## Key takeaways

- The Middle East and Africa pharmaceutical drug market is worth USD 46.8 billion in 2025 and USD 103 billion in 2035, an 8.21% revenue CAGR.
- 230 billion daily doses at USD 0.203 make up the 2025 base; doses grow 3.10% and price per dose 4.96% a year.
- Branded small molecules hold 34.0%; biosimilars grow fastest at 15.6% a year.
- Saudi Arabia buys USD 10.3 billion, 22.0% of value; Nigeria grows fastest at 10.2%.
- Local makers supply about 55% of doses but about 33% of value: localisation cheapens tablets, not biologics.

Egypt's medicine market reached EGP 422 billion in 2025, about USD 8.5 billion and 37% more in pounds than in 2024, with roughly 91% of products made locally, the [Egyptian Drug Authority (EDA) said on 15 June 2026](https://edaegypt.gov.eg/en/media-center/news/dr-ali-el-ghamrawy-chairman-of-the-eda-participated-in-africa-health-excon-2026-where-he-presented-the-authority-s-strategic-vision-for-the-pharmaceutical-sector-through-2030/); three months later South Africa granted drug makers a second 2026 price rise of 2.88%. The region's medicines grow on repricing, local factories and new specialty drugs at the same time. The Middle East and Africa pharmaceutical drug market covers the finished prescription and over-the-counter medicines sold to patients, pharmacies, hospitals and public buyers in Saudi Arabia, the United Arab Emirates (UAE), Israel, Egypt, South Africa, Nigeria, the rest of the Gulf Cooperation Council (GCC), the rest of the Middle East and the rest of Africa, valued at manufacturer invoice prices, across branded small molecules, generics, branded biologics, biosimilars and over-the-counter (OTC) medicines. Douglas Insights sizes it at USD 46.8 billion in 2025, from 230 billion defined daily doses (DDDs) at an average USD 0.203 a dose, and at USD 103 billion in 2035, a revenue CAGR (compound annual growth rate) of 8.21%. The study sits in our [pharmaceuticals coverage](https://www.douglasinsights.com/industry/pharmaceuticals/), is the Middle East and Africa row of our worldwide [Pharmaceutical Drug Market](https://www.douglasinsights.com/pharmaceutical-drug-market/) report, and every figure follows the Douglas Insights [research methodology](https://www.douglasinsights.com/research-methodology/).

## Which pharmaceutical drug product types earn the most: branded tablets, generics or biologics?

Branded small molecules earn the most pharmaceutical drug value, 34.0% of 2025, USD 15.9 billion, because Gulf pharmacies and private clinics prescribe by brand. Biosimilars grow fastest, 15.6% a year, from USD 1.87 billion to USD 7.98 billion.

| Product type | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Branded small molecules | 34.0% | USD 15.9 billion | 6.40% | USD 29.6 billion |
| Generics | 31.0% | USD 14.5 billion | 8.00% | USD 31.3 billion |
| Branded biologics | 18.0% | USD 8.42 billion | 10.6% | USD 23.1 billion |
| Over-the-counter medicines | 13.0% | USD 6.08 billion | 6.80% | USD 11.7 billion |
| Biosimilars | 4.0% | USD 1.87 billion | 15.6% | USD 7.98 billion |

Branded small molecules, USD 15.9 billion, lead because insured patients in Saudi Arabia, the UAE and Israel, and cash-paying patients across Africa's private pharmacies, still ask for the originator or a branded copy. They grow 6.40% a year, the slowest line, as patents lapse and tender buyers switch. Generics, USD 14.5 billion or 31.0%, hold a larger share here than in Europe because local factories supply most doses: Egypt alone makes about 91% of what it consumes, the EDA says. They grow 8.00% a year and overtake branded small molecules by 2035, at USD 31.3 billion.

