Japan |
| 15 July 2025 | 11th VBP round sets rules for 55 generics | Down per unit, up in volume, China |
| September 2025 | PLI sales reach INR 3,16,797 crore | Up, India |
| 7 December 2025 | NRDL adds 114 drugs; commercial list of 19 | Up, China |
| 31 December 2025 | 17,990 Jan Aushadhi stores open | Up in volume, India |
| 1 January 2026 | NRDL and commercial list take effect | Up, China |
| 5 March 2026 | MHLW notifies fiscal 2026 cut of 4.02% | Down, Japan |
| 1 April 2026 | Fiscal 2026 NHI prices apply | Down, Japan |
| 18 August 2026 | CSL cites albumin value fall in China | Down, China plasma |

Four of the ten resets cut Japanese prices, while four lift Chinese and Indian access. The finding: Douglas Insights counts about USD 4.83 billion of annual value removed by the two Japanese revisions, JPY 246.6 billion in fiscal 2025 plus 4.02% of our USD 79.30 billion Japan base in fiscal 2026, against USD 17.59 billion of value China adds in 2026 alone. A launch team choosing where to file first in Asia now gains more from an NRDL listing than from a Japanese premium price.

## How does the methodology turn 1,872.6 billion units into USD 436.80 billion of Asia Pacific pharmaceutical drugs?

Our methodology multiplies 1,872.6 billion standard units of 2025 Asia Pacific pharmaceutical drug consumption by an average USD 0.233 per unit to reach USD 436.80 billion. Units come from seven country and sub-region builds; price comes from five segment bands weighted by units.

The model covers 7 country and sub-region rows spanning 24 countries and uses 86 data points, 27 of them sourced and 59 modelled. Arithmetic: 1,872.6 billion x USD 0.23326 = USD 436.80 billion; 436,800 x (1.0283 x 1.0429)^10 = 878,836, or USD 878.84 billion. Units reach 2,475.4 billion in 2035. Currency conversions use 2025 averages of JPY 150, CNY 7.19, INR 87, KRW 1,420 and USD 0.645 per AUD.

Two cross-checks agree. First, Australian PBS spending of AUD 19.1 billion by government and AUD 1.7 billion by patients, about USD 13.42 billion, equals 67.8% of our USD 19.80 billion Australia value, leaving hospital, private and OTC sales for the rest, per the [PBS report](https://www.pbs.gov.au/info/statistics/expenditure-prescriptions/expenditure-prescriptions-report-1-july-2024-30-june-2025). Second, Indian turnover of INR 4,71,898 crore less exports of INR 2,45,962 crore leaves about INR 2,25,936 crore, or USD 25.97 billion, 96.5% of our USD 26.90 billion India value, per the [Department of Pharmaceuticals](https://pharma-dept.gov.in/sites/default/files/Annual%20Report%202025-26_0.pdf).

## Which medicines count in the Asia Pacific pharmaceutical drug market, and what is excluded?

The Asia Pacific pharmaceutical drug market counts finished prescription and OTC medicines consumed in the region, valued at manufacturer selling prices after statutory discounts. Vaccines, active pharmaceutical ingredients (APIs) sold for export, dietary supplements, medical devices and pharmacy margins stay out.

Indian exports, INR 2,45,962 crore in FY2024-25, count in the buying country. Contract manufacturing revenue stays out; our [Biologics Outsourcing Market](https://www.douglasinsights.com/biologics-outsourcing-market/) study sizes it, and the [Therapeutic Monoclonal Antibodies Market](https://www.douglasinsights.com/therapeutic-monoclonal-antibodies-market/) report follows antibody medicines worldwide. The by-therapy split covers oncology, cardiovascular and metabolic, anti-infectives, central nervous system, immunology and respiratory medicines, and the channel split covers hospital pharmacy, retail pharmacy and online pharmacy.
