Douglas Insights sizes it at USD 112.10 billion in 2025, about 95 million implant procedures at an average USD 1,180 of implant value each, and at USD 194.27 billion in 2035, a 5.65% compound annual growth rate (CAGR). The study sits in our [medical devices coverage](https://www.douglasinsights.com/industry/medical-devices/), beside the [Dental Implants Market](https://www.douglasinsights.com/dental-implants-market/), [Cardiac Prosthetic Devices Market](https://www.douglasinsights.com/cardiac-prosthetic-devices-market/) and [Zygomatic and Pterygoid Implants Market](https://www.douglasinsights.com/zygomatic-and-pterygoid-implants-market/) reports; every figure follows the Douglas Insights [research methodology](https://www.douglasinsights.com/research-methodology/).

## Which medical implant segment leads: orthopaedic, cardiovascular or dental?

Orthopaedic implants lead the medical implants market with 34.0% of 2025 value, about USD 38.11 billion, Douglas Insights estimates. Cardiovascular implants, at 32.0%, grow fastest, 7.0% a year to USD 70.57 billion by 2035.

| Implant type | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Orthopaedic implants (joints, spine, trauma, sports medicine) | 34.0% | USD 38.11 billion | 4.6% | USD 59.76 billion |
| Cardiovascular implants (valves, pacemakers, defibrillators, stents, occluders) | 32.0% | USD 35.87 billion | 7.0% | USD 70.57 billion |
| Dental implants and abutments | 9.0% | USD 10.09 billion | 6.6% | USD 19.12 billion |
| Ophthalmic implants (intraocular lenses) | 7.0% | USD 7.85 billion | 5.0% | USD 12.78 billion |
| Neurostimulation implants | 6.0% | USD 6.73 billion | 6.4% | USD 12.51 billion |
| Hearing implants (cochlear and bone conduction) | 2.0% | USD 2.24 billion | 6.0% | USD 4.02 billion |
| Breast, aesthetic and other implants | 10.0% | USD 11.21 billion | 3.6% | USD 15.97 billion |

Orthopaedic implants lead because a hip or knee system sells for several thousand dollars and spine and trauma hardware is used in millions of operations. Zimmer Biomet sold USD 3.32 billion of knees and USD 2.09 billion of hips in 2025, both up 4.7%, and USD 2.15 billion of sports, extremities and trauma (S.E.T.) products, its [results of 10 February 2026](https://www.sec.gov/Archives/edgar/data/1136869/000119312526043620/zbh-ex99_1.htm) show. The segment grows 4.6% a year in our model, slower than the market, because joint prices are under steady pressure from hospital buying groups.

Cardiovascular implants, USD 35.87 billion, grow fastest. Transcatheter valves, leadless pacemakers and left atrial appendage occluders move patients who were too frail for open surgery into catheter-based implant procedures; Edwards' transcatheter mitral and tricuspid therapies (TMTT) sales jumped 56.4% to USD 551 million in 2025, its [February 2026 results](https://www.businesswire.com/news/home/20260210513744/en/Edwards-Lifesciences-Reports-Fourth-Quarter-Results/) show. Dental implants, USD 10.09 billion, grow 6.6% as value brands spread in Latin America and Asia; Straumann said its iEXCEL rollout passed one million implants sold, its [full-year release of 18 February 2026](https://www.straumann.com/content/dam/data/media-release/en/2026/32053e54-7676-4f7e-9d3d-92d71b19e0ea/StraumannGroup_Press+Release_FY2025.pdf) states. Intraocular lenses, USD 7.85 billion, grow 5.0% with cataract surgery volumes. Neurostimulation, USD 6.73 billion, grows 6.4%. Hearing implants, USD 2.24 billion, grow 6.0%: Cochlear implanted 56,692 units in its year to June 2026, up 5%, its [FY26 results of 18 August 2026](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vlb13j40ckk.pdf) show. Breast, aesthetic and other implants, USD 11.21 billion, grow 3.6%. Segment values for 2035 sum to USD 194.71 billion, 0.23% above the headline.