Branded biologics, USD 8.42 billion, grow 10.6% a year as cancer antibodies, immunology injectables and insulin analogues reach more Gulf and South African patients. Biosimilars, USD 1.87 billion, grow fastest, 15.6% a year, because public buyers switch quickly once an antibody loses protection; Hikma said its Middle East and North Africa (MENA) injectables grew 9% in 2025, "supported by biosimilars", in its [full-year 2025 results](https://www.hikma.com/html-summary/hikma-pharmaceuticals-plc-2025-full-year-results-combined-press-release-vfinal/). Over-the-counter medicines, USD 6.08 billion or 13.0%, keep that share because pain relievers, cold remedies and vitamins are bought without a prescription across Africa, and they grow 6.80% a year. Product-type values for 2035 sum to USD 104 billion, 0.67% above the headline. Our [Latin America Pharmaceutical Drug Market](https://www.douglasinsights.com/latin-america-pharmaceutical-drug-market/) report shows a similar branded-copy mix.

## What drives pharmaceutical drug value per dose up 4.96% a year?

Pharmaceutical drug value per daily dose rises 4.96% a year, from USD 0.203 in 2025 to about USD 0.330 in 2035, because specialty launches, repricing after devaluations, GLP-1 drugs and biologic mix add 6.40 points before restraints.

Daily doses grow 3.10% a year, from 230 billion to about 312 billion as Africa's population grows and Gulf insurance widens access, a Douglas Insights estimate. Four drivers make up the gross value-per-dose leg.

### Specialty launches: 1.90 points

New specialty medicines add 1.90 points. The Emirates Drug Establishment (EDE) approved 420 conventional and biological pharmaceuticals in the first half of 2026, including 63 innovator products, it said on [23 September 2026](https://www.zawya.com/en/business/uae-approves-942-medical-products-in-h1-over-quarter-locally-made-1129071). Each innovator replaces older therapy at a higher value per dose.

### Repricing after devaluations: 1.50 points

Repricing adds 1.50 points. Egypt's market rose 37% in pounds in 2025 as prices caught up with the currency, the [EDA](https://edaegypt.gov.eg/en/media-center/news/dr-ali-el-ghamrawy-chairman-of-the-eda-participated-in-africa-health-excon-2026-where-he-presented-the-authority-s-strategic-vision-for-the-pharmaceutical-sector-through-2030/) reports, and the drug makers listed on the Nigerian Exchange more than doubled their 2025 earnings, from NGN 6.5 billion to NGN 14.8 billion, on a stable naira and a tax waiver, [BusinessDay reported on 3 March 2026](https://businessday.ng/news/article/pharmaceutical-firms-profits-hit-decade-high-on-tax-waiver-stable-naira/).

### GLP-1 drugs for diabetes and obesity: 1.40 points

Glucagon-like peptide-1 (GLP-1) drugs add 1.40 points. Aspen said it holds more than 50% of South Africa's GLP-1 market and beat its ZAR 1 billion target for the class, on strong Mounjaro demand, at its results reported by [EWN on 2 September 2026](https://www.ewn.co.za/aspen-sees-stronger-earnings-ahead-as-demand-for-weight-loss-drugs-grows/). A weekly pen costs many times the metformin it partly replaces.

### Mix toward biologics: 1.60 points

Mix adds 1.60 points. Hikma's MENA branded business grew 10% to USD 849 million in 2025 and its operating profit rose 19% to USD 224 million, its [results presentation](https://www.hikma.com/media/d54itxkb/hikma-pharmaceuticals-plc-2025-fy-presentation-vfinal-260226.pdf) shows, naming Algeria, Saudi Arabia and Jordan as leading markets for products such as dapagliflozin and palbociclib. The four contributions, 1.90 + 1.50 + 1.40 + 1.60, give 6.40 points.

## What holds back pharmaceutical drug value growth: local generics, caps or currencies?

Three headwinds remove 1.44 points from pharmaceutical drug value-per-dose growth, leaving 4.96% a year: local generics and tender preference take 0.62 points, government ceilings 0.50 points and currency weakness 0.32 points.

