## Key takeaways

- The smart orthopedic implants market is worth USD 36.0 million in 2025 and USD 252.8 million in 2035, a 21.52% CAGR.
- About 20,000 procedures at USD 1,800 form the 2025 base; procedures grow 24.0% a year while value per procedure falls 2.0%.
- Smart knee tibial extensions hold 70.0% of value; smart spine, trauma and other implants grow fastest at 40.0%.
- North America uses 95.0% of value because the only cleared smart knee is sold mainly in the United States.
- AAOS 2026 claims data show 0.4% revision against 1.1%, but the label still says no clinical benefit has been shown.

Patients fitted with a smart implantable knee device and a digital care platform had a one-year revision rate of 0.4% against 1.1% for 3,484 matched controls, and a periprosthetic joint infection rate of 0.2% against 1.1%, an [American Academy of Orthopaedic Surgeons (AAOS) 2026 abstract](https://index.mirasmart.com/AAOS2026/PDFfiles/AAOS2026-007516.pdf) of 1,231 patients reports. Zimmer Biomet, which sells the only cleared smart knee, still calls it "the first and only smart knee implant" in its [AAOS 2026 release of 25 February 2026](https://investor.zimmerbiomet.com/news-and-events/news/2026/02-25-2026-120701913). The smart orthopedic implants market covers implants that carry their own sensors, battery and radio, chiefly tibial extensions for total knee arthroplasty, plus smart spinal cages and trauma devices in development, and the data platforms and remote monitoring sold with them. Douglas Insights sizes it at USD 36.0 million in 2025, from about 20,000 smart implant procedures at USD 1,800 each, and at USD 252.8 million in 2035, a revenue CAGR (compound annual growth rate) of 21.52%. The study sits inside our wider [Medical Implants Market](https://www.douglasinsights.com/medical-implants-market/) report, beside our [Orthopedic Trauma Implants Market](https://www.douglasinsights.com/orthopedic-trauma-implants-market/) and [Sacroiliac Joint Fusion Market](https://www.douglasinsights.com/sacroiliac-joint-fusion-market/) studies in our [medical devices coverage](https://www.douglasinsights.com/industry/healthcare/medical-devices/), and every figure follows the Douglas Insights [research methodology](https://www.douglasinsights.com/research-methodology/).

## Which smart orthopedic implant products earn the most: knee extensions, data platforms or spine cages?

Smart knee tibial extensions earn the most, 70.0% of 2025 value or USD 25.2 million, a Douglas Insights estimate. Smart spine, trauma and other implants grow fastest, 40.0% a year, from USD 2.16 million to USD 62.5 million.

| Product | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Smart knee tibial extensions | 70.0% | USD 25.2 million | 17.0% | USD 121.1 million |
| Data platforms and remote monitoring | 24.0% | USD 8.64 million | 23.0% | USD 68.5 million |
| Smart spine, trauma and other implants | 6.0% | USD 2.16 million | 40.0% | USD 62.5 million |

Smart knee tibial extensions lead at USD 25.2 million because they are the only sensor-bearing orthopedic implant on sale in volume. The Canary Tibial Extension, fitted in the stem of a Persona knee, measures gait, step count and range of motion and sends the readings to a base station in the patient's home. We count about 18,800 knee extensions in 2025 at about USD 1,340 each, Douglas Insights estimates. The product grows 17.0% a year to USD 121.1 million, slower than the market, because new cleared devices from other makers will widen the field beyond the knee after 2028.

Data platforms and remote monitoring earn USD 8.64 million, 24.0% of value, or about USD 432 per smart procedure, a Douglas Insights estimate. This covers the cloud software that turns sensor readings into reports for surgeons and the remote therapeutic monitoring that clinics bill while they watch recovery. The product grows 23.0% a year to USD 68.5 million as more clinics bill monitoring and as each implant keeps reporting for years after surgery. Smart spine, trauma and other implants hold USD 2.16 million, 6.0% of value, mostly devices supplied to clinical studies and early-access sites; they grow 40.0% a year to USD 62.5 million if a smart spinal cage clears the Food and Drug Administration ([FDA](https://www.fda.gov/)) late this decade. Product values for 2035 sum to USD 252.1 million, 0.28% below the USD 252.8 million headline, inside our 2.5% tolerance.

