## Which dry eye therapy earns the most: artificial tears, anti-inflammatory drops or new tear-film drugs?

Over-the-counter artificial tears earn the most, 44.0% of 2025 revenue or about USD 3.17 billion, Douglas Insights estimates. Tear-film and new-mechanism prescription drugs grow fastest, 12.0% a year to USD 2.68 billion by 2035.

| Therapy type | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Over-the-counter artificial tears and lubricants | 44.0% | USD 3.17 billion | 4.0% | USD 4.69 billion |
| Prescription anti-inflammatory drops (cyclosporine, lifitegrast, steroids) | 30.0% | USD 2.16 billion | 3.0% | USD 2.90 billion |
| Tear-film and new-mechanism prescription drugs | 12.0% | USD 864 million | 12.0% | USD 2.68 billion |
| MGD procedures and devices | 10.0% | USD 720 million | 7.0% | USD 1.42 billion |
| Other therapies (autologous serum, punctal plugs, supplements) | 4.0% | USD 288 million | 4.0% | USD 426 million |

Artificial tears lead because almost every patient starts with them and many never move beyond them. Alcon says about 38 million Americans have dry eye disease and fewer than 10% of diagnosed patients use a prescription product, its [release of 28 May 2025](https://www.sec.gov/Archives/edgar/data/0001167379/000116737925000023/tryptyrapprovalpressrelease.htm) states. Bausch + Lomb named over-the-counter dry eye products as a Vision Care growth driver in its [29 July 2026 results](https://www.sec.gov/Archives/edgar/data/0001860742/000186074226000086/ex991q2-2026.htm). Tears grow 4.0% a year to USD 4.69 billion, a Douglas Insights estimate, held back by low prices per bottle.

Anti-inflammatory drops, USD 2.16 billion, grow only 3.0% because generic cyclosporine cuts the price of the largest molecule. Tear-film and new-mechanism drugs, USD 864 million, include perfluorohexyloctane (Miebo), acoltremon (Tryptyr) and varenicline nasal spray; Miebo alone sold USD 112 million in the fourth quarter of 2025, Bausch + Lomb's [results of 18 February 2026](https://www.businesswire.com/news/home/20260218111553/en/Bausch-Lomb-Announces-Fourth-Quarter-and-Full-Year-2025-Results-Provides-2026-Guidance) show. MGD procedures, USD 720 million, grow 7.0%. Therapy values for 2035 sum to USD 12.12 billion, 1.12% above the headline.

## Where do dry eye patients buy treatment: retail pharmacy, eye clinic or online?

Retail pharmacies sell most dry eye treatment, 58.0% of 2025 revenue or about USD 4.18 billion, Douglas Insights estimates. Online channels grow fastest, 7.6% a year to USD 3.00 billion by 2035, on repeat orders of tears.

| Channel | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Retail and chain pharmacies | 58.0% | USD 4.18 billion | 4.6% | USD 6.55 billion |
| Hospital pharmacies and eye clinics | 22.0% | USD 1.58 billion | 5.2% | USD 2.63 billion |
| Online pharmacies and e-commerce | 20.0% | USD 1.44 billion | 7.6% | USD 3.00 billion |

Retail pharmacies lead because both prescription drops and shelf tears pass through them, and because pharmacists handle the generic switch at the counter. Their share slips from 58.0% to about 54% by 2035, a Douglas Insights estimate, as subscription buying of preservative-free tears moves online. Hospital pharmacies and eye clinics, USD 1.58 billion, carry procedures and specialist drops and grow 5.2% a year to USD 2.63 billion.

Online channels, USD 1.44 billion, grow 7.6% because a chronic patient reorders the same lubricant every month and because direct-to-patient pharmacies now ship branded prescriptions. Harrow, which sells Vevye cyclosporine 0.1%, grew Vevye revenue 216% to USD 88.7 million in 2025 from USD 28.1 million, its [results of 2 March 2026](https://harrowinc.gcs-web.com/news-releases/news-release-details/harrow-announces-q4-and-full-year-2025-financial-results-and) show, helped by a cash-price pharmacy model. Channel values for 2035 sum to USD 12.17 billion, 1.57% above the headline.

## What drives dry eye treatment demand: new prescription drugs, ageing eyes or Asian launches?

Dry eye treatment revenue rises 5.23% a year in our base case: treated patients grow 4.6% a year to about 185 million and average spend rises 0.6%. Four drivers add 5.80 points; restraints remove 1.20.