Local copies are the largest brake. Egypt supplies about 91% of its own products, the UAE approved 200 locally made conventional pharmaceuticals in the first half of 2026, and Saudi Arabia's food and drug regulator built its December 2025 generic drug conference around local manufacturing, so tender buyers can pick a cheaper local version as soon as one is registered; Douglas Insights removes 0.62 points. Ceilings remove 0.50 points: South Africa's 2026 adjustments, 1.47% in January and 2.88% from 1 October, add up to about 2.1% for the year against 3.3% inflation at the January review, the pharmacy body Pharmisa calculated, [EWN reported](https://www.ewn.co.za/second-medicine-price-hike-for-2026-kicks-in-as-healthcare-costs-keep-rising/).

Currencies remove 0.32 points. Our forecast is in U.S. dollars, and in Egypt, Nigeria and much of Africa local price rises can lag depreciation for a year or more, which is why repricing appears as a driver and currency as a restraint at once. SPIMACO shows the squeeze on private sales: its revenue rose only 1.5% to SAR 1.71 billion in 2025, with government sales up 32% and private sales down 2%, its [Saudi Exchange filing of 30 March 2026](https://www.saudiexchange.sa/Resources/fsPdf/4962_387_2026-03-30_20-33-23_en.pdf) shows.

## Where is Middle East and Africa pharmaceutical drug money spent: Saudi Arabia, Egypt or South Africa?

Saudi Arabia spends the most on pharmaceutical drugs in the Middle East and Africa, USD 10.3 billion or 22.0% of 2025, a Douglas Insights estimate. Nigeria grows fastest, 10.2% a year to USD 3.71 billion by 2035.

| Country or sub-region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| Saudi Arabia | USD 10.3 billion | 22.0% | USD 11.2 billion | 8.40% | USD 23.1 billion |
| Egypt | USD 6.36 billion | 13.6% | USD 6.96 billion | 9.40% | USD 15.6 billion |
| South Africa | USD 4.49 billion | 9.6% | USD 4.78 billion | 6.40% | USD 8.35 billion |
| United Arab Emirates | USD 4.21 billion | 9.0% | USD 4.57 billion | 8.40% | USD 9.44 billion |
| Israel | USD 3.04 billion | 6.5% | USD 3.21 billion | 5.60% | USD 5.25 billion |
| Rest of GCC | USD 2.53 billion | 5.4% | USD 2.72 billion | 7.80% | USD 5.36 billion |
| Nigeria | USD 1.40 billion | 3.0% | USD 1.55 billion | 10.2% | USD 3.71 billion |
| Rest of Middle East | USD 6.08 billion | 13.0% | USD 6.53 billion | 7.40% | USD 12.4 billion |
| Rest of Africa | USD 8.38 billion | 17.9% | USD 9.16 billion | 9.40% | USD 20.6 billion |

Saudi Arabia leads because it has the Gulf's largest population, a single public buyer in the National Unified Procurement Company (NUPCO) and a private market that still favours branded medicines; it grows 8.40% a year as biologics reach public lists and local plants add volume. Egypt, USD 6.36 billion, is the second market: Douglas Insights takes about 75% of the EDA's USD 8.5 billion as manufacturer invoice value, our estimate of what remains after pharmacy and distributor margins. It grows 9.40% a year on repricing and 10,000-plus registered products, up from 8,500. The UAE, USD 4.21 billion, grows 8.40% on innovator approvals; Israel, USD 3.04 billion, grows 5.60%, the slowest, because its health funds already cover most patients and buy at negotiated prices.

