## Key takeaways

- The India pharmaceutical drug market is worth USD 26.9 billion in 2025 and USD 73.9 billion in 2035, a 10.64% revenue CAGR.
- Volume adds 5.80% a year and price per unit 4.57%, mostly from mix as patients move to chronic combinations, pens and biologics.
- Branded generics lead with 64.5%, while biologics and biosimilars grow fastest at 15.10% a year.
- South India spends USD 7.26 billion, 27.0% of value; East and Northeast India grows fastest at 12.40% a year.
- Sun Pharma, Abbott India and Mankind Pharma hold only 19.0%, and semaglutide's 2026 patent expiry showed price cuts growing value.

Semaglutide lost patent protection in India on 21 March 2026, and within weeks nearly 40 Indian drug makers were selling copies at about Rs 2,000 to Rs 3,500 a month against Rs 11,000 or more for the originator, [The South First reported](https://thesouthfirst.com/bold/in-india-generic-competition-slashes-semaglutide-prices-after-patent-expiry-sales-jump-nearly-sevenfold/); monthly packs sold rose from about 25,000 in February to about 170,000 in April. Novo Nordisk answered on 31 March 2026 with cuts of up to 48%, [Business Today reported](https://www.businesstoday.in/amp/industry/pharma/story/novo-further-cuts-semaglutide-prices-as-generics-enter-india-523336-2026-03-31/). The India pharmaceutical drug market covers finished prescription and over-the-counter medicines sold to patients, pharmacies and hospitals inside India, valued at manufacturer invoice prices; exports are excluded. Douglas Insights sizes it at USD 26.9 billion in 2025, from 410 billion standard units at USD 0.0656 (about Rs 5.72) each, and at USD 73.9 billion in 2035, a revenue CAGR (compound annual growth rate) of 10.64%. The study sits in our [healthcare and life sciences coverage](https://www.douglasinsights.com/industry/healthcare/) and every figure follows the Douglas Insights [research methodology](https://www.douglasinsights.com/research-methodology/).

## Which companies lead the India pharmaceutical drug market, and how fragmented is it?

Sun Pharmaceutical Industries leads the India pharmaceutical drug market with about 8.2% of 2025 value, USD 2.21 billion, Douglas Insights estimates. With Abbott India and Mankind Pharma, the top three hold only 19.0%.

| Company | What the position rests on | Estimated share 2025 | Estimated value 2025 |
| --- | --- | --- | --- |
| Sun Pharmaceutical Industries | Chronic brands in cardiology, neurology and dermatology; Rs 19,290 crore of FY26 India formulation sales | 8.2% | USD 2.21 billion |
| Abbott India and Abbott Healthcare | Thyronorm, Duphaston, Udiliv; Novo Nordisk diabetes distribution partner | 5.6% | USD 1.51 billion |
| Mankind Pharma | Acute and chronic brands sold in smaller towns; Rs 12,217 crore of FY26 domestic revenue | 5.2% | USD 1.40 billion |
| Cipla | Respiratory leadership, trade generics, consumer health | 4.8% | USD 1.29 billion |
| Torrent Pharmaceuticals | Cardiac and gastro brands, JB Chemicals added from 21 January 2026; Rs 7,645 crore of FY26 India revenue | 3.3% | USD 877.3 million |
| Other companies | Alkem, Lupin, Intas, Zydus Lifesciences, Dr. Reddy's, Emcure and smaller makers among India's 10,500 drug plants | 72.9% | USD 19.6 billion |

The filed figures come from company releases. Sun Pharma reported 14% growth in India formulations and a market share rising from 8.1% to 8.4% in its [FY26 results of 22 May 2026](https://sunpharma.com/wp-content/uploads/2026/05/Press-Release-Sun-Pharma-Q4FY26-Financial-Result.pdf). Mankind Pharma reported domestic revenue up 14.4% and a 15.1% prescription share in its [results filed on 19 May 2026](https://nsearchives.nseindia.com/corporate/MANKIND1_19052026200732_PR_Final.pdf). Torrent reported India revenue up 20% and fifth place in the Indian market in its [FY26 release of 22 May 2026](https://www.torrentpharma.com/docs/Torrent_Pharma_Press_release_Q4_25_26_e5822c6449.pdf). Abbott and Cipla shares are Douglas Insights estimates; FY26 rupee sales are converted at Rs 87.15 per dollar, the 2025 average in [Federal Reserve data](https://fred.stlouisfed.org/series/AEXINUS).