### New prescription mechanisms pulling patients off tears: 2.40 points

New drugs are the largest driver, worth 2.40 points. The US Food and Drug Administration (FDA) approved Alcon's Tryptyr (acoltremon 0.003%) on 28 May 2025 after two Phase 3 trials, COMET-2 and COMET-3, in more than 930 patients; 42.6% and 53.2% of treated patients gained at least 10 mm of tear production by Day 14, against 8.2% and 14.4% on vehicle, Alcon's [28 May 2025 release](https://www.sec.gov/Archives/edgar/data/0001167379/000116737925000023/tryptyrapprovalpressrelease.htm) states. Alcon's Ocular Health sales then rose 13% to USD 486 million in the second quarter of 2026, "led by" dry eye products including Tryptyr and Systane, its [results of 10 August 2026](https://www.alcon.com/media-release/new-product-launches-and-strong-commercial-execution-drive-alcons-second-quarter-2026/) show. Douglas Insights estimates prescription penetration of diagnosed US patients rises from about 9% in 2025 to about 15% in 2035.

### Ageing populations, screen use and contact lens wear: 1.60 points

Prevalence adds 1.60 points. The Tear Film and Ocular Surface Society (TFOS) DEWS III report, published in the American Journal of Ophthalmology in 2025, puts dry eye prevalence at 5.4% to 44.2% under DEWS II criteria, 7.3% to 31.6% by symptoms alone and MGD prevalence at 35.8% worldwide, and finds higher rates in Asian than in Western populations, its [digest](https://publications.aston.ac.uk/id/eprint/47668/3/TFOS_DEWS_III_Digest.pdf) shows. Douglas Insights estimates the number of people aged 65 and over with symptomatic dry eye rises about 3% a year to 2035, faster than the total population.

### Launches in China and other Asian markets: 1.20 points

Asia adds 1.20 points. Santen launched Verkazia, its cyclosporine for severe vernal keratoconjunctivitis, in China on 23 December 2025 and resumed shipments of Diquas LX 3% on 1 December 2025, its [news page](https://www.santen.com/en/news/2025/2025_3) lists. China's National Medical Products Administration (NMPA) approved Heng Yi, a water-free cyclosporine 0.1% drop from Hengrui and Novaliq, on 23 June 2026, [Optometry Times reported](https://www.optometrytimes.com/view/heng-yi-cyclosporine-drop-approved-in-china). Douglas Insights estimates China's treated patient pool grows about 7% a year.

### In-office MGD procedures and lipid therapies: 0.60 points

MGD care adds the last 0.60 points. Azura Ophthalmics said on 10 December 2025 that the FDA agreed its existing data are enough to file a new drug application (NDA) for AZR-MD-001 in MGD, with the filing targeted for the second half of 2026, its [release](https://www.businesswire.com/news/home/20251210553083/en) states. Thermal pulsation and intense pulsed light sessions add procedure revenue at eye clinics. Summed, 2.40 + 1.60 + 1.20 + 0.60 = 5.80 points, a Douglas Insights estimate.

## What holds dry eye treatment back: generic cyclosporine, failed trials or contaminated drops?

Two restraints remove 1.20 points a year from treated-patient growth: payer step therapy and discontinuation take 0.70 points, and lost trust after contaminated-drop recalls takes 0.50 points; generic cyclosporine then caps the spend leg at 0.6%.

Discontinuation is the heavier drag because many patients stop a prescription drop within months when relief is slow or co-pays are high, and insurers often require a trial of tears or generic cyclosporine first. AbbVie's Other Eye Care revenue, which holds Restasis with Refresh and Optive tears, fell to USD 1.01 billion in 2025 from USD 1.07 billion in 2024 and USD 1.24 billion in 2023, its [2025 annual report](https://www.sec.gov/Archives/edgar/data/1551152/000155115226000008/abbv-20251231.htm) shows. Douglas Insights estimates about half of new prescription starts lapse within a year.

Failed trials narrow the pipeline. The FDA issued a complete response letter for Aldeyra's reproxalap in dry eye on 17 March 2026, citing a lack of substantial evidence of effectiveness, and raised no safety or manufacturing issue, Aldeyra's [filing of 17 March 2026](https://www.sec.gov/Archives/edgar/data/1341235/000119312526109511/aldx-20260317.htm) states; the company counts it as the second such letter for that program. Repeated over-the-counter recalls for sterility failures since 2023 have also pushed some patients away from store-brand tears, a Douglas Insights estimate, which shifts volume to branded preservative-free products but slows first-line uptake.