## What drives medical implant demand: older patients, catheter valves or outpatient surgery?

Implant procedures rise 4.4% a year in our base case, from about 95 million in 2025 to about 146 million in 2035. Four drivers add 5.60 points of gross volume growth; restraints remove 1.20 points.

### Ageing populations and joint replacement backlogs: 2.00 points

Ageing is the largest driver, worth 2.00 points. The world's 80-and-over population is projected to reach 265 million by the mid-2030s, outnumbering infants, the United Nations [World Population Prospects 2024 key messages](https://population.un.org/wpp/assets/Files/WPP2024_Key-Messages.pdf) state. The American Joint Replacement Registry held more than 4.6 million cumulative procedures, about 4.4 million of them hip and knee arthroplasties from 2012 to 2024, and grew 8.8% in a year, its [2025 annual report release of 23 October 2025](https://www.aaos.org/aaos-home/newsroom/press-releases/ajrr-annual-report-press-release/) states. The UK National Joint Registry passed 4.5 million recorded procedures, its [2025 annual report of 4 November 2025](https://www.hqip.org.uk/news/njr_annual_report25/) shows. Douglas Insights estimates each extra 1% of people over 75 adds about 0.6 million implant procedures a year worldwide.

### Catheter-delivered structural heart implants: 1.50 points

Structural heart implants add 1.50 points. On 1 May 2025 the US Food and Drug Administration (FDA) approved Edwards' SAPIEN 3 valve for patients with asymptomatic severe aortic stenosis, the first approval of its kind; in the EARLY TAVR trial 26.8% of 455 valve patients and 45.3% of 446 patients under surveillance suffered death, stroke or unplanned cardiovascular hospitalisation, Edwards' [release](https://www.businesswire.com/news/home/20250501280519/en/Edwards-TAVR-Receives-FDA-Approval-for-Patients-With-Asymptomatic-Severe-Aortic-Stenosis) states. Abbott's structural heart sales grew 12.3% to USD 2.52 billion and its heart failure sales 13.2% to USD 1.45 billion in 2025, its [results of 22 January 2026](https://abbott.mediaroom.com/2026-01-22-Abbott-Reports-Fourth-Quarter-and-Full-Year-2025-Results-Issues-2026-Financial-Outlook?asPDF=1) show. Douglas Insights estimates that treating asymptomatic patients earlier widens the eligible valve pool by about 15% in the US alone.

### Outpatient and ambulatory surgery: 1.10 points

Ambulatory surgery adds 1.10 points by freeing hospital beds and cutting the cost of each joint case. The US Centers for Medicare and Medicaid Services (CMS) began a three-year phase-out of its Inpatient Only list, removing 285 mostly musculoskeletal procedures in 2026 and adding 289 procedures to the ambulatory surgical centre (ASC) list, its [CY2026 final rule fact sheet of 21 November 2025](https://www.cms.gov/newsroom/fact-sheets/calendar-year-2026-hospital-outpatient-prospective-payment-system-opps-ambulatory-surgical-center) states. Stryker's MedSurg and Neurotechnology sales rose 15.7% to USD 15.65 billion in 2025, while its orthopaedics sales rose 4.3% to USD 9.47 billion, the [29 January 2026 release](https://s22.q4cdn.com/857738142/files/doc_financials/2025/q4/26-02.pdf) shows.

### Value dental and neuro implants in emerging markets: 1.00 point

Wider access adds the last 1.00 point. Straumann grew revenue 8.9% organically to CHF 2.61 billion in 2025 and credited its Neodent value brand with implant growth in Latin America, its [release of 18 February 2026](https://www.straumann.com/content/dam/data/media-release/en/2026/32053e54-7676-4f7e-9d3d-92d71b19e0ea/StraumannGroup_Press+Release_FY2025.pdf) states. Medtronic's neuromodulation revenue rose 7.0% to USD 2.07 billion in its year to April 2026, its [results of 3 June 2026](https://www.sec.gov/Archives/edgar/data/0001613103/000162828026040034/exhibit991-fy26q4earningsr.htm) show. Summed, 2.00 + 1.50 + 1.10 + 1.00 = 5.60 points, a Douglas Insights estimate.

