Essity calls itself the global market leader in casting, sells its fracture management range in about 150 countries and covers every layer of a cast: Delta-Cast and Delta-Lite synthetic tape, Gypsona plaster, Dynacast splints, Delta-Dry padding and Delta Thermo thermoplastics. Its reach into liners came on 21 December 2021, when [Essity bought AquaCast Liner](https://reports.essity.com/2021/annual-and-sustainability-report/group/acquisitions-investments-and-divestments.html), a Newark, Delaware maker of waterproof cast liners founded in 2012, at a price it called not material.

Solventum sells 3M Scotchcast Plus casting tape, which allows weight bearing within 20 minutes and comes in 13 colours; Douglas Insights puts its cast and splint share near 9.8%, or USD 277.6 million. Enovis, owner of the Aircast brand, sits close behind at an estimated 8.3%, or USD 235.1 million, once walkers and bracing outside our scope are stripped out.

## Why are fragility fractures in older adults lifting cast and splint volumes?

Orthopedic splint and cast applications grow 3.1% a year to 2035, from three forces. Ageing and fragility fractures add 1.4 points, rising trauma care in Asia Pacific adds 0.9 points and the outpatient shift to prefabricated splints for sprains adds 0.8 points, together the full volume leg.

### Ageing and fragility fractures add 1.4 points

The fracture pool is large and rising. The [World Health Organization (WHO)](https://www.who.int/news-room/fact-sheets/detail/fragility-fractures) counts 178 million new fractures worldwide in 2019, 33.4% more than in 1990, and 455 million people living with acute or long-term fracture symptoms, up 70.1%. Fractures caused 25.8 million years lived with disability that year, a 65.3% rise. WHO notes that wrist, hip, spine and upper arm fractures dominate among older people, especially older women, and the wrist is the classic cast fracture. Douglas Insights converts that trend into 1.4 points of yearly growth in orthopedic splint and cast applications, the largest single driver, because every distal forearm fracture in an older patient means a back slab, a full cast and often a removable splint afterwards.

### Trauma care in Asia Pacific adds 0.9 points

Hospital capacity is the second force. Road traffic and fall injuries in China, India and Southeast Asia are increasingly treated in hospitals with casting rooms rather than by traditional bone setters, and every patient moved into formal care becomes a buyer of casting tape or plaster. Douglas Insights places this effect at 0.9 points, which is why Asia Pacific orthopedic splint and cast revenue grows 6.4% a year in our model against 3.6% in North America. Synthetic tape gains most here, since new casting rooms tend to stock fiberglass from day one rather than plaster, and each of those rooms buys tape, padding and stockinette on a single contract with one of the six suppliers named above.

### Prefabricated splints for sprains add 0.8 points

The third force is the outpatient shift. Urgent care centres and physiotherapists now treat ankle sprains, scaphoid suspicions and stable wrist fractures with a prefabricated splint that a nurse fits in a few minutes. Enovis reported US Bracing and Support sales up 5.6% in the second quarter of 2026, the clearest public sign of that pull. Douglas Insights adds 0.8 points of application growth for it. The three contributions, 1.4, 0.9 and 0.8 points, add to the 3.1% volume leg, while price adds 1.2% a year as synthetics replace plaster.

## What stands in the way of orthopedic cast and splint volumes outside the fracture clinic?

Three restraints remove about 1.2 points from orthopedic splint and cast volume growth. Surgical plating of wrist fractures takes 0.5 points, tight budgets in low-income hospitals 0.4 points and soft bracing weakness 0.3 points; without them, applications would grow near 4.3% a year.

Surgical fixation removes 0.5 points. More unstable distal radius fractures are now fixed with a volar plate, which shortens or replaces the cast period; each plated wrist means one fewer full cast and often only a short splint. Budget limits remove 0.4 points. In many low-income hospitals one plaster roll still costs a fraction of a synthetic roll, and WHO data show the fracture burden growing fastest where health budgets are thinnest. Orthopedic splint and cast use rises with fractures there, but spend per application stays near the plaster floor of about USD 4.

Soft bracing weakness removes 0.3 points. Embla Medical's Bracing and Supports sales fell 1% organically in 2025 even as its group grew 6%, and Enovis's Prevention and Recovery segment fell 0.8% on a reported basis in its latest quarter after divestitures. Prefabricated splints share that channel, so slow reimbursement for off-the-shelf orthoses drags on volume.

North America leads orthopedic splint and cast spending with USD 985.8 million in 2025, or 34.8%, because synthetic casts and prefabricated splints dominate its emergency departments. Asia Pacific grows fastest at 6.4% a year, from USD 753.6 million to USD 1.40 billion by 2035, as formal trauma care spreads.

North America reaches USD 1.40 billion by 2035 at 3.6% a year. Middle East and Africa is the smallest region at USD 127.5 million and grows 3.75% a year to USD 184.2 million, held near plaster prices by public tenders.

The average rises 1.2% a year because the mix moves to synthetics, not because list prices climb. A hospital that swaps a USD 6 plaster cast for a USD 24 fiberglass cast quadruples its materials spend on that patient while saving on cast changes. Waterproof liners and coloured tapes push the top of the synthetic band; Scotchcast Plus alone comes in 13 colours. Group purchasing contracts in North America and public tenders in Europe cap the top of each band, and prices for the same roll can differ by a third between a tendered hospital and a small clinic.

Hospitals lead because the first cast follows the first X-ray, usually in an emergency department. Orthopedic splint and cast components sold in the US are Class I devices: 21 CFR 888.5940 covers cast heels, toe caps, cast supports and walking irons and exempts them from premarket notification. In the European Union (EU), Regulation (EU) 2023/607 entered into force on 20 March 2023 to extend MDR transition periods.

The [US rule](https://www.ecfr.gov/current/title-21/chapter-I/subchapter-H/part-888/subpart-F/section-888.5940) also exempts these devices from most current good manufacturing practice requirements, keeping only records and complaint files. The consequence is a low entry barrier: a new casting or splint supplier needs a listing and a quality file, not a clearance, so private-label tape competes directly with branded tape. That cap on pricing power is part of why the price leg is only 1.2% a year.

The slower case runs volume at 1.9% and price at 0.6%: volar plating spreads further, plaster keeps its share in Asia Pacific and bracing reimbursement tightens, as Embla's 1% organic decline in Bracing and Supports already hints.
