## How much do providers pay per CPAP mask cushion and full face frame?

USD 62.40 is our blended realised manufacturer price per CPAP mask unit in 2025, averaged across complete masks and replacement parts. Complete full face masks realise USD 88 to USD 112, nasal masks USD 64 to USD 82 and cushion kits USD 17 to USD 29, because providers buy direct or through home medical equipment (HME) distributors. These bands are Douglas Insights estimates.

Retail shelf prices sit well above those factory bands. A Löwenstein Medical JOYCEone full face mask lists at EUR 84.51 at a German online retailer, and a BMC N5A+ nasal mask starts at GBP 79.00 at a UK retailer. The spread between a EUR 84.51 shelf tag and a USD 62.40 blended factory price is the distributor, fitting and returns margin.

Douglas Insights expects the price leg to add only 0.58% a year through 2035, lifting the average to USD 66.12. Mask pricing barely moves. In our model each new frame carries a premium of 8% to 12% over the one it replaces, while older frames drift down as they age, and the two effects almost cancel. Replacement cushions, about 31.5% of units in our build, carry the lowest price points and the most stable volume.

Full face masks hold USD 1.12 billion because they suit the widest range of pressures and breathing habits, and their 4.92% growth trails the market as patients step down to lighter designs once settled. Nasal masks carry USD 1.00 billion, a 36.8% share, since under-the-nose cushions such as the one on the React Health Rio II suit patients who breathe through the nose; they grow at 5.61%. Nasal pillow masks are worth USD 478.8 million and reach USD 927.9 million by 2035, the fastest line at 6.84%, because their small silicone parts are replaced most often. Oral and hybrid masks are a USD 117.0 million niche growing 4.37% a year, held back by narrow clinical indications.

## Why are CPAP mask resupply volumes still climbing?

CPAP mask unit volume grows 4.86% a year in our base case because diagnosis widens, resupply cadence tightens and new frames pull patients back to providers. Four drivers supply those points: home sleep testing 2.07, resupply 1.48, product launches 0.86 and device-start recovery 0.45, all net of the restraints below.

### Diagnosis is the largest lever for CPAP masks

Home sleep testing adds 2.07 points a year to CPAP mask volume. A literature-based analysis published in a Lancet journal counted 936 million people with OSA worldwide, and ResMed says more than 85% of sleep apnea patients remain undiagnosed. Every newly diagnosed patient who starts therapy buys a first mask and then joins the resupply base, so a one-point rise in the diagnosed share compounds for years. Douglas Insights estimates 28.4 million patients on PAP therapy worldwide in 2025, of whom 61% are on an active resupply schedule.

### Replacement schedules turn compliance into CPAP mask volume

Resupply discipline adds 1.48 points. The Medicare [local coverage determination (LCD) L33718](https://www.cms.gov/medicare-coverage-database/view/lcd.aspx?lcdid=33718) from the Centers for Medicare and Medicaid Services (CMS) treats one full face mask per 3 months, one nasal mask per 3 months and two cushions or pillows per month as the usual maximum. Automated resupply programmes call or message patients as each window opens, and our model assumes realised replacement rising from 2.6 to 3.1 full face masks per US patient-year by 2035, still below the four allowed.

### New frames and the Philips gap keep CPAP mask demand fresh

Product launches add 0.86 points. Fisher and Paykel Healthcare launched the F&P Nova Nasal in the United States in January 2026, and its OSA masks revenue rose 7%, or 5% in constant currency, in the year to March 2026, the company said in its [results of 26 May 2026](https://www.fphcare.com/nz/corporate/investor/news/fph-reports-strong-revenue-and-profit-growth-2026/). ResMed reported masks and other revenue of USD 1.51 billion in the Americas, up 13%, and USD 572 million elsewhere, up 15%, in the fiscal year to June 2026, according to its [fiscal 2026 results filed on August 6, 2026](https://www.sec.gov/Archives/edgar/data/0000943819/000119312526337962/d158732dex991.htm). The same release names the AirTouch F30i Comfort launch in Brazil and Chile.

The last 0.45 points come from new device starts that shifted after Philips Respironics stopped selling new sleep devices in the US. Those patients still need a first CPAP mask, and most now arrive on a competitor's flow generator with a matching interface. Douglas Insights estimates that 0.45 points fades to near zero by 2029 as the shift completes, and the tirzepatide approval is the main reason our net volume leg is not higher.

## Which headwinds slow CPAP mask volume as tirzepatide spreads?

Three headwinds remove 1.21 points a year from an unconstrained 6.07% CPAP mask volume path: tirzepatide and other weight-loss drugs take 0.74 points, therapy abandonment 0.31 and magnet-related labelling and switching friction 0.16. Each is modelled as a cut to patient-years on therapy, not to price.