## What restrains medical implants: price controls, device rules or hospital capacity?

Two restraints remove 1.20 points a year from medical implant volume: conformity and approval bottlenecks take 0.50 points, and surgeon and hospital capacity limits take 0.70 points.

Approval bottlenecks are real in Europe. The European Commission proposed on 16 December 2025 to simplify the Medical Device Regulation (MDR) and In Vitro Diagnostic Regulation, setting deadlines for conformity assessments, creating a list of critical devices and giving the European Medicines Agency a role in monitoring shortages, with expected savings of EUR 3.3 billion a year, its [press release IP/25/3077](https://ec.europa.eu/commission/presscorner/api/files/document/print/en/ip_25_3077/IP_25_3077_EN.pdf) states. Until those changes apply, Douglas Insights estimates that slow notified-body reviews keep some low-volume implant lines off European shelves and delay new models by 12 to 18 months.

Capacity limits take 0.70 points. Stryker's knee sales grew only 1.1% and its hip sales fell 1.0% in the third quarter of 2026, its [results of 29 October 2026](https://s22.q4cdn.com/857738142/files/doc_financials/2026/q3/SYK-Ex-99-1-Earnings-Q3-2026.pdf) show, and Zimmer Biomet's knee sales fell 3.1% in the same quarter, its [results of 4 November 2026](https://www.sec.gov/Archives/edgar/data/0001136869/000119312526446145/zbh-ex99_1.htm) state. Douglas Insights reads those numbers as a sign that the post-pandemic catch-up in joint surgery is over and that operating-room staff, not patient demand, now set the pace.

## Which region spends the most on medical implants, and where is growth fastest?

North America spends the most on medical implants, USD 50.45 billion or 45.0% of 2025 value, because implant prices and procedure rates are highest there. Asia Pacific grows fastest, 7.8% a year to USD 49.89 billion by 2035.

| Region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| North America | USD 50.45 billion | 45.0% | USD 52.97 billion | 5.0% | USD 82.17 billion |
| Europe | USD 28.02 billion | 25.0% | USD 29.31 billion | 4.6% | USD 43.94 billion |
| Asia Pacific | USD 23.54 billion | 21.0% | USD 25.38 billion | 7.8% | USD 49.89 billion |
| Latin America | USD 5.61 billion | 5.0% | USD 5.97 billion | 6.6% | USD 10.62 billion |
| Middle East and Africa | USD 4.48 billion | 4.0% | USD 4.79 billion | 6.9% | USD 8.74 billion |

North America leads on price and on early adoption of catheter valves and robotic joint surgery. The CMS shift of joint and spine cases to outpatient settings under its [CY2026 rule](https://www.cms.gov/newsroom/fact-sheets/calendar-year-2026-hospital-outpatient-prospective-payment-system-opps-ambulatory-surgical-center), with a 2.6% payment update, keeps US volume growing, and the region rises 5.0% a year to USD 82.17 billion in our model. Europe, USD 28.02 billion, grows 4.6%: the European device sector employs close to 1 million people and is worth about EUR 170 billion, and cardiovascular disease kills 1.7 million Europeans a year, the [Commission release of 16 December 2025](https://ec.europa.eu/commission/presscorner/api/files/document/print/en/ip_25_3077/IP_25_3077_EN.pdf) states.

Asia Pacific, USD 23.54 billion, grows fastest as China, India, Japan and South Korea add joint and valve capacity, though tendering keeps Chinese implant prices low, Douglas Insights estimates. Latin America, USD 5.61 billion, grows 6.6%, led by dental implants in Brazil, where Straumann's Neodent brand is based. The Middle East and Africa, USD 4.48 billion, grow 6.9% from Gulf hospital building. Japan and Australia buy at prices close to Europe, while China and India buy far more units at a fraction of the price, so the region's unit share is about 35% against a value share of 21.0%, a Douglas Insights estimate. Cochlear, an Australian company, said its implant units grew 5% in its year to June 2026 even as revenue was flat in constant currency, its [FY26 announcement](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vlb13j40ckk.pdf) shows, a pattern that mirrors the shift of growth toward lower-priced Asian and emerging markets. Regional values sum to USD 118.43 billion in 2026 and USD 195.36 billion in 2035, 0.56% above the headline.