## Which region spends the most on dry eye treatment, and where does it grow fastest?

North America spends the most on dry eye treatment, USD 3.02 billion or 42.0% of 2025 revenue, led by branded US prescriptions. Asia Pacific grows fastest, 6.4% a year to USD 3.75 billion by 2035.

| Region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| North America | USD 3.02 billion | 42.0% | USD 3.17 billion | 4.9% | USD 4.88 billion |
| Asia Pacific | USD 2.02 billion | 28.0% | USD 2.15 billion | 6.4% | USD 3.75 billion |
| Europe | USD 1.51 billion | 21.0% | USD 1.57 billion | 4.1% | USD 2.26 billion |
| Latin America | USD 360 million | 5.0% | USD 380 million | 5.6% | USD 621 million |
| Middle East and Africa | USD 288 million | 4.0% | USD 305 million | 6.0% | USD 516 million |

North America leads because branded drops cost several hundred dollars a month at list price and because the United States has the most new-mechanism launches. It grows 4.9% a year to USD 4.88 billion as Tryptyr and Miebo gain prescriptions while generic cyclosporine pulls the price of the largest molecule down. US sales of single-use cyclosporine 0.05% vials were about USD 2.0 billion in the 12 months to September 2025, IQVIA data cited in Amneal's [release of 1 December 2025](https://www.biospace.com/press-releases/amneal-announces-u-s-fda-approval-of-cyclosporine-ophthalmic-emulsion-0-05) show, a gross figure before rebates.

Asia Pacific, USD 2.02 billion, grows 6.4% on high prevalence, heavy screen use in China, Japan and South Korea, and new cyclosporine launches. Europe, USD 1.51 billion, grows 4.1% under tight reimbursement; Novaliq and Laboratoires Théa won EU approval for Vevizye ciclosporin 0.1% on [2 October 2024](https://www.businesswire.com/news/home/20241002958802/en/Novaliq-and-Laboratoires-Tha-Announce-Partnership-and-EU-Approval-for-Vevizye-Ciclosporin-0.1-Eye-Drops-Solution). Latin America, USD 360 million, grows 5.6%, and the Middle East and Africa, USD 288 million, grow 6.0%. Regional values sum to USD 7.58 billion in 2026 and USD 12.02 billion in 2035, 0.33% above the headline.

## Which companies lead dry eye syndrome treatment?

Bausch + Lomb leads dry eye treatment with about 15% of 2025 revenue, Douglas Insights estimates, and the five largest, Bausch + Lomb, AbbVie, Alcon, Santen and Harrow, hold about 45% together; private-label tears hold much of the rest.

| Company | Latest reported figure | Period and source date |
| --- | --- | --- |
| Bausch + Lomb | Pharmaceuticals revenue USD 367 million, up 11% | Quarter to September 2026; 28 October 2026 |
| Bausch + Lomb | Dry eye portfolio about USD 1.1 billion | Year 2025; 18 February 2026 |
| AbbVie | Other Eye Care revenue USD 1.01 billion | Year 2025; 2025 annual report |
| Alcon | Ocular Health USD 486 million, up 13% | Quarter to June 2026; 10 August 2026 |
| Harrow | Vevye revenue USD 29.4 million | Quarter to June 2026; 10 August 2026 |
| Amneal | Generic cyclosporine 0.05% approved | 1 December 2025 |
| Aldeyra Therapeutics | Reproxalap complete response letter | 17 March 2026 |

Bausch + Lomb owns the broadest franchise, from Xiidra lifitegrast and Miebo to Artelac tears, and it put its dry eye portfolio at about USD 1.1 billion of 2025 revenue within total revenue of USD 5.10 billion, up 6%, its [18 February 2026 release](https://www.businesswire.com/news/home/20260218111553/en/Bausch-Lomb-Announces-Fourth-Quarter-and-Full-Year-2025-Results-Provides-2026-Guidance) shows. Its pharmaceuticals segment grew 15% to USD 354 million in the second quarter of 2026 and 11% to USD 367 million in the third, its [28 October 2026 results](https://www.sec.gov/Archives/edgar/data/0000885590/000088559026000121/ex991-q32026.htm) show, where total revenue reached USD 1.38 billion, up 7%.