Tirzepatide is the largest drag on CPAP mask volume. The Eli Lilly drug Zepbound reduced breathing interruptions by about 25 an hour against 5 for placebo in patients not on PAP, and up to 50% of treated adults no longer had OSA symptoms after one year, Lilly reported. Douglas Insights removes 0.74 points from mask volume for 2026 to 2035, assuming lower new-start rates in obese patients rather than mass exits from therapy, since PAP users in the same trials lost 20% of body weight on the drug while still on PAP.

Abandonment removes 0.31 points. A patient who stops using a CPAP mask stops ordering cushions, and our model assumes 17% of new starts quit within the first year. Coaching apps and better first-fit masks have narrowed that gap, but in our estimate the loss still costs the market about 1.9 million mask units a year by 2030.

Magnet labelling removes 0.16 points. The US Food and Drug Administration (FDA) classed a [ResMed correction of masks with magnets as a Class I recall on January 18, 2024](https://www.fda.gov/medical-devices/safety-communications/certain-resmed-ltd-masks-bipap-cpap-machines-recalled-due-safety-issue-magnets-may-affect-certain), after six injuries, and told users with certain metallic implants to keep the masks at least six inches away or switch to non-magnetic models. Clip redesigns add fitting time and slow some replacement orders.

ResMed's share rests on scale. Its masks and other revenue reached USD 1.51 billion in the Americas in fiscal 2026, and Douglas Insights assumes about 74% of that line is masks and interface parts, the rest being accessories and tubing. Philips Respironics keeps 11.6% because the consent decree of April 10, 2024 still lets it [sell accessories including masks, consumables and replacement parts](https://www.usa.philips.com/about/news-and-insights/press-release/philips-respironics-reaches-agreement-with-us-government-on-a-consent-decree-creating-a-clear-path-forward) to patients already on its devices, even though it stopped selling new CPAP devices in the US. Fisher and Paykel Healthcare's 10.2% leans on the Nova and Solo frames, which the company says continued to drive OSA mask growth. React Health sells the Rio II nasal mask with a 360-degree hose swivel and a 20 dB vent. Löwenstein Medical anchors the JOYCE range in Europe. BMC Medical competes on price with the N5A+, sold in four sizes plus a fit pack.

The wildcard is the Middle East and Africa: only USD 87.1 million today, but its 6.82% rate in our model rests on private sleep clinics in the Gulf, and a single national resupply scheme would move it sharply.

## Which rules govern CPAP mask magnets and replacement schedules?

Two US rule sets shape CPAP mask revenue most: the CMS replacement maximums of 1 full face mask per 3 months and 2 cushions per month, and FDA recall labelling for masks with magnets. Together they set how often 52.4% of global mask value gets replaced and which clip designs sell.

Replacement maximums are the revenue rule. Quantities above the usual maximum are denied as not reasonable and necessary under L33718, so US resupply programmes schedule orders to land on each window. Headgear is limited to one per 6 months and tubing to one per 3 months under the same policy. Magnet rules are the design rule. After the Class I designation, the FDA said ResMed was making replacement masks without magnets available to mask providers, which turned clip design into a buying criterion. Philips Respironics works under its consent decree with the US Department of Justice and FDA, which governs when it can resume new device sales but leaves CPAP mask resupply open.

## What if tirzepatide shrinks the CPAP mask forecast for 2035?

Our base case reaches USD 4.63 billion in 2035, at 4.86% volume and 0.58% price growth. A slower case, where tirzepatide cuts new OSA starts harder, gives USD 3.80 billion at 3.21% volume and 0.18% price. A faster case, with wider home sleep testing, gives USD 5.58 billion at 6.42% volume and 0.97% price.

Douglas Insights puts the value of one extra point of CPAP mask volume growth at USD 461 million in 2035. The slower case assumes tirzepatide and similar drugs reach a quarter of obese OSA patients by 2030 and that resupply cadence stalls at 2.6 full face masks per US patient-year. The faster case assumes national screening in China and India and US replacement realised at 3.4 masks per patient-year. Our 5.47% base CAGR sits just under published forecasts, which run from about 5.5% to 6.7% a year, because we deduct the weight-loss drug effect explicitly.

It sets one published payer rule, the US allowance under L33718, against our own estimates of how many CPAP mask items a compliant patient actually receives in each region, so buyers can see where replacement volume may be unclaimed.

The atlas compares allowed and realised full face mask replacements per patient-year. Only the US figure for allowed replacement comes straight from a published payer rule, four a year under L33718; every realised figure is our estimate. The model result is a large unclaimed gap. Our estimate puts realised US replacement at 2.6 full face masks per patient-year, so 9.8 million US resupply-active patients leave about 13.7 million allowed mask units unshipped each year. Closing a quarter of that gap would add 3.43 million units, worth about USD 214 million at today's price.