## Which companies dominate medical implants?

Medtronic, Stryker, Johnson & Johnson MedTech, Abbott, Zimmer Biomet, Boston Scientific and Edwards Lifesciences hold about 55% of medical implants value, Douglas Insights estimates; no single company holds more than about 12%.

| Company | Main implant lines | Latest reported figure | Source date |
| --- | --- | --- | --- |
| Medtronic | Pacemakers, defibrillators, valves, spine, neurostimulators | FY2026 revenue USD 36.36 billion, up 8.4% | 3 June 2026 |
| Stryker | Knees, hips, trauma, spine | 2025 orthopaedics USD 9.47 billion, up 4.3% | 29 January 2026 |
| Zimmer Biomet | Knees, hips, S.E.T. | 2025 net sales USD 8.23 billion, up 7.2% | 10 February 2026 |
| Edwards Lifesciences | Transcatheter and surgical heart valves | 2025 sales USD 6.07 billion, up 11.5% | 10 February 2026 |
| Abbott | Rhythm management, structural heart, heart pumps | 2025 rhythm management USD 2.65 billion, up 10.9% | 22 January 2026 |
| Smith+Nephew | Knees, hips, trauma | 2025 orthopaedics USD 2.44 billion | 2 March 2026 |
| Straumann | Dental implants | 2025 revenue CHF 2.61 billion | 18 February 2026 |
| Cochlear | Cochlear implants | FY26 implant revenue AUD 1.44 billion | 18 August 2026 |

Medtronic is the broadest implant maker. In its year to 24 April 2026 its Cardiac Rhythm and Heart Failure division grew 17.4% to USD 7.50 billion, Structural Heart and Aortic 7.4% to USD 3.82 billion and Cranial and Spinal Technologies 5.0% to USD 5.22 billion, its [3 June 2026 results](https://www.sec.gov/Archives/edgar/data/0001613103/000162828026040034/exhibit991-fy26q4earningsr.htm) show; in the quarter to July 2026 revenue rose 13.7% to USD 9.76 billion, its [results of 1 September 2026](https://www.sec.gov/Archives/edgar/data/0001613103/000162828026059697/exhibit991-fy27q1earningsr.htm) state. Not all of that revenue is implants, so Douglas Insights counts about USD 13 billion of it in this market. Stryker is the fastest-growing large orthopaedic maker, with 2025 sales up 10.7% in constant currency, but most of that growth came from instruments, neurotechnology and medical-surgical equipment rather than from implants, its [29 January 2026 release](https://s22.q4cdn.com/857738142/files/doc_financials/2025/q4/26-02.pdf) shows.

Johnson & Johnson's MedTech arm, which includes DePuy Synthes orthopaedics, sold USD 33.79 billion in 2025, up 6.1%, its [results of 21 January 2026](https://jnj.com/media-center/press-releases/johnson-johnson-reports-q4-and-full-year-2025-results) state. Boston Scientific's cardiovascular sales rose 23.2% to USD 13.25 billion and its neuromodulation sales 8.4% to USD 1.20 billion in 2025, its [release of 4 February 2026](https://news.bostonscientific.com/2026-02-04-Boston-Scientific-announces-results-for-fourth-quarter-and-full-year-2025) shows. Smith+Nephew sold USD 1.01 billion of knee implants and USD 641 million of hip implants, both up 2.9% underlying, its [full-year results of 2 March 2026](https://data.fca.org.uk/artefacts/NSM/RNS/235dc121-6a12-4315-95b9-f6d9b433cc55.html) show. Cochlear's implant revenue was AUD 1.44 billion, 61% of its sales, its [FY26 announcement](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vlb13j40ckk.pdf) states.

## How much does a medical implant cost, from a dental fixture to a heart valve?

A medical implant procedure carries an average USD 1,180 of implant value in 2025, Douglas Insights estimates, from about USD 150 for an intraocular lens or USD 250 for a value dental implant to USD 20,000 or more for a transcatheter heart valve.