No India pharmaceutical drug company reaches 10% because doctors, not pharmacists, choose the brand, and each company's field force covers only some of India's prescribers. Scale comes from hundreds of brands, each a leader in one molecule. The global ranking of these firms against Eli Lilly and Pfizer sits in our [pharmaceutical drug market](https://www.douglasinsights.com/pharmaceutical-drug-market/) study.

## Which India pharmaceutical drug segment earns most: branded generics, OTC, trade generics, patented drugs or biologics?

Branded generics earn most, 64.5% of 2025 India pharmaceutical drug value, USD 17.4 billion, because Indian doctors prescribe by brand. Biologics and biosimilars grow fastest, 15.10% a year to USD 7.68 billion.

| Segment | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Branded generics | 64.5% | USD 17.4 billion | 9.70% | USD 43.8 billion |
| Over-the-counter (OTC) and consumer health | 11.5% | USD 3.09 billion | 10.30% | USD 8.25 billion |
| Trade generics and Jan Aushadhi generics | 9.0% | USD 2.42 billion | 12.60% | USD 7.93 billion |
| Patented innovative medicines | 8.0% | USD 2.15 billion | 12.20% | USD 6.80 billion |
| Biologics and biosimilars | 7.0% | USD 1.88 billion | 15.10% | USD 7.68 billion |

Branded generics, USD 17.4 billion, hold their share because a pharmacist in India may not switch a prescribed brand, so medical representatives compete for the doctor's pen. They grow 9.70% a year, a little below the market, as trade generics take price-sensitive patients. OTC and consumer health, USD 3.09 billion, grow 10.30% a year as antacids, vitamins and pain relief move to self-care and online pharmacy.

Trade generics and Jan Aushadhi generics, USD 2.42 billion, grow 12.60% a year because they sell without a field force at a fraction of brand prices. Patented innovative medicines, USD 2.15 billion, grow 12.20% a year on GLP-1 launches and oncology. Biologics and biosimilars, USD 1.88 billion, grow fastest because Indian biosimilar makers launch antibody copies at home first; insulin, the largest biologic by volume, is sized in our [Insulin Drug Market](https://www.douglasinsights.com/insulin-drug-market/) report.

## How do therapy areas and channels split India pharmaceutical drug spending?

Cardiovascular drugs take 13.5% of 2025 India pharmaceutical drug value, USD 3.63 billion, Douglas Insights estimates, ahead of anti-infectives at 12.5%. Retail pharmacy sells 78.0% of value, USD 21.0 billion.

By therapy area, anti-infectives earn USD 3.36 billion, gastrointestinal drugs 11.0% or USD 2.96 billion, anti-diabetic and obesity drugs 9.5% or USD 2.56 billion, and respiratory drugs 7.5% or USD 2.02 billion. Chronic therapies gain share each year: cardiac and diabetes brands are taken for life, so a patient started in 2025 is still buying in 2035, while an antibiotic course ends in a week.

By channel, hospital pharmacy takes 16.0%, USD 4.30 billion, and online pharmacy 6.0%, USD 1.61 billion. Indian chemists stock by brand, which is why field forces matter more here than in markets where insurers set formularies.

## Why is India pharmaceutical drug demand growing 5.80% a year in volume?

India pharmaceutical drug volume grows 5.80% a year, from 410 billion standard units in 2025 to 721 billion in 2035. Five forces add 6.30 points; chronic disease supplies 2.20 and wider access 1.60.

### Chronic disease: 2.20 points

Diabetes, hypertension and heart disease add 2.20 points. Patients on two or three daily tablets for decades lift units faster than any acute therapy, and the semaglutide jump from about 25,000 to about 170,000 monthly packs in two months shows how far price was holding chronic demand back.