South Africa, USD 4.49 billion, grows 6.40% a year because the Single Exit Price (SEP) caps private-sector prices and the state buys by tender; GLP-1 pens are its main upside. Nigeria, USD 1.40 billion, grows fastest, 10.2% a year, as local makers such as Fidson and May & Baker expand plants under the duty and value-added tax waiver. The rest of the GCC, Kuwait, Qatar, Oman and Bahrain, holds USD 2.53 billion; the rest of the Middle East, from Iraq and Jordan to Lebanon and Iran, USD 6.08 billion; and the rest of Africa, from Algeria and Morocco to Kenya and Ethiopia, USD 8.38 billion, growing 9.40%. Country values for 2035 sum to USD 104 billion, 0.75% above the headline; in the data table the row labelled Global holds the Middle East and Africa total. The same build for other areas is in our [Europe Pharmaceutical Drug Market](https://www.douglasinsights.com/europe-pharmaceutical-drug-market/) and [Asia Pacific Pharmaceutical Drug Market](https://www.douglasinsights.com/asia-pacific-pharmaceutical-drug-market/) reports.

## Who sells the most pharmaceutical drugs: Sanofi, Hikma or Aspen?

Sanofi is the largest pharmaceutical drug seller here, with about 2.9% of 2025 value, USD 1.36 billion, a Douglas Insights estimate. The top three companies, Sanofi, Hikma and Pfizer, hold about 7.8%; local makers hold far more than in Europe.

| Company | What the position rests on | Estimated share 2025 | Estimated value 2025 |
| --- | --- | --- | --- |
| Sanofi | Insulins, vaccines, Dupixent, consumer health; Gulf insulin partnerships | 2.9% | USD 1.36 billion |
| Hikma Pharmaceuticals | MENA branded generics and injectables, in-licensed biosimilars | 2.6% | USD 1.22 billion |
| Pfizer | Vaccines, anti-infectives, oncology | 2.3% | USD 1.08 billion |
| Novartis | Cardiology, oncology, immunology | 2.2% | USD 1.03 billion |
| AstraZeneca | Oncology, diabetes, respiratory | 1.9% | USD 889 million |
| Aspen Pharmacare | South African generics, Mounjaro in South Africa, anaesthetics | 1.6% | USD 749 million |
| SPIMACO | Saudi generics, government and contract manufacturing | 1.0% | USD 468 million |
| Julphar | UAE generics and insulin | 0.6% | USD 281 million |
| Other companies | GSK, Novo Nordisk, Roche, Teva, Jamjoom Pharma, Tabuk Pharmaceuticals, EIPICO, Pharco, Fidson, May & Baker and hundreds of local firms | 84.9% | USD 39.7 billion |

All shares are Douglas Insights estimates because multinationals report these markets inside wider regions. Hikma's MENA branded revenue was USD 849 million in 2025, up 10%, and its MENA injectables add the rest of our USD 1.22 billion. SPIMACO's SAR 1.71 billion equals USD 455 million, and Julphar's revenue from continuing operations rose 8.4% to AED 1.08 billion, about USD 293 million, its [results of 26 March 2026](https://www.julphar.net/en/julphar-delivers-strong-fy2025-results-with-accelerated-growth-and-improved-profitability) state; part of both sit outside our scope as exports or contract work. Fidson raised its 2025 profit after tax 61% to NGN 9.31 billion. Local scale matters: a maker with a plant and a registration in Riyadh, Cairo or Lagos wins tenders on day one, while multinationals win in insured specialty care.

