HER2-targeted antibodies and ADCs, meaning trastuzumab, pertuzumab, their subcutaneous combination and the trastuzumab-based conjugates, earn USD 15.19 billion because HER2 testing is universal and treatment runs for years in early disease. Their growth is the slowest of the big classes at 3.54%, as biosimilar trastuzumab drags down the oldest products. CDK4/6 inhibitors hold USD 14.76 billion, a 43.9% share, since ribociclib, abemaciclib and palbociclib are the default first-line partner for endocrine therapy in HR-positive HER2-negative disease; they grow 4.36% a year to USD 22.61 billion.

TROP2 antibody-drug conjugates are the fastest class: USD 1.78 billion in 2025 rising at 12.84% to USD 5.96 billion. Two products, sacituzumab govitecan and datopotamab deruxtecan, are moving from later lines into first-line triple-negative breast cancer. PI3K/AKT/mTOR pathway inhibitors, led by AstraZeneca's Truqap, account for USD 1.28 billion and grow 10.18% as tumour genotyping widens the eligible pool. PARP inhibitors are the smallest class at USD 605.1 million, a 1.8% share, and grow only 2.71% because they serve the narrow germline BRCA-mutated group.

Short verdict: two classes, 89.1% of value. The other three supply almost all of the new growth.

## Why are CDK4/6 and HER2 antibody-drug conjugate patient-years growing faster than breast cancer incidence?

Treated patient-years in targeted therapy for breast cancer grow 7.37% a year to 2035, about five times faster than new diagnoses. Adjuvant CDK4/6 use, first-line HER2 ADCs and wider Asian access all lengthen or add treatment years. Four parts, measured in points.

Adjuvant CDK4/6 therapy adds 2.84 points. Novartis reported Kisqali fourth-quarter sales of USD 1.32 billion, up 44% at constant currency, and credited strong momentum from the early breast cancer indication as well as share gains in metastatic disease ([Novartis](https://www.novartis.com/news/media-releases/novartis-delivered-high-single-digit-sales-growth-achieved-40-core-margin-and-further-advanced-pipeline-2025)). An adjuvant patient adds treatment years before any relapse. That is new volume. Eli Lilly, which sells abemaciclib in the same adjuvant setting, booked Verzenio sales of USD 5.72 billion, up 8%, with ex-US sales up 18% in the fourth quarter.

First-line HER2 antibody-drug conjugates add 1.63 points. In DESTINY-Breast09, 1,157 adults with untreated HER2-positive metastatic disease received Enhertu plus pertuzumab or the taxane, trastuzumab and pertuzumab regimen; median progression-free survival reached 40.7 months against 26.9 months, a hazard ratio of 0.56 ([FDA](https://www.fda.gov/drugs/drug-approvals-and-databases/fda-approves-fam-trastuzumab-deruxtecan-nxki-pertuzumab-unresectable-or-metastatic-her2-positive)). The FDA approved that combination on 15 December 2025. Every extra month without progression is an extra month of paid targeted therapy, and our model adds about 13.8 months of HER2 therapy per first-line patient by 2030. Enhertu already had combined global sales of USD 4.98 billion, with the AstraZeneca share up 40% to USD 2.78 billion ([AstraZeneca full-year 2025 results](https://www.astrazeneca.com/content/dam/az/PDF/2025/Q4-FY/Full-year-Q4-2025-results-announcement.pdf)).

Wider access in Asia and other emerging markets adds 1.63 points. AstraZeneca lists a Chinese approval of Enhertu for chemotherapy-naive HER2-low and HER2-ultralow metastatic breast cancer ([AstraZeneca](https://www.astrazeneca.com/content/dam/az/PDF/2025/Q4-FY/Full-year-Q4-2025-results-announcement.pdf)). China, India and Southeast Asia together hold the largest untreated pool in our model: Asia Pacific logs 196,349 patient-years against an estimated 690,000 eligible women.

