## Which nasal therapy area earns the most: allergy sprays, congestion relief or brain-targeted drugs?

Allergic rhinitis and rhinosinusitis sprays earn the most, 45.0% of 2025 nasal drug delivery value or about USD 11.9 billion, Douglas Insights estimates. Central nervous system and pain products grow fastest, 11.6% a year to USD 9.49 billion by 2035.

| Therapy area | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Allergic rhinitis and rhinosinusitis | 45.0% | USD 11.9 billion | 4.6% | USD 18.6 billion |
| Cold, congestion and saline | 25.0% | USD 6.60 billion | 4.2% | USD 9.96 billion |
| Central nervous system and pain | 12.0% | USD 3.17 billion | 11.6% | USD 9.49 billion |
| Emergency rescue | 4.0% | USD 1.06 billion | 8.8% | USD 2.45 billion |
| Nasal vaccines | 2.0% | USD 528 million | 9.0% | USD 1.25 billion |
| Other systemic therapies | 12.0% | USD 3.17 billion | 7.8% | USD 6.71 billion |

Allergy sprays lead because steroid and antihistamine pumps such as Flonase, Nasonex, Avamys and Xhance are used daily for months each season by hundreds of millions of patients. Paratek paid USD 9 a share in cash plus contingent rights worth up to USD 5 for Optinose, maker of the Xhance exhalation delivery system, in a deal worth about USD 330 million that closed on 21 May 2025, its [completion release](https://www.biospace.com/press-releases/paratek-pharmaceuticals-completes-acquisition-of-optinose-creating-an-expanded-portfolio-of-specialty-therapies) states; the rights pay out only if Xhance sales reach USD 150 million or USD 225 million in a year. Allergy value grows 4.6% a year to USD 18.6 billion, a Douglas Insights estimate.

Cold, congestion and saline sprays, USD 6.60 billion, grow 4.2% to USD 9.96 billion; Aptar named nasal decongestants among the drivers of its consumer health growth in its [third-quarter 2026 results of 29 October 2026](https://finviz.com/news/412253/aptar-reports-third-quarter-2026-results). Central nervous system and pain products, USD 3.17 billion, grow fastest because Spravato, migraine sprays and seizure rescue sprays reach the brain within minutes. Emergency rescue, USD 1.06 billion, grows 8.8% to USD 2.45 billion. Nasal vaccines, USD 528 million, grow 9.0% to USD 1.25 billion. Other systemic therapies, from desmopressin to new cardiac and diuretic sprays, grow 7.8% to USD 6.71 billion. Therapy values for 2035 sum to USD 48.5 billion, 0.24% below the headline.

## Which nasal device format is used most: multidose pumps, unit-dose devices or powders?

Multidose spray pumps carry 68.0% of 2025 value, about USD 18.0 billion, Douglas Insights estimates. Nasal powder devices grow fastest, 10.0% a year to USD 4.11 billion by 2035.

| Device format | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Multidose spray pumps | 68.0% | USD 18.0 billion | 5.6% | USD 31.0 billion |
| Unit-dose and bi-dose liquid devices | 18.0% | USD 4.75 billion | 8.6% | USD 10.8 billion |
| Nasal powder devices | 6.0% | USD 1.58 billion | 10.0% | USD 4.11 billion |
| Drops, atomisers and other formats | 8.0% | USD 2.11 billion | 3.2% | USD 2.89 billion |

Multidose pumps lead because one bottle delivers 60 to 200 metered sprays of an allergy or congestion medicine, a Douglas Insights estimate, so the format suits chronic, self-managed use. Nemera sells its SP270+, SP370+ and UniSpray liquid pumps alongside a new dry powder range after it agreed to offer Therakind's DriDose intranasal powder device in two multi-dose versions and one unit-dose version, [OINDPnews reported on 9 April 2026](https://www.oindpnews.com/2026/04/nemera-to-offer-therakinds-dridose-intranasal-dry-powder-nasal-device/). Multidose value grows 5.6% a year to USD 31.0 billion.

Unit-dose and bi-dose liquid devices, USD 4.75 billion, grow 8.6% to USD 10.8 billion because rescue drugs and Spravato need one exact dose with no priming. Nasal powder devices, USD 1.58 billion, grow fastest as powders keep peptides and dihydroergotamine stable without preservatives. Drops, atomisers and other formats, USD 2.11 billion, grow 3.2% to USD 2.89 billion. Device values for 2035 sum to USD 48.8 billion, 0.39% above the headline.