### Spend by care setting and joint

By care setting, hospital inpatient and outpatient departments carry about 64% of 2025 value and ambulatory surgery centres about 36%, Douglas Insights estimates; the centres gain share as more knee replacements leave the hospital. By joint, the knee holds about 94% of value and the spine about 4%, with trauma, hip and shoulder devices in the remaining 2%.

## What drives smart orthopedic implant demand up 21.52% a year?

Smart orthopedic implant spend grows 21.52% a year because procedures using smart implants rise 24.0% a year while the average price per procedure falls 2.0%, from USD 1,800 in 2025 to about USD 1,470 in 2035, Douglas Insights estimates.

Three forces lift procedure volume, all Douglas Insights estimates unless a source is linked. Outcome evidence adds about 11.0 points a year, the growing base of joint replacements about 5.0 points, and remote monitoring billing and digital care platforms about 8.0 points; together, compounded, they give the 24.0% volume leg. Price falls 2.0 points a year as batteries, radios and sensor packages get cheaper and as a second supplier enters.

### Outcome evidence: about 11.0 points

Evidence on outcomes adds about 11.0 points a year. In the claims study presented at AAOS 2026, 1,231 patients with a smart implantable device and digital care platform had a one-year revision rate of 0.4% against 1.1% for 3,484 matched controls, a periprosthetic joint infection rate of 0.2% against 1.1%, and opioid prescription fills of 23.9% against 28.7%, the [abstract](https://index.mirasmart.com/AAOS2026/PDFfiles/AAOS2026-007516.pdf) reports. A Zimmer Biomet executive cautioned that "we can't really say that there is causality in this", [MedTech Dive reported on 6 March 2026](https://www.medtechdive.com/news/zimmer-biomet-shares-smart-knee-data-at-aaos/814055/). A revision knee costs a hospital many times the price of a sensor, so even a modest fall in revisions makes the case to hospital buyers, and each new dataset widens the group of surgeons willing to try the device.

### The joint replacement base: about 5.0 points

The procedure base adds about 5.0 points. OECD countries performed an average of 156 knee replacements and 198 hip replacements per 100,000 people in 2023, and rates rose over the decade in every member except New Zealand and Mexico, the OECD's [Health at a Glance 2025](https://www.oecd.org/fr/publications/2025/11/health-at-a-glance-2025_a894f72e/full-report/hip-and-knee-replacement_a4d0ff34.html) shows. The UK National Joint Registry recorded 161,458 knee and 173,568 hip procedures in 2025, its [StatsOnline data](https://surgeonprofile.njrcentre.org.uk/Home/StatsIndex) show, and the American Joint Replacement Registry reported an 8.8% rise in submitted procedures in its [2025 annual report of 23 October 2025](https://www.aaos.org/aaos-home/newsroom/press-releases/ajrr-annual-report-press-release/). Stryker's knee sales rose 8.5% to USD 2.66 billion in 2025, its [full-year results of 29 January 2026](https://s22.q4cdn.com/857738142/files/doc_financials/2025/q4/26-02.pdf) show. Every new knee replacement is a candidate for a smart stem, and the smart share of that base is still well under 2%, a Douglas Insights estimate.

### Remote monitoring billing and digital care: about 8.0 points

Billing and digital care add about 8.0 points. From 1 January 2026 the Centers for Medicare & Medicaid Services (CMS) recognises new remote therapeutic monitoring code 98985 for musculoskeletal device supply over 2 to 15 days in a 30-day period and code 98979 for the first 10 minutes of treatment management, alongside codes 98975 to 98981, its [transmittal MM14250 of 24 November 2025](https://www.cms.gov/files/document/mm14250-therapy-code-list-2026-annual-update.pdf) lists. Shorter monitoring windows let clinics bill for patients who wear or carry a device for only part of a month, which suits a sensor that reports every day. Robotic total knee arthroplasty was "approaching 16.1%" of cases in 2024, the AJRR's [2025 report highlights](https://www.aaos.org/globalassets/registries/1.29.26_highlights-from-the-ajrr-2025-annual-report.pdf) state; hospitals that already run robots and app-based recovery programmes find it easier to add an implant that sends data. Together, 1.11 x 1.05 x 1.064 minus 1 gives the 24.0% volume leg, and (1.24 x 0.98) minus 1 gives the 21.52% revenue rate.