## How much does a pharmaceutical drug daily dose cost, from a USD 0.03 generic to a USD 28 biologic?

A pharmaceutical drug daily dose cost an average USD 0.203 here in 2025, a Douglas Insights estimate, about 26% of Europe's USD 0.772, from USD 0.03 for a generic tablet to USD 28 for a biologic.

| Product type | Typical invoice price per daily dose, 2025 | What sets it |
| --- | --- | --- |
| Generics | USD 0.03 to USD 0.12 | Public tenders, local plants, Egyptian pound and naira |
| OTC medicines | USD 0.08 to USD 0.25 | Pharmacy competition, brand |
| Branded small molecules | USD 0.30 to USD 1.80 | National ceilings, Gulf reference pricing |
| Biosimilars | USD 2 to USD 10 | NUPCO and ministry tenders |
| Branded biologics | USD 6 to USD 28 | Insurer and health-fund negotiation |

The bands are Douglas Insights estimates from national price lists and company disclosures. The low average reflects mix: cheap local generics supply most African doses, while Gulf patients pay prices closer to Europe's. South Africa shows how tightly prices move: the SEP is the only price at which a maker may sell to a private buyer, and its two 2026 rises, 1.47% and 2.88%, together lift prices about 2.1% in the year. The UAE took 20 pricing decisions in the first half of 2026, the EDE says. The average reaches about USD 0.214 a dose in 2026 and USD 0.330 in 2035, 62% above 2025 and above Latin America's USD 0.278 of 2025.

## Which rules shape pharmaceutical drug sales: SEP adjustments, SFDA localisation or the new EDE?

Four rule sets shape pharmaceutical drug sales most: South Africa's Single Exit Price, the Saudi Food and Drug Authority (SFDA) push for local generics, the UAE's Emirates Drug Establishment and the new African Medicines Agency (AMA).

South Africa's Department of Health set a 2.88% SEP increase for all registered medicines from 1 October 2026, to "respond to exceptional domestic and geopolitical pricing pressures", [SAnews reported on 30 September 2026](https://www.sanews.gov.za/node/84652); the SEP does not apply to the state, which buys by tender, and the next adjustment is due in January 2027. The SFDA held a generic drug conference, "Empowering Local Pharmaceutical Manufacturing", in Riyadh on 30 December 2025, with pricing, reimbursement and Good Manufacturing Practice (GMP) on the agenda, its [notice of 11 December 2025](https://www.sfda.gov.sa/en/news/18776) says.

The EDE made 2,030 inspection visits and issued 38 recall and supply circulars by 30 June 2026, and approved 942 products, 247 of them made in the UAE. Nigeria's Customs Service granted manufacturers a two-year waiver of import duty and value-added tax on active pharmaceutical ingredients (APIs), excipients and packaging from March 2025, [the Gazette reported](https://gazettengr.com/customs-grant-two-year-tax-waiver-on-pharmaceutical-raw-materials/). At continental level, the African Union appointed Delese Mimi Darko as the AMA's first Director General in Kigali in June 2025, [CTI Exchange reported](https://www.ctiexchange.org/whats-new/news/dr-delese-mimi-darko-appointed-first-director-general-african-medicines-agency), a step toward shared approvals that would cut launch delays.

## What if Gulf tenders tighten: pharmaceutical drug value in 2035 under three cases

The pharmaceutical drug market ends 2035 between USD 85.3 billion and USD 122 billion across our three cases, around a base of USD 103 billion. Tender discipline and currency paths set most of the spread.

| Case | Daily dose leg | Value per dose leg | 2035 value |
| --- | --- | --- | --- |
| Slower | 2.50% | 3.60% | USD 85.3 billion |
| Base case | 3.10% | 4.96% | USD 103 billion |
| Faster | 3.60% | 6.20% | USD 122 billion |

In the slower case NUPCO and Egypt's Unified Procurement Authority push more spending to lowest-bid local tenders, South Africa's SEP rises stay below inflation and the pound and naira weaken again. In the faster case biologics and GLP-1 pens reach public lists quickly and currencies hold. One extra point of yearly value-per-dose growth adds USD 10.3 billion to 2035 pharmaceutical drug value; one point less removes USD 9.37 billion.

The 8.21% base rate is the fastest of the five areas in our worldwide model, above Latin America's 7.15%, Asia Pacific's 7.24% and Europe's 4.43%, because volume still grows 3.10% a year here against 0.82% worldwide.