## What drives nasal drug delivery demand: Spravato, needle-free rescue sprays or new systemic approvals?

Nasal device volume rises 3.2% a year in our base case, from 3.30 billion units in 2025 to about 4.52 billion in 2035. Four drivers add 4.10 points of gross growth; restraints remove 0.90 points.

### Brain-targeted therapies led by Spravato: 1.30 points

Central nervous system demand is the largest driver, worth 1.30 points. Spravato sales rose 40.8% to USD 584 million in the second quarter of 2026, with USD 514 million in the United States, J&J's [results of 15 July 2026](https://s203.q4cdn.com/636242992/files/doc_financials/2026/q2/2Q26-Earnings-Press-Release-Final.pdf) show, and grew 36% operationally in the third quarter, the company said on its [earnings call of 13 October 2026](https://s203.q4cdn.com/636242992/files/content_files/JNJ-USQ_Transcript_2026-10-13.pdf). The US Food and Drug Administration (FDA) approved Spravato as the first monotherapy for treatment-resistant depression, after more than 140,000 patients had been treated, J&J's [release of 21 January 2025](https://www.jnj.com/media-center/press-releases/spravato-esketamine-approved-in-the-u-s-as-the-first-and-only-monotherapy-for-adults-with-treatment-resistant-depression) states. Douglas Insights estimates each Spravato patient uses about 60 single-use devices in the first year.

### Needle-free emergency rescue: 0.80 points

Rescue sprays add 0.80 points. ARS Pharmaceuticals' US net product revenue for neffy epinephrine nasal spray rose to USD 33.4 million in the third quarter of 2026 from USD 31.3 million, and unique prescribers rose 49% to 19,605, its [results of 5 November 2026](https://www.sec.gov/Archives/edgar/data/0001671858/000119312526449748/spry-ex99_1.htm) show; full-year 2025 neffy revenue was USD 72.2 million, its [filing of 9 March 2026](https://www.sec.gov/Archives/edgar/data/1671858/000119312526097467/spry-ex99_1.htm) states. Amphastar's Baqsimi nasal glucagon earned USD 185.4 million in 2025, up 46%, its [results of 26 February 2026](https://www.biospace.com/press-releases/amphastar-pharmaceuticals-reports-financial-results-for-the-three-months-and-full-year-ended-december-31-2025) show. Douglas Insights estimates that fewer than 1 in 10 people prescribed an epinephrine rescue product carry a nasal one today.

### New systemic approvals: 0.70 points

New nose-to-blood medicines add 0.70 points. The FDA approved Satsuma's Atzumi dihydroergotamine nasal powder for migraine on 30 April 2025, the [Drugs.com approval history](https://www.drugs.com/history/atzumi.html) records, Corstasis' Enbumyst bumetanide spray for oedema against a 14 September 2025 action date, [OINDPnews reported on 15 September 2025](https://www.oindpnews.com/2025/09/corstasis-announces-fda-approval-of-enbumyst-bumetanide-diuretic-nasal-spray-new-vp-of-medical-affairs/), and Milestone's Cardamyst etripamil spray for paroxysmal supraventricular tachycardia, [reported on 15 December 2025](https://www.oindpnews.com/2025/12/fda-approves-milestones-cardamyst-etripamil-nasal-spray-for-psvt/). Neurelis' Valtoco diazepam spray gained use in children aged 2 to 5, [OINDPnews reported on 17 April 2025](https://www.oindpnews.com/2025/04/fda-approves-neureliss-valtoco-diazepam-nasal-spray-for-use-in-children-aged-2-5). Douglas Insights counts at least 4 new systemic nasal approvals or label extensions in 2025.

### Asian demand and local device capacity: 1.30 points

Asia adds 1.30 points as allergy and congestion spray use rises in India, China and Southeast Asia. Aptar inaugurated an expanded 11,300 square metre plant in Mumbai on 5 October 2026 that adds local moulding of multi-dose nasal spray systems, [Indian Pharma Post reported on 13 October 2026](https://www.indianpharmapost.com/news/aptar-expands-mumbai-operations-as-it-adds-local-production-of-pharma-inhalation-tech-22137), and China's regulator approved neffy 2 mg in December 2025, the [ARS filing of 9 March 2026](https://www.sec.gov/Archives/edgar/data/1671858/000119312526097467/spry-ex99_1.htm) shows. Summed, 1.30 + 0.80 + 0.70 + 1.30 = 4.10 points, a Douglas Insights estimate.