## What holds back smart orthopedic implants: unproven benefit, soft knee sales or recalls?

Three restraints slow smart orthopedic implants: labelling that says the data have not been shown to help patients, weak knee implant sales in 2026, and the added work of recalls, batteries and device security, Douglas Insights estimates.

Unproven benefit is the largest brake. Zimmer Biomet's own [release of 10 March 2025](https://investor.zimmerbiomet.com/news-and-events/news/2025/03-10-2025-113043622), which launched a 30 mm stem version, states that the kinematic data from the Canary Canturio tibial extension "are not intended to support clinical decision-making and have not been shown to provide any clinical benefit". Until a randomised trial shows better outcomes, many surgeons and payers will treat the sensor as an optional extra, which takes about 4.0 points a year off procedure growth on our estimate. Observational claims data help, but they cannot rule out that healthier patients were chosen for the device.

Soft knee sales take about 1.5 points. Zimmer Biomet's knee sales fell 3.1% to USD 767.5 million in the third quarter of 2026, its [results of 4 November 2026](https://www.sec.gov/Archives/edgar/data/0001136869/000119312526446145/zbh-ex99_1.htm) show, and Smith+Nephew said "US Knee Implants revenue declined -7.3%" in the same quarter, its [trading report of 4 November 2026](https://www.sec.gov/Archives/edgar/data/0000845982/000110465926122324/snn-20261104x6k.htm) states. When the only seller of smart knees is losing knee share, the smart extension reaches fewer operating rooms.

Recalls, batteries and security take the last 1.0 point. The FDA posted Class 2 recall Z-2099-2025 of 106 Canturio Smart Extension units on 8 July 2025 because the product was "incorrectly labeled", its [recall database](https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfres/res.cfm?id=213897) shows. Each implant carries a battery with a finite reporting life and a radio that must meet cybersecurity rules, which adds cost and paperwork for every new design. These restraints are already netted into the 24.0% volume leg and the minus 2.0% price leg above.

## Where are smart orthopedic implants used most: North America, Europe or Asia Pacific?

North America uses the most, USD 34.2 million or 95.0% of 2025 smart orthopedic implant value, a Douglas Insights estimate, because the only cleared smart knee is sold mainly in the United States. Asia Pacific grows fastest, 36.0% a year.

| Region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- | --- |
| North America | USD 34.2 million | 95.0% | USD 41.2 million | 20.5% | USD 220.7 million |
| Europe | USD 1.08 million | 3.0% | USD 1.41 million | 31.0% | USD 16.1 million |
| Asia Pacific | USD 0.54 million | 1.5% | USD 0.73 million | 36.0% | USD 11.7 million |
| Latin America | USD 0.11 million | 0.3% | USD 0.14 million | 30.0% | USD 1.5 million |
| Middle East and Africa | USD 0.07 million | 0.2% | USD 0.09 million | 30.0% | USD 1.0 million |

North America leads because FDA clearance came first and because United States hospitals can bill remote monitoring codes that most other health systems lack. It grows 20.5% a year to USD 220.7 million, slightly below the world rate as other regions start from a tiny base. Europe, USD 1.08 million, mostly clinical-study and early-access supply, grows 31.0% a year to USD 16.1 million; its large joint replacement volumes in Germany, Switzerland, Finland and Denmark, which the OECD lists among the highest rates, give it room once devices carry a CE mark under the Medical Device Regulation.