AbbVie holds Restasis and the Refresh tear line, but no longer reports Restasis on its own; its Other Eye Care line earned USD 588 million in the United States and USD 421 million abroad in 2025, its [annual report](https://www.sec.gov/Archives/edgar/data/1551152/000155115226000008/abbv-20251231.htm) shows. Alcon competes with Systane tears and Tryptyr, and its total second-quarter 2026 sales rose 8% to USD 2.78 billion, its [10 August 2026 release](https://www.alcon.com/media-release/new-product-launches-and-strong-commercial-execution-drive-alcons-second-quarter-2026/) states.

Harrow is the fastest-growing branded challenger: its total 2025 revenue rose 36% to USD 272.3 million and it guides USD 350 million to USD 365 million for 2026, its [2 March 2026 results](https://harrowinc.gcs-web.com/news-releases/news-release-details/harrow-announces-q4-and-full-year-2025-financial-results-and) show, and Vevye sold USD 29.4 million in the second quarter of 2026, its [10 August 2026 results](https://investors.harrow.com/node/14366/pdf) show. Santen leads in Japan with Diquas and Hyalein, Sun Pharma sells Cequa in the United States, and Viatris markets Tyrvaya nasal spray.

## How much does dry eye treatment cost a patient each year, from a bottle of tears to a branded prescription?

Dry eye treatment costs an average USD 61 per treated patient a year in 2025, Douglas Insights estimates, from about USD 40 for occasional tears to several thousand dollars a year at list price for a branded US prescription.

The average is low because most patients only buy tears. Douglas Insights puts the yearly price ladder at about USD 40 to USD 120 for over-the-counter lubricants, USD 150 to USD 400 for preservative-free and lipid tears used daily, USD 500 to USD 1,500 net for generic cyclosporine, and USD 3,000 to USD 8,000 at list price for branded US prescription drops before rebates. One thermal pulsation session costs about USD 500 to USD 1,500 out of pocket, and it is rarely reimbursed.

The spend leg rises only 0.6% a year in the base case. Generic entry is the main brake: Amneal's FDA approval of a generic cyclosporine ophthalmic emulsion 0.05%, with launch expected in the first quarter of 2026, opened the largest US molecule to more price competition, its [1 December 2025 release](https://www.biospace.com/press-releases/amneal-announces-u-s-fda-approval-of-cyclosporine-ophthalmic-emulsion-0-05) states. Mix lifts the leg in the other direction, as each patient moved to Tryptyr, Miebo or Vevye spends ten times or more what a tear user spends, a Douglas Insights estimate. Cash-price pharmacy programmes cap list prices for uninsured buyers but widen access.

Asia lowers the global average. A Chinese hospital patient pays far less per bottle of sodium hyaluronate or cyclosporine than a US patient, and national price negotiation keeps it there, a Douglas Insights estimate. Douglas Insights therefore holds the global spend per patient near USD 65 by 2035 even as US branded prices climb.

## What if new drugs stall or launches beat plan: how large is dry eye treatment in 2035?

The dry eye syndrome treatment market ends 2035 between USD 9.67 billion and USD 14.81 billion across our three cases, around a base of USD 11.98 billion. Prescription uptake and generic price erosion set most of the spread.

| Case | Volume leg | Price leg | Revenue rate | 2035 value |
| --- | --- | --- | --- | --- |
| Slower | 3.10% | -0.10% | 3.00% | USD 9.67 billion |
| Base case | 4.60% | 0.60% | 5.23% | USD 11.98 billion |
| Faster | 6.10% | 1.30% | 7.48% | USD 14.81 billion |

In the slower case new-mechanism drugs win few patients beyond early adopters, more candidates fail like reproxalap, and generic cyclosporine and lifitegrast cut branded prices faster, taking 1.5 points off treated-patient growth and 0.7 points off spend. In the faster case China and India adopt prescription drops quickly and US prescription penetration reaches about 20%, adding 1.5 points of volume. One extra point of yearly revenue growth adds about USD 1.2 billion to 2035 revenue, a Douglas Insights estimate.

Live signals sit near the base case. Bausch + Lomb's 11% pharmaceuticals growth and Alcon's 13% Ocular Health growth point up, while AbbVie's falling Other Eye Care line and the generic cyclosporine launch point down. Our 5.23% rate reflects steady patient growth with little net price gain.

## Which rules shape dry eye treatment, from FDA approvals to China's drug regulator?

Three rule sets shape dry eye treatment: FDA new drug and generic approvals in the United States, NMPA approvals and price talks in China, and the European Medicines Agency (EMA) and national health technology assessment in Europe.