## What holds nasal drug delivery growth back: naloxone price cuts, generic erosion or device qualification?

Two restraints remove 0.90 points a year from nasal device volume growth: public-channel naloxone and emergency destocking take 0.40 points, and slow device qualification and generic substitution take 0.50 points.

Naloxone is the clearest drag. Emergent BioSolutions' naloxone revenue, from Narcan and Kloxxado, fell 43% to USD 226.1 million in 2025 from USD 398.9 million, its [results of 26 February 2026](https://www.sec.gov/Archives/edgar/data/1367644/000136764426000012/ebs2025-12x31exhibit99er.htm) show, and fell a further 42% to USD 43.5 million in the third quarter of 2026 on lower prices and volumes in public interest channels, its [results of 2 November 2026](https://www.sec.gov/Archives/edgar/data/0001367644/000136764426000110/ebs202609-30ex99er.htm) state. Aptar's Pharma core sales rose only 1% in that quarter, or 8% excluding the "anticipated reduction in emergency medicine sales", its [29 October 2026 release](https://finviz.com/news/412253/aptar-reports-third-quarter-2026-results) shows.

Qualification takes 0.50 points. A new nasal drug must prove spray pattern, droplet size and dose uniformity on one specific pump, so switching devices can take two to four years, a Douglas Insights estimate. Generic substitution also moves branded allergy volume to cheaper sprays once product-specific bioequivalence rules are set; the FDA published draft guidance on fluticasone propionate nasal spray in [Federal Register notice 2025-09067 on 21 May 2025](https://www.federalregister.gov/d/2025-09067), with comments due on 21 July 2025.

## Which region buys the most nasal drug delivery technology, and where does it grow fastest?

North America buys the most, USD 11.1 billion or 42.0% of 2025 value, led by Spravato, rescue sprays and over-the-counter allergy brands. Asia Pacific grows fastest, 7.3% a year to USD 12.3 billion by 2035.

| Region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| North America | USD 11.1 billion | 42.0% | USD 11.8 billion | 6.4% | USD 20.6 billion |
| Europe | USD 6.60 billion | 25.0% | USD 6.94 billion | 5.2% | USD 11.0 billion |
| Asia Pacific | USD 6.07 billion | 23.0% | USD 6.52 billion | 7.3% | USD 12.3 billion |
| Latin America | USD 1.58 billion | 6.0% | USD 1.68 billion | 6.2% | USD 2.89 billion |
| Middle East and Africa | USD 1.06 billion | 4.0% | USD 1.13 billion | 6.8% | USD 2.04 billion |

North America leads because the highest-priced systemic sprays sell there first. Of Spravato's USD 1.70 billion in 2025, USD 1.49 billion came from the United States and USD 210 million from other markets, J&J's [21 January 2026 release](https://s203.q4cdn.com/636242992/files/doc_news/Johnson--Johnson-reports-Q4-and-Full-Year-2025-results-2026.pdf) shows, and AstraZeneca began shipping FluMist Home to adults and caregivers in 34 states for the 2025-26 season, its [release of 15 August 2025](https://www.businesswire.com/news/home/20250815310613/en) states. Regional value grows 6.4% a year to USD 20.6 billion.

Europe, USD 6.60 billion, grows 5.2% as the European Commission approved EURneffy 1 mg for children aged 4 and over weighing 15 to 30 kg across the European Union, Iceland, Norway and Liechtenstein, ALK-Abello's [release of 31 March 2026](https://ir.alk-abello.com/news-releases/news-release-details/eurneffy-1-mg-approved-across-eu-first-and-only-needle-free) states. Asia Pacific, USD 6.07 billion, grows fastest on Indian and Chinese demand. Latin America, USD 1.58 billion, grows 6.2% and the Middle East and Africa, USD 1.06 billion, grow 6.8%. Regional values sum to USD 28.1 billion in 2026 and USD 48.8 billion in 2035, 0.36% above the headline.