### Wider access through Jan Aushadhi: 1.60 points

Access adds 1.60 points. The government's Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP) ran more than 18,646 Jan Aushadhi Kendras on 28 February 2026, selling a basket of 2,110 medicines, according to a [Press Information Bureau (PIB) paper of 21 March 2026](https://static.pib.gov.in/WriteReadData/specificdocs/documents/2026/mar/doc2026321831401.pdf), which puts buyer savings at about Rs 5,637 crore in FY26 up to 30 November 2025.

### New molecules and patent expiries: 0.90 points

Launches add 0.90 points. Off-patent semaglutide, new oral anti-diabetics and oncology launches bring patients who were previously untreated, not switched.

### Cheaper medicines after the GST cut: 0.80 points

Tax relief adds 0.80 points. From 22 September 2025 the goods and services tax (GST) on most medicines fell from 12% to 5%, and on 33 lifesaving drugs to nil, under the [56th GST Council decisions](https://gstcouncil.gov.in/sites/default/files/2025-09/press_release_press_information_bureau.pdf).

### Self-care and online pharmacy: 0.80 points

Self-care adds 0.80 points as OTC brands reach smaller towns through e-pharmacy delivery. The five forces, 2.20 + 1.60 + 0.90 + 0.80 + 0.80, give 6.30 points before restraints.

## What holds back India pharmaceutical drug growth as price caps and Schedule M bite?

Two restraints remove 0.50 points from India pharmaceutical drug volume growth, leaving 5.80% a year. Factory upgrades under Revised Schedule M take 0.30 points and antibiotic stewardship 0.20 points.

Revised Schedule M, India's upgraded good manufacturing practice (GMP) code, applied to large makers from 1 July 2023, and the Health Ministry gave firms with turnover under Rs 250 crore until 31 December 2025 to comply, [DT Next reported](https://www.dtnext.in/news/business/msme-pharma-companies-get-additional-one-year-to-upgrade-facilities-under-schedule-m-818067); about 8,500 of India's 10,500 drug plants are small units. Douglas Insights books 0.30 points of lost volume as some units shut or pause while they upgrade, with larger makers taking over their brands.

Stewardship removes 0.20 points as hospitals and regulators push back on fixed-dose antibiotic combinations. Price control is the other brake, but it acts on the price leg: the National Pharmaceutical Pricing Authority (NPPA) let ceiling prices of scheduled formulations rise only 0.64956% from 1 April 2026, by [notification S.O. 1581(E) of 25 March 2026](https://teamleaseregtech.com/updates/article/54164/nppa-notified-the-revised-ceiling-prices-for-scheduled-formulations-effective-april-1-2026/), so list prices on essential drugs barely move.

## Where in India does pharmaceutical drug spending sit, and which region grows fastest?

South India spends most, USD 7.26 billion or 27.0% of 2025 India pharmaceutical drug value. East and Northeast India grows fastest, 12.40% a year to USD 12.6 billion by 2035, Douglas Insights estimates.

| Region | Main states | Value 2025 | Share 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| South India | Tamil Nadu, Karnataka, Kerala, Telangana, Andhra Pradesh | USD 7.26 billion | 27.0% | 10.20% | USD 19.2 billion |
| North India | Uttar Pradesh, Delhi, Punjab, Haryana, Rajasthan | USD 6.99 billion | 26.0% | 10.80% | USD 19.5 billion |
| West India | Maharashtra, Gujarat, Goa | USD 6.59 billion | 24.5% | 9.90% | USD 16.9 billion |
| East and Northeast India | West Bengal, Bihar, Odisha, Assam | USD 3.90 billion | 14.5% | 12.40% | USD 12.6 billion |
| Central India | Madhya Pradesh, Chhattisgarh | USD 2.15 billion | 8.0% | 11.60% | USD 6.45 billion |

South India leads because Kerala has India's oldest population and the South diagnoses diabetes and hypertension early, so chronic prescriptions are dense. North India, USD 6.99 billion, grows 10.80% a year as Uttar Pradesh, India's most populous state, adds pharmacies and Jan Aushadhi outlets. West India, USD 6.59 billion, grows slowest at 9.90% because Maharashtra and Gujarat already start from the highest spending per person in our model.