Asia Pacific, USD 0.54 million, grows fastest at 36.0% a year to USD 11.7 million on Australia's high knee replacement rate and on Japanese, Korean and Chinese hospitals that already run robotic knee programmes, Douglas Insights estimates. Latin America and the Middle East and Africa hold USD 0.11 million and USD 0.07 million and grow about 30.0% a year as private hospitals in Brazil, Mexico, Saudi Arabia and the United Arab Emirates buy premium knees. Regional values sum to USD 43.6 million in 2026, 0.35% below the USD 43.7 million headline, and USD 251.0 million in 2035, 0.72% below it.

## Who leads smart orthopedic implants: Zimmer Biomet, Canary Medical or a new entrant?

Zimmer Biomet, selling devices made by Canary Medical, holds about 92.0% of 2025 smart orthopedic implant value, USD 33.1 million, Douglas Insights estimates. Canary's own spine programmes and early-stage developers share the rest.

| Company | What the position rests on | Estimated share 2025 | Estimated value 2025 |
| --- | --- | --- | --- |
| Zimmer Biomet (with Canary Medical) | Persona IQ knee with the Canary tibial extension and the mymobility care platform | 92.0% | USD 33.1 million |
| Canary Medical (direct programmes) | Canturio LC smart spine cartridge, NanoHive smart cage partnership, sensor licensing | 4.0% | USD 1.44 million |
| Other developers | Intelligent Implants (SmartFuse), NanoHive Medical, university and hospital programmes | 4.0% | USD 1.44 million |

The shares are Douglas Insights estimates because no company reports smart implant revenue on its own. Zimmer Biomet's 2025 net sales were USD 8.23 billion, up 7.2%, and its knee sales USD 3.32 billion, up 4.7%, with USD 1.87 billion in the United States, its [full-year release of 10 February 2026](https://www.sec.gov/Archives/edgar/data/1136869/000119312526043620/zbh-ex99_1.htm) shows; on our estimate, smart extensions are under 1% of those knee sales. The FDA granted Canary Medical's De Novo request DEN200064 on 27 August 2021, its [De Novo database](https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/denovo.cfm?id=DEN200064) shows, and only four records exist under the new product code, all Canary's.

Canary Medical and NanoHive Medical signed an exclusive partnership on 20 April 2026 to develop and sell smart spinal cages that pair NanoHive's soft titanium cage with Canary sensing, [MassDevice reported](https://www.massdevice.com/nanohive-canary-partner-smart-spinal-cages/). Intelligent Implants holds an FDA Breakthrough Device designation for its SmartFuse spinal fusion system, [Becker's Spine Review reported](https://www.beckersspine.com/spinal-tech/intelligent-implants-smartfuse-gets-fda-clearance-for-spinal-fusions/). Stryker, Smith+Nephew and Johnson & Johnson MedTech, whose DePuy Synthes orthopaedics business had about USD 9.2 billion of 2024 sales and is being separated, its [release of 14 October 2025](https://www.jnj.com/media-center/press-releases/johnson-johnson-announces-intent-to-separate-its-orthopaedics-business) states, sell no implanted sensor today; Stryker's VERASENSE sensor measures knee balance during surgery and is then removed. Smith+Nephew's knee implant revenue was USD 1.01 billion in 2025, its [results of 2 March 2026](https://www.sec.gov/Archives/edgar/data/845982/000110465926021793/snn-20260302x6k.htm) show. Any of the three could license or buy sensor technology, which is the largest risk to Zimmer Biomet's lead.

## What does a smart knee sensor cost, and what do hospitals pay per procedure?

A smart orthopedic implant procedure costs about USD 1,800 on average in 2025, a Douglas Insights estimate, about USD 1,340 for the sensor hardware and USD 432 for data and monitoring. Medicare's past add-on payment implies a sensor cost of at least USD 1,309.

| Item | Value | Basis |
| --- | --- | --- |
| Medicare add-on payment cap, one knee | USD 850.85 | CMS new technology add-on payment, fiscal 2024 |
| Medicare add-on payment cap, both knees | USD 1,701.70 | CMS new technology add-on payment, fiscal 2024 |
| Implied minimum sensor cost | USD 1,309 | USD 850.85 divided by the 65% add-on rate |
| Average sensor hardware value 2025 | USD 1,340 | Douglas Insights estimate |
| Average data and monitoring value 2025 | USD 432 | Douglas Insights estimate |
| Blended value per procedure 2035 | USD 1,470 | Douglas Insights estimate, minus 2.0% a year |