The price ladder is wide because the mix of implants is wide. Edwards' USD 4.49 billion of 2025 valve sales, set against more than 1 million patients treated with SAPIEN valves since launch, its [release](https://www.businesswire.com/news/home/20250501280519/en/Edwards-TAVR-Receives-FDA-Approval-for-Patients-With-Asymptomatic-Severe-Aortic-Stenosis) notes, points to a premium of USD 20,000 or more per valve, a Douglas Insights estimate. Joint implants sit in the middle of the ladder: Zimmer Biomet's USD 5.42 billion of 2025 knee and hip sales, its [10 February 2026 results](https://www.sec.gov/Archives/edgar/data/1136869/000119312526043620/zbh-ex99_1.htm) show, spread over an estimated 1.2 million to 1.4 million procedures implies about USD 4,000 to USD 4,500 of implant value per joint, a Douglas Insights estimate. Cochlear's AUD 1.44 billion of implant revenue across 56,692 units works out to about AUD 25,300 per implant system, from its [FY26 figures](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vlb13j40ckk.pdf).

Average realised price rises only 1.2% a year in our base case, to about USD 1,330 in 2035. New premium devices such as catheter valves and robot-placed joints lift the mix, while hospital tenders and outpatient payment rates, raised 2.6% for 2026 by [CMS](https://www.cms.gov/newsroom/fact-sheets/calendar-year-2026-hospital-outpatient-prospective-payment-system-opps-ambulatory-surgical-center), hold like-for-like prices roughly flat, Douglas Insights estimates.

## What if joint surgery slows or valves spread faster: what is the 2035 forecast?

The medical implants market ends 2035 between USD 162.99 billion and USD 226.72 billion across our three cases, around a base of USD 194.27 billion. Procedure volume and structural heart uptake set most of the spread.

| Case | Procedure volume leg | Price leg | Revenue rate | 2035 value |
| --- | --- | --- | --- | --- |
| Slower | 3.40% | 0.40% | 3.81% | USD 162.99 billion |
| Base case | 4.40% | 1.20% | 5.65% | USD 194.27 billion |
| Faster | 5.40% | 1.80% | 7.30% | USD 226.72 billion |

In the slower case joint volumes stay flat after the catch-up ends, European approvals stay slow and tenders in China and India spread to valves and pacemakers, taking 1.0 point off volume and 0.8 points off price. In the faster case asymptomatic valve treatment spreads beyond the US, outpatient joint surgery frees capacity and value dental implants reach more middle-income patients, adding 1.0 point of volume and 0.6 points of price. One extra point of yearly revenue growth adds about USD 19.19 billion to 2035 value, a Douglas Insights estimate.

Live signals sit near the base case. Stryker's organic growth was 7.7% in the third quarter of 2026, its [29 October 2026 release](https://s22.q4cdn.com/857738142/files/doc_financials/2026/q3/SYK-Ex-99-1-Earnings-Q3-2026.pdf) shows, but its knees and hips were flat, while Zimmer Biomet's nine-month organic growth was 3.5%, its [4 November 2026 results](https://www.sec.gov/Archives/edgar/data/0001136869/000119312526446145/zbh-ex99_1.htm) state. The signal to watch is quarterly hip and knee growth at the large orthopaedic makers.

## Which rules govern medical implants, from FDA approval to EU MDR reform?

Three rule sets govern medical implants: premarket approval of high-risk implants by the FDA, the EU Medical Device Regulation and its 2025 reform proposal, and payment rules that decide where implant surgery takes place.

Most implants are Class III devices in the US and need premarket approval backed by clinical trials. The SAPIEN 3 approval for asymptomatic patients of 1 May 2025 rested on a randomised trial of 901 patients, Edwards' [release](https://www.businesswire.com/news/home/20250501280519/en/Edwards-TAVR-Receives-FDA-Approval-for-Patients-With-Asymptomatic-Severe-Aortic-Stenosis) shows, and such trials set the pace at which new indications open.