## Which companies lead nasal drug delivery, from device makers to drug owners?

Aptar supplies an estimated 60% to 65% of prescription nasal spray pumps, a Douglas Insights estimate; Johnson & Johnson owns Spravato, the largest nasal drug. No company holds above about 8% of nasal value.

| Company | Nasal role | Latest reported figure | Period and source date |
| --- | --- | --- | --- |
| AptarGroup | Spray pumps, unit-dose devices | Pharma segment sales USD 1.35 billion, nine months | To 30 September 2026; 10-Q |
| Johnson & Johnson | Spravato | USD 1.70 billion, up 57.4% | 2025; 21 January 2026 |
| Emergent BioSolutions | Narcan, Kloxxado | Naloxone USD 226.1 million, down 43% | 2025; 26 February 2026 |
| Amphastar | Baqsimi | USD 185.4 million, up 46% | 2025; 26 February 2026 |
| ARS Pharmaceuticals | neffy | Total revenue USD 37.7 million | Quarter to September 2026; 5 November 2026 |
| Paratek (Optinose) | Xhance | Acquisition about USD 330 million | Closed 21 May 2025 |
| Nemera | Pumps, DriDose powder | Private; no revenue disclosed | 9 April 2026 |
| Kindeva | Nasal contract development | Private; no revenue disclosed | 18 June 2025 |

Aptar's Pharma segment sold USD 454.0 million in the third quarter of 2026 against USD 445.4 million a year earlier and USD 1.35 billion in nine months, up 4.1%, its [10-Q for the quarter to 30 September 2026](https://www.sec.gov/Archives/edgar/data/0000896622/000089662226000190/atr-20260930.htm) shows, and its whole company sold USD 3.78 billion in 2025, up 5%, its [2025 results](https://www.biospace.com/press-releases/aptar-reports-fourth-quarter-and-annual-2025-results) state. Pharma's adjusted EBITDA margin was 36.4%, down 80 basis points, in the third quarter, the [29 October 2026 release](https://finviz.com/news/412253/aptar-reports-third-quarter-2026-results) shows.

Contract developers widen choice for drug owners. Kindeva, which bought nasal specialist Summit Biosciences in 2024, agreed to develop Rocket Science Health's olfactory delivery device for the olfactory clefts, [OINDPnews reported on 18 June 2025](https://www.oindpnews.com/2025/06/kindeva-and-rocket-science-health-partner-on-development-of-intranasal-device-for-delivery-to-the-olfactory-clefts). Haleon, Bayer, Viatris, Hikma, Teva and Neurelis share most remaining brand value with hundreds of generic spray makers, a Douglas Insights estimate.

## How much does a nasal device and its medicine cost, from a saline spray to Spravato?

A nasal drug delivery unit earns its manufacturer an average USD 8.00 in 2025, Douglas Insights estimates, from under USD 2 for a generic saline or decongestant bottle to several hundred dollars for a branded systemic dose.

The price ladder is wide. Douglas Insights puts generic saline and decongestant bottles at USD 1 to USD 3 at manufacturer level, branded allergy steroid sprays at USD 4 to USD 25, migraine and seizure rescue units at USD 150 to USD 400, and epinephrine or glucagon two-packs at USD 150 to USD 350. Low-price allergy and congestion bottles make up about 88% of units but about 45% of value, a Douglas Insights estimate. Spravato alone is about 6.4% of 2025 value from a small fraction of units.

Average price rises 3.0% a year in the base case, to about USD 10.75 in 2035, mostly from mix. Systemic sprays such as Spravato and rescue products grow two to three times faster than allergy pumps, so the average rises even where list prices fall. Public naloxone shows the opposite pull: Emergent's 42% fall in quarterly naloxone revenue came from an "unfavourable" price and volume mix, its [2 November 2026 release](https://www.sec.gov/Archives/edgar/data/0001367644/000136764426000110/ebs202609-30ex99er.htm) states. Douglas Insights holds the pump itself at about USD 0.30 to USD 0.80 a unit, 5% to 10% of a branded allergy spray's factory price.

## What if rescue sprays scale faster or naloxone keeps shrinking: how large is the nasal drug delivery market in 2035?

The nasal drug delivery technology market ends 2035 between USD 38.5 billion and USD 57.8 billion across our three cases, around a base of USD 48.6 billion. Systemic drug uptake and allergy volume set most of the spread.

| Case | Device volume leg | Price leg | Revenue rate | 2035 value |
| --- | --- | --- | --- | --- |
| Slower | 1.80% | 2.00% | 3.84% | USD 38.5 billion |
| Base case | 3.20% | 3.00% | 6.30% | USD 48.6 billion |
| Faster | 4.40% | 3.60% | 8.16% | USD 57.8 billion |

In the slower case naloxone keeps falling, generic allergy sprays erode branded volume and only two new systemic sprays launch each year, taking 1.4 points off volume and 1.0 point off price. In the faster case Spravato, neffy and nasal vaccines all scale outside North America, adding 1.2 points of volume and 0.6 points of price. One extra point of yearly revenue growth adds about USD 4.77 billion to 2035 value, a Douglas Insights estimate.