The FDA sets the pace of innovation. It approved Tryptyr on 28 May 2025, approved generic cyclosporine 0.05% from Amneal by 1 December 2025, and rejected reproxalap on 17 March 2026 for lack of substantial evidence, the company filings above show. The FDA's evidence bar for dry eye is demanding because signs and symptoms often fail to move together in trials, and Aldeyra's [17 March 2026 filing](https://www.sec.gov/Archives/edgar/data/1341235/000119312526109511/aldx-20260317.htm) says it plans a Type A meeting with the agency. FDA good manufacturing practice (GMP) inspections of sterile ophthalmic plants also matter, because over-the-counter tears are made under drug rules in the United States.

China's regulator now approves new formulations faster. The NMPA approval of Heng Yi on 23 June 2026, [Optometry Times reported](https://www.optometrytimes.com/view/heng-yi-cyclosporine-drop-approved-in-china), added a second cyclosporine option for Chinese hospitals. In Europe, EMA approval is followed by country-by-country pricing, which delays launches; the TFOS DEWS III [executive summary](https://pubmed.ncbi.nlm.nih.gov/41005521/), published online on 24 September 2025, gives regulators and payers an updated diagnostic definition to judge new claims against.

## Douglas Exclusive: the Prescription Conversion Gap

The Prescription Conversion Gap is a Douglas Insights model built from 11 inputs: 9 company filings and releases and 2 peer-reviewed society reports, listed below. It measures how many diagnosed patients still use only tears.

| Date | Input | What it moves |
| --- | --- | --- |
| 2 October 2024 | Vevizye ciclosporin 0.1% EU approval | Europe prescription base |
| 28 May 2025 | Alcon: Tryptyr approved; fewer than 10% use prescriptions | Conversion rate |
| 24 September 2025 | TFOS DEWS III executive summary | Diagnosis definition |
| 2025 | TFOS DEWS III digest: MGD prevalence 35.8% | Patient pool |
| 1 December 2025 | Amneal generic cyclosporine 0.05% approved | Spend per patient |
| 10 December 2025 | Azura AZR-MD-001 NDA path agreed | MGD pipeline |
| 18 February 2026 | Bausch + Lomb dry eye portfolio about USD 1.1 billion | Branded revenue |
| 2 March 2026 | Harrow: Vevye USD 88.7 million in 2025 | Challenger uptake |
| 17 March 2026 | Aldeyra reproxalap complete response letter | Pipeline risk |
| 10 August 2026 | Alcon Ocular Health up 13% | New-mechanism uptake |
| 28 October 2026 | Bausch + Lomb pharmaceuticals up 11% | Branded growth |

The finding, a Douglas Insights estimate: about 91% of diagnosed US patients used no prescription product in 2025, and that gap narrows to about 85% by 2035. Each point of conversion adds about USD 110 million of US revenue at 2025 spend rates, which is why the market grows faster than patient numbers.

The gap changes how to read company results. Fast growth at Bausch + Lomb, Alcon and Harrow does not mean the whole market is booming; it means a small share of patients is trading up while most stay on cheap tears. Douglas Insights therefore weights conversion rates as heavily as prevalence in the forecast.

## How does the methodology turn 118 million treated patients into USD 7.20 billion?

Our methodology multiplies about 118 million treated dry eye patients by an average annual spend of USD 61 to reach USD 7.20 billion, then grows patients 4.6% and spend 0.6% a year.

The model covers 5 therapy types, 3 channels and 5 regions, using 58 data points, 21 sourced and 37 modelled. The patient base starts from TFOS DEWS III prevalence ranges applied to adult populations, less undiagnosed and untreated people; spend comes from company revenue, the IQVIA figure in Amneal's release and a Douglas Insights price ladder. Arithmetic: 118 million x USD 61 = USD 7.20 billion; USD 7.20 billion x (1.046 x 1.006)^10 = USD 11.98 billion. Treated patients reach about 185 million in 2035.

Two cross-checks agree. First, the reported or estimated dry eye revenue of Bausch + Lomb, AbbVie, Alcon, Santen and Harrow sums to about USD 3.2 billion, about 45% of our total, matching our top-five share. Second, Harrow's Vevye at USD 88.7 million is about 4% of our anti-inflammatory segment, consistent with a challenger brand in its second full year. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our [editorial standards](https://www.douglasinsights.com/research-methodology/) explain.