The add-on payment caps of USD 850.85 for one knee and USD 1,701.70 for both, which covered discharges from 1 October 2023 to 30 September 2024, come from Zimmer Biomet's [2024 inpatient reimbursement update](https://www.zimmerbiomet.com:443/content/dam/zb-corporate/en/support/reimbursement/4413.1-US-en%202024%20Inpatient%20Reimbursement%20Update.pdf); Medicare pays at most 65% of a new technology's cost, so the cap implies a cost of at least USD 1,309. Since that payment expired, hospitals absorb the sensor inside the standard joint replacement payment, which is why the hardware price is under pressure. We model the blended value per procedure at USD 1,764 in 2026 and USD 1,470 in 2035. A fall of USD 100 in the average value per procedure would cut 2035 market value by about USD 17.2 million at our 2035 procedure count.

## Which rules govern smart orthopedic implants: the De Novo pathway, cybersecurity law or medical device regulation transition deadlines?

Three rule sets govern smart orthopedic implants: the FDA's De Novo classification that created product code QPP, the FDA's 2025 cybersecurity guidance for connected devices, and the extended transition deadlines of the European Union Medical Device Regulation.

The FDA's De Novo grant DEN200064 of 27 August 2021 placed implantable post-surgical kinematic measurement knee devices under 21 CFR 888.3600 as Class II devices with special controls, the [De Novo record](https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/denovo.cfm?id=DEN200064) shows. Later versions followed by 510(k), including K234056 for the canturio se smart extension on 24 April 2024, the [510(k) record](https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfPMN/pmn.cfm?ID=K234056) shows. A rival can now cite Canary's device as a predicate, which lowers the cost of entry. Smart spine and trauma devices outside the knee still need their own classification.

Cybersecurity is the second layer. The FDA published final guidance on "Cybersecurity in Medical Devices: Quality System Considerations and Content of Premarket Submissions" on 27 June 2025, covering cyber devices under section 524B of the Federal Food, Drug, and Cosmetic Act, the [Federal Register notice](https://www.govinfo.gov/content/pkg/FR-2025-06-27/html/2025-11669.htm) shows; every implant that sends data by radio needs a software bill of materials and a plan for patches. In Europe, Regulation (EU) 2023/607 extended the Medical Device Regulation transition to 31 December 2027 for class III and implantable class IIb devices and to 31 December 2028 for other classes, the [Official Journal text](https://eur-lex.europa.eu/eli/reg/2023/607/oj) sets out. Notified body reviews of new smart implants will queue behind thousands of legacy devices until then.

## What if evidence firms up or stalls: where do smart orthopedic implants land in 2035?

The smart orthopedic implants market ends 2035 between USD 107.4 million and USD 484.6 million across our three cases, around a base of USD 252.8 million. Trial evidence and a second supplier set most of the spread.

| Case | Procedure leg | Value per procedure leg | Revenue rate | 2035 value |
| --- | --- | --- | --- | --- |
| Slower | 15.0% | -3.0% | 11.55% | USD 107.4 million |
| Base case | 24.0% | -2.0% | 21.52% | USD 252.8 million |
| Faster | 31.0% | -1.0% | 29.69% | USD 484.6 million |

In the slower case no randomised trial reports a benefit, Zimmer Biomet keeps losing knee share and no smart spinal cage is cleared before 2032, taking 9.0 points off procedures and 1.0 point off value per procedure. In the faster case a large trial confirms fewer revisions, a smart spinal cage reaches the market by 2029 and a second knee maker launches a sensor stem, adding 7.0 points to procedures and 1.0 point to value. One extra point of yearly revenue growth adds about USD 21.6 million to 2035 value; one point less removes about USD 20.0 million. Even the faster case leaves smart implants under 5% of world knee implant sales in 2035, a Douglas Insights estimate.