Live signals sit near the base case. Spravato's 36% third-quarter growth, ARS' 49% rise in prescribers and Baqsimi's 46% gain point up. Emergent's 42% naloxone fall and Aptar's 1% core Pharma growth point down. Our 6.30% rate weighs fast systemic growth against flat allergy and congestion volume.

## Which rules shape nasal drug delivery, from FDA bioequivalence guidance to EU approvals?

Three rule sets shape nasal drug delivery: FDA product-specific bioequivalence guidance for generic sprays, FDA and European approvals that set which systemic drugs may use the nose, and self-administration labels that move products from clinics to homes.

Generic rules matter most for allergy sprays. The FDA's draft fluticasone propionate guidance, [notice 2025-09067 of 21 May 2025](https://www.federalregister.gov/d/2025-09067), sets the in vitro and clinical tests a generic must pass, and notes a pending Optinose citizen petition on generics that reference Xhance. Douglas Insights estimates such guidance cuts branded allergy spray prices by 30% to 60% within three years of generic entry.

Approval and label rules decide the systemic pipeline. The European Commission extended EURneffy to a 1 mg dose for children, the [31 March 2026 release](https://ir.alk-abello.com/news-releases/news-release-details/eurneffy-1-mg-approved-across-eu-first-and-only-needle-free) shows, after the FDA approved neffy 1 mg for children weighing 15 to 30 kg, ARS' [announcement of 5 March 2025](https://www.biospace.com/press-releases/ars-pharmaceuticals-announces-fda-approval-of-neffy-1-mg-epinephrine-nasal-spray-for-type-i-allergic-reactions-including-anaphylaxis-in-pediatric-patients-weighing-15-to-30-kilograms) states. FluMist self-administration was approved by the FDA in September 2024, AstraZeneca's [15 August 2025 release](https://www.businesswire.com/news/home/20250815310613/en) states, which turned a clinic vaccine into a home delivery product. Spravato remains a clinic-administered product under a US safety programme, which caps how fast patients can start, a Douglas Insights estimate.

| 5 November 2026 | ARS: neffy prescribers up 49% | Rescue volume |

The finding, a Douglas Insights estimate: systemic products, meaning central nervous system and pain, emergency rescue, vaccines and other systemic therapies, made up 30.0% of nasal value in 2025 but only about 6% of units. By 2035 their value share rises to about 41%, while allergy and congestion sprays fall from 70.0% to about 59%.

The ledger changes how to read the market. Device makers gain from systemic drugs because unit-dose and powder devices sell at several times the price of a multidose pump. Douglas Insights therefore forecasts the two groups separately, and weights pharma pipeline approvals more heavily than allergy season data in the price leg.

## How does the methodology turn 3.30 billion devices into USD 26.4 billion?

Our methodology multiplies about 3.30 billion nasal drug delivery units sold in 2025 by an average USD 8.00 of manufacturer revenue each to reach USD 26.4 billion, then grows units 3.2% and price 3.0% a year.

The model covers 6 therapy areas, 4 device formats and 5 regions, using 62 data points, 22 sourced and 40 modelled. Units come from patient counts multiplied by devices used per patient for each therapy; prices come from company filings and Douglas Insights price ladders. Arithmetic: 3.30 billion x USD 8.00 = USD 26.4 billion; USD 26.4 billion x (1.032 x 1.030)^10 = USD 48.6 billion. Units reach about 3.41 billion in 2026 and 4.52 billion in 2035.

Two cross-checks agree. First, four drug figures in our company table, Spravato, Emergent naloxone, Baqsimi and neffy, total about USD 2.2 billion, consistent with our USD 4.22 billion for central nervous system and emergency products once migraine, seizure and generic rescue sprays are added. Second, Aptar's roughly USD 1.8 billion yearly Pharma sales include about USD 900 million of nasal and consumer spray systems, a Douglas Insights estimate, close to 3.4% of our total, consistent with pumps costing 5% to 10% of branded factory prices. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our [editorial standards](https://www.douglasinsights.com/research-methodology/) explain.

## What counts in the nasal drug delivery study, and what is excluded?

The nasal drug delivery study counts nasal medicines and the pumps, unit-dose, bi-dose and powder devices that deliver them, valued at manufacturer prices worldwide. Inhalers for the lungs, nebulisers, nasal irrigation pots and surgical sinus implants are excluded.

The study splits value by 6 therapy areas, 4 device formats and 5 regions: North America, Europe, Asia Pacific, Latin America, and the Middle East and Africa.