East and Northeast India grows fastest from a low base: Bihar and Odisha spend far less per person than Kerala in our model, and wider insurance cover and generic stores close that gap. Central India, USD 2.15 billion, grows 11.60% a year on the same pattern. In the data table the row labelled Global holds the India total, USD 26.9 billion in 2025, which matches the India row in our [Asia Pacific Pharmaceutical Drug Market](https://www.douglasinsights.com/asia-pacific-pharmaceutical-drug-market/) report.

## What price does an India pharmaceutical drug unit sell at, and why does it rise 4.57% a year?

An India pharmaceutical drug standard unit sold at USD 0.0656, about Rs 5.72, in 2025, Douglas Insights estimates, from under Rs 2 for a Jan Aushadhi tablet to thousands for a biologic. Price per unit rises 4.57% a year.

| Price tier | Typical product | Price per unit | Share of 2025 units | Share of 2025 value |
| --- | --- | --- | --- | --- |
| Trade and Jan Aushadhi generics | Metformin, amlodipine, paracetamol strips | Rs 1 to 2 | 27.0% | 9.0% |
| Branded generics and OTC | Telmisartan, pantoprazole, cough syrups | Rs 3 to 8 | 71.4% | 76.0% |
| Patented drugs and biologics | GLP-1 pens, oncology antibodies, insulin analogues | Rs 50 and above | 1.6% | 15.0% |

Mix, not list price, lifts the India pharmaceutical drug price per unit. Essential medicines can rise only with the NPPA wholesale price index (WPI) allowance, 0.64956% for 2026, but patients move from acute antibiotics to chronic combination tablets and from tablets to pens and biologics. Douglas Insights projects USD 0.0686 per unit in 2026 and USD 0.103 by 2035.

The GST cut lowered what patients pay but not the invoice value this report measures. India's average price per unit stays well below the USD 0.233 Asia Pacific average, and far below the U.S., where prices are set very differently, as our [U.S. Pharmaceutical Drug Market](https://www.douglasinsights.com/u-s-pharmaceutical-drug-market/) report shows.

## How do GST, NPPA price rules, Schedule M and PLI shape India pharmaceutical drug sales?

Four rules shape India pharmaceutical drug sales: the GST cut from 12% to 5% on 22 September 2025, NPPA ceiling prices, Revised Schedule M plant standards from 31 December 2025 for small firms, and production-linked incentives (PLI).

The [GST Council](https://gstcouncil.gov.in/sites/default/files/2025-09/press_release_press_information_bureau.pdf) also moved three drugs for cancer and rare diseases, Agalsidase Beta, Imiglucerase and Eptacog alfa, from 5% to nil. Under the Drugs (Prices Control) Order 2013, the NPPA fixes ceiling prices for medicines on the National List of Essential Medicines (NLEM) and allows one WPI-linked change a year, 0.64956% from 1 April 2026.

The PLI scheme for pharmaceuticals had drawn Rs 40,890 crore of investment against a target of Rs 17,274.96 crore by September 2025, with sales of Rs 3,16,797 crore, of which Rs 2,03,730 crore were exports, according to the [PIB paper of 21 March 2026](https://static.pib.gov.in/WriteReadData/specificdocs/documents/2026/mar/doc2026321831401.pdf); the same paper reports the 2026-27 Union Budget proposal of Biopharma SHAKTI, Rs 10,000 crore over five years. Revised Schedule M raises costs for small plants, which Douglas Insights books as 0.30 points of lost volume.

## What if prices stay capped: where does the India pharmaceutical drug forecast land in 2035?

The India pharmaceutical drug market ends 2035 between USD 58.4 billion and USD 89.5 billion across our three cases, around a base of USD 73.9 billion. Chronic-disease volume and the pace of mix shift set most of the spread.

| Case | Volume leg | Price per unit leg | 2035 value |
| --- | --- | --- | --- |
| Slower | 4.30% | 3.60% | USD 58.4 billion |
| Base case | 5.80% | 4.57% | USD 73.9 billion |
| Faster | 7.00% | 5.40% | USD 89.5 billion |

In the slower case the NPPA widens price control to more chronic molecules and Schedule M closures run longer. In the faster case GLP-1 generics and insurance cover pull many more untreated diabetics into care. One extra point of yearly volume growth adds USD 7.29 billion to 2035 India pharmaceutical drug value.