## Douglas Exclusive: the Medicine Localisation Ledger

The Medicine Localisation Ledger is a Douglas Insights model, an estimate rather than an official register, built from 6 inputs published by 3 public bodies and 2 companies. It shows how much pharmaceutical drug supply is made locally.

| Input | Figure | Source and date |
| --- | --- | --- |
| Egypt products made locally | About 91% | EDA, 15 June 2026 |
| Egypt medicine exports, end-2025 | About USD 1.3 billion | EDA, 15 June 2026 |
| UAE-made share of pharmaceutical approvals, H1 2026 | 200 of 420, 47.6% | EDE, 23 September 2026 |
| Nigeria duty and VAT waiver on inputs | Two years from March 2025 | Nigeria Customs Service |
| SPIMACO government sales, 2025 | +32%, private sales -2% | Saudi Exchange filing, 30 March 2026 |
| Hikma MENA branded revenue, 2025 | USD 849 million, +10% | Hikma results, 2026 |

The finding, a Douglas Insights estimate: makers based in the Middle East and Africa supply about 55% of the area's 230 billion daily doses but only about 33% of its USD 46.8 billion value, roughly USD 15.4 billion, because local plants make tablets and imports still supply most biologics. Governments are now steering their own tenders to those plants, as SPIMACO's 32% public growth shows. That is why we take 0.62 points off value per dose for local generics, yet add 1.60 points for biologic mix: localisation lowers what a tablet costs, not what a cancer antibody costs.

## How does the methodology turn 230 billion daily doses into USD 46.8 billion of pharmaceutical drug sales?

Our methodology multiplies 230 billion defined daily doses sold in 2025 by an average USD 0.2035 a dose to reach USD 46.8 billion. Doses come from nine country and sub-region builds; Egypt is anchored on EDA data.

The model covers 9 countries or sub-regions, 5 product types and 3 channels, and uses 104 data points, 18 sourced and 86 modelled. Arithmetic: 230 billion doses x USD 0.2035 = USD 46.8 billion; USD 46.8 billion x (1.0310 x 1.0496)^10 = USD 103 billion. Doses reach 237 billion in 2026 and 312 billion in 2035; value per dose reaches USD 0.214 in 2026. Currency conversions use SAR 3.75 and AED 3.6725 per U.S. dollar, the two pegged rates.

Two cross-checks agree. First, the Middle East and Africa row of our worldwide [Pharmaceutical Drug Market](https://www.douglasinsights.com/pharmaceutical-drug-market/) model is USD 46.8 billion in 2025 and USD 103 billion in 2035, 2.62% of the USD 1.79 trillion world total. Second, Egypt's USD 6.36 billion, built from the official USD 8.5 billion, is 13.6% of the total, consistent with Egypt's place as the second-largest market after Saudi Arabia in our build. For comparison, insulin values per patient are set out in our [Asia Pacific Insulin Drug Market](https://www.douglasinsights.com/asia-pacific-insulin-drug-market/) report. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our [editorial standards](https://www.douglasinsights.com/research-methodology/) explain.

## Do tender supplies and free public-clinic medicines count in the pharmaceutical drug total, and what is excluded?

Yes: all USD 46.8 billion of the pharmaceutical drug total is finished medicine valued at manufacturer invoice prices, so tender supplies and free public-clinic medicines count. Supplies to NUPCO or South Africa's state depots count at the price the maker invoices. Pharmacy and wholesale margins, sales taxes, APIs sold to other makers, exports, veterinary medicines and medical devices are excluded.

The by-product-type split covers branded small molecules, generics, branded biologics, biosimilars and OTC medicines; the by-channel split covers retail pharmacy, hospital and public institutional supply, and online pharmacy; the by-country split covers Saudi Arabia, the United Arab Emirates, Israel, Egypt, South Africa, Nigeria, the rest of GCC, the rest of Middle East and the rest of Africa.