In Europe, the Commission's 16 December 2025 proposal would put time limits on conformity assessment, list critical devices and add shortage monitoring, and is paired with a Safe Hearts Plan because cardiovascular disease costs EUR 282 billion a year and is projected to rise 90% by 2050 without action, its [press release](https://ec.europa.eu/commission/presscorner/api/files/document/print/en/ip_25_3077/IP_25_3077_EN.pdf) states. Payment rules complete the picture: CMS's three-year phase-out of the Inpatient Only list moves joint and spine implants into outpatient and ambulatory centres from 2026, its [fact sheet](https://www.cms.gov/newsroom/fact-sheets/calendar-year-2026-hospital-outpatient-prospective-payment-system-opps-ambulatory-surgical-center) states.

## Douglas Exclusive: the Implant Mix Shift Index for medical implants

The Implant Mix Shift Index is a Douglas Insights model built from 15 inputs: 10 company filings or releases, 3 government documents and 2 registry reports. It measures how fast implant value moves from orthopaedics to cardiovascular devices.

| Date | Input | What it moves |
| --- | --- | --- |
| 1 May 2025 | FDA: SAPIEN 3 asymptomatic approval | Valve demand |

The finding, a Douglas Insights estimate: across the company inputs, cardiovascular implant lines grew about 10 points a year faster than orthopaedic joint lines in 2025, and the gap widened in 2026 as joint growth slowed to near zero. Carried through our model, cardiovascular implants rise from 32.0% of value in 2025 to 36.2% in 2035, overtaking orthopaedics, which fall from 34.0% to 30.7%.

The index changes how to read the market. A supplier that tracks only joint replacement counts will see slow growth, while catheter-delivered heart devices gather most of the added value. Douglas Insights therefore forecasts value by implant family rather than by procedure count alone, and expects cardiovascular implants to pass orthopaedics in value around 2028.

## How does the methodology turn 95 million procedures into USD 112.10 billion?

Our methodology multiplies about 95 million medical implant procedures in 2025 by an average USD 1,180 of implant value each to reach USD 112.10 billion, then grows procedures 4.4% and price 1.2% a year.

The model covers 7 implant types and 5 regions, using 62 data points, 26 sourced and 36 modelled. Procedures come from a family build: about 32 million cataract lens implants, 24 million dental implants, 20 million orthopaedic implant procedures, 12 million cardiovascular implant procedures and 7 million neuro, hearing, breast and other implants, Douglas Insights estimates drawn from registry counts and company unit disclosures such as Cochlear's 56,692 units. Arithmetic: 95 million x USD 1,180 = USD 112.10 billion; USD 112.10 billion x (1.044 x 1.012)^10 = USD 194.27 billion. Procedures reach about 99 million in 2026 and 146 million in 2035.

Two cross-checks agree. First, the reported implant lines of Stryker, Zimmer Biomet, Smith+Nephew, Edwards, Abbott, Straumann and Cochlear, plus Douglas Insights estimates of the implant share of Medtronic, J&J MedTech and Boston Scientific, sum to about USD 63 billion, 56% of our market, in line with a moderately concentrated industry. Second, Edwards' USD 6.07 billion of sales equals about 17% of our cardiovascular segment, consistent with its leading valve position. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our [editorial standards](https://www.douglasinsights.com/editorial-standards/) explain.

## What counts in the medical implants study, and what is excluded?

The medical implants study counts devices left inside the body after surgery or a catheter procedure, valued at manufacturer selling prices. Surgical instruments, robots, ablation catheters, external hearing aids, surgeon fees and hospital charges are excluded.

The study splits value by 7 implant types and 5 regions: North America, Europe, Asia Pacific, Latin America, and the Middle East and Africa. Detailed dental coverage sits in our [Dental Implants Market](https://www.douglasinsights.com/dental-implants-market/) report, heart valves and related devices in the [Cardiac Prosthetic Devices Market](https://www.douglasinsights.com/cardiac-prosthetic-devices-market/) study and implants for the severely resorbed upper jaw in the [Zygomatic and Pterygoid Implants Market](https://www.douglasinsights.com/zygomatic-and-pterygoid-implants-market/) report.