Our 10.64% base is the fastest of the seven Asia Pacific country rates in our regional model, against 7.24% for Asia Pacific and 5.71% worldwide, because India starts from the lowest spending per person.

## Douglas Exclusive: the India Drug Price-Volume Ledger

The India Drug Price-Volume Ledger is a Douglas Insights model built from 9 sourced inputs, 3 company filings, 4 government actions and 2 news reports, setting each dated event against its effect on units or price per unit.

| Date | Event | Effect in the model |
| --- | --- | --- |
| 22 September 2025 | GST on most medicines cut from 12% to 5% | 0.80 points of volume growth |
| 31 December 2025 | Revised Schedule M deadline for small makers | 0.30 points of volume lost |
| 21 January 2026 | Torrent consolidates JB Chemicals | Fifth-largest seller gains cardiac brands |
| 28 February 2026 | 18,646 Jan Aushadhi Kendras open | Part of 1.60 access points |
| 21 March 2026 | Semaglutide patent expires; nearly 40 makers launch | Packs up about 6.8 times by April |
| 25 March 2026 | NPPA allows 0.64956% WPI rise | List prices on essential drugs held flat |
| 31 March 2026 | Novo Nordisk cuts semaglutide prices up to 48% | Brand defends against generics |

The finding, a Douglas Insights estimate: semaglutide monthly packs rose about 6.8 times while the typical monthly price fell about 75%, from Rs 11,000 to about Rs 2,750, so monthly molecule value rose about 1.7 times. In India a deep price cut on a chronic drug grows the market rather than shrinking it, and the same pattern sits behind the 4.57% mix-driven price leg.

## How does the methodology turn 410 billion units into USD 26.9 billion of India pharmaceutical drug sales?

Our methodology multiplies 410 billion standard units of medicine sold in India in 2025 by USD 0.0656 per unit to reach USD 26.9 billion. Units come from five regional builds; price from three price tiers.

The model covers 5 regions, 5 product segments, 6 therapy areas, 3 channels and 3 price tiers, and uses 68 data points, 14 sourced and 54 modelled. Arithmetic: 410 billion x USD 0.0656 = USD 26.9 billion; 26.9 x (1.0580 x 1.0457)^10 = 73.9, or USD 73.9 billion. Units reach 434 billion in 2026 and 721 billion in 2035.

Two cross-checks agree. First, the PIB paper puts FY25 industry turnover at Rs 4.72 lakh crore and exports at USD 30.5 billion; at the 2024 average rate of Rs 83.66 per dollar, turnover is USD 56.4 billion, leaving USD 25.9 billion sold at home by Indian makers, and our USD 26.9 billion is 3.9% higher because it adds imported finished medicines. Second, our USD 26.9 billion equals Rs 2.34 lakh crore, and Sun Pharma's Rs 19,290 crore of FY26 India sales is 8.2% of it, close to the 8.4% share the company reports.

## What counts in the India pharmaceutical drug market, and what is excluded?

The India pharmaceutical drug market counts invoice-price sales of finished prescription and OTC medicines consumed in India. Exports, bulk active ingredients, contract manufacturing for foreign clients, medical devices and Ayurvedic products stay out.

The by-product split covers branded generics, OTC and consumer health, trade generics and Jan Aushadhi generics, patented innovative medicines, and biologics and biosimilars. The by-therapy-area split covers cardiovascular, anti-infectives, gastrointestinal, anti-diabetic and obesity, respiratory, and other therapies. The by-channel split covers retail pharmacy, hospital pharmacy and online pharmacy. The by-region split covers South, North, West, East and Northeast, and Central India. India's USD 30.5 billion of FY25 drug exports are counted in the destination markets, not in this India pharmaceutical drug sizing.