## Douglas Exclusive: the Smart Knee Evidence Ledger

The Smart Knee Evidence Ledger is a Douglas Insights model built from 9 inputs in 8 regulator, payer and company documents dated August 2021 to November 2026. It tracks whether the evidence and payment base for smart implants is growing faster than the knee market around them.

| Date | Input | What it moves |
| --- | --- | --- |
| 27 August 2021 | FDA De Novo DEN200064, product code QPP | Entry pathway |
| 24 April 2024 | 510(k) K234056 for canturio se | Product range |
| 10 March 2025 | 30 mm stem launch; no shown clinical benefit | Surgeon uptake |
| 8 July 2025 | Recall Z-2099-2025, 106 units | Supply risk |
| 27 June 2025 | FDA final cybersecurity guidance | Design cost |
| 24 November 2025 | CMS codes 98985 and 98979 from 1 January 2026 | Monitoring billing |
| 10 February 2026 | Zimmer Biomet 2025 knees USD 3.32 billion, up 4.7% | Host market |
| March 2026 | AAOS 2026 revision 0.4% against 1.1% | Evidence |
| 4 November 2026 | Zimmer Biomet third-quarter knees down 3.1% | Host market |

The finding, a Douglas Insights estimate: the evidence and payment inputs have moved in the device's favour since 2025, while the host knee market has softened. Revision and infection gaps of 0.7 and 0.9 percentage points in the AAOS 2026 claims data, if they held, would avoid about 7 revisions and 9 infections per 1,000 smart knees. Against that, Zimmer Biomet's knee sales fell 3.1% in the third quarter of 2026 while Stryker's grew 1.1%, its [results of 29 October 2026](https://s22.q4cdn.com/857738142/files/doc_financials/2026/q3/SYK-Ex-99-1-Earnings-Q3-2026.pdf) show, so the smart extension is gaining evidence inside a shrinking share of operating rooms.

The ledger changes how to read the market. Growth now depends less on whether surgeons like the data and more on whether the sensor reaches implants other than Persona, through licensing, a second supplier or smart spinal cages. A spine programme such as the NanoHive partnership opens a larger field, since the partners cite more than one million spinal fusions a year worldwide; the knee alone keeps the market below USD 200 million in 2035 on our model.

## How does the methodology turn 20,000 procedures into USD 36.0 million of smart orthopedic implants?

Our methodology multiplies about 20,000 smart implant procedures worldwide in 2025 by USD 1,800 of sensor hardware and data value per procedure to reach USD 36.0 million. Procedures are a Douglas Insights estimate from knee sales, registry volumes and published patient cohorts.

The model covers 5 regions, 3 products, 2 care settings and 3 joint groups, and uses 84 data points, 19 sourced and 65 modelled. Arithmetic: 20,000 x USD 1,800 = USD 36.0 million; USD 36.0 million x (1.24 x 0.98)^10 = USD 252.8 million. Procedures reach about 24,800 in 2026 and 171,900 in 2035; value per procedure falls to USD 1,764 in 2026; 2026 value is USD 43.7 million.

Two cross-checks agree. First, the AAOS 2026 claims study found 1,231 smart-device patients in a single commercial claims dataset, consistent with tens of thousands of procedures since launch once Medicare and uninsured cases are added. Second, our USD 25.2 million of knee extension value is 0.76% of Zimmer Biomet's USD 3.32 billion of 2025 knee sales, in line with a premium option used in a small share of its cases. ## What counts in the smart orthopedic implants market, and what is excluded?

The smart orthopedic implants market counts implants that stay in the body and carry their own sensors, power and communication, plus the data and monitoring services sold with them. Conventional implant components, intraoperative sensors and wearables are excluded.

The by-product split covers smart knee tibial extensions, data platforms and remote monitoring, and smart spine, trauma and other implants; the by-care-setting split covers hospitals and ambulatory surgery centres; the by-region split covers five regions. Value is counted at the price the hospital pays for the sensor element and the monitoring service, not for the femoral, tibial or insert components of the knee, which sit in our wider [Medical Implants Market](https://www.douglasinsights.com/medical-implants-market/) report. Stryker's VERASENSE, removed after surgery, and stand-alone wearables are left out.