Incidence adds the last 1.27 points. The International Agency for Research on Cancer counts about 2.3 million new breast cancer cases a year and projects 3.2 million by 2050, a 39% rise that works out to roughly 1.3% a year. Each newly diagnosed HER2-positive or HR-positive woman in a reimbursed system becomes a candidate for at least one targeted drug.

The four parts sum to 7.37 points: 2.84 plus 1.63 plus 1.63 plus 1.27. Treatment duration, not diagnosis counts, is the real engine of targeted breast cancer volume, and it carries patient-years from 647,200 in 2025 to about 1.32 million in 2035.

## Which headwinds pull targeted breast cancer drug revenue below patient growth once palbociclib loses exclusivity?

Price erosion of 2.26 points a year is the main restraint on targeted therapy for breast cancer, split between generic palbociclib, HER2 biosimilars and Medicare negotiation. A fourth headwind, trial misses in new settings, threatens the volume leg rather than price.

Generic palbociclib removes 1.14 points. Ibrance revenue fell 6% to USD 4.12 billion, including USD 2.71 billion in the United States, down 5%, and USD 1.41 billion internationally, down 7% ([Pfizer](https://s206.q4cdn.com/795948973/files/doc_financials/2025/q4/Q4-2025-PFE-Earnings-Release.pdf)). Once generics arrive in the US, Douglas Insights models the realised palbociclib price falling by about 70% within three years.

HER2 biosimilars remove 0.71 points. Roche reported Herceptin sales down 22% at constant exchange rates to CHF 1.03 billion and Perjeta down 13% to CHF 2.97 billion, while the subcutaneous Phesgo grew 48%. Volume survives. Price does not. Medicare negotiation removes the last 0.41 points: the negotiated price for Ibrance is USD 7,871 per 30-day supply in 2027, half the USD 15,741 list price of 2024 ([CMS](https://www.cms.gov/files/document/fact-sheet-negotiated-prices-ipay-2027.pdf)).

Trial risk sits on the volume side. Gilead reported that ASCENT-07, testing Trodelvy against chemotherapy in first-line HR-positive HER2-negative metastatic disease, did not meet its progression-free survival endpoint. Our slower case assumes more such misses and cuts targeted breast cancer volume growth from 7.37% to 5.12%, removing 2.25 points.

## How much does a year of palbociclib, ribociclib or trastuzumab therapy cost payers?

A year of targeted breast cancer therapy cost payers USD 51,940 on average in 2025, by Douglas Insights estimate, net of rebates and across all regions. US list prices sit far higher: palbociclib lists at USD 15,741 per 30-day supply, about USD 188,900 a year.

The CMS fact sheet on negotiated prices for 2027 shows Ibrance with a 2024 list price of USD 15,741 for 30 days and a negotiated maximum fair price of USD 7,871, a 50% discount ([CMS](https://www.cms.gov/files/document/fact-sheet-negotiated-prices-ipay-2027.pdf)). Ibrance accounted for about USD 2.04 billion of gross Part D drug costs for roughly 16,000 Medicare enrollees in 2024, or about USD 127,500 per enrollee. It is the clearest public benchmark. Nothing else comes close.

Realised prices differ sharply by region. Douglas Insights puts the average North American net price at USD 104,600 per patient-year, against USD 46,900 in Europe, USD 32,700 in Asia Pacific, USD 24,400 in the Middle East and Africa and USD 18,600 in Latin America. The blended price falls 2.26% a year to about USD 41,300 by 2035 as generic palbociclib and biosimilar trastuzumab spread, partly offset by higher-priced antibody-drug conjugates.

Douglas Insights estimates Roche holds a 27.8% share of 2025 targeted therapy for breast cancer revenue through its HER2 franchise, and the top three companies, Roche, Eli Lilly and Novartis, hold 59.0%. Shares are anchored on reported product sales and our breast-only attribution.

### CDK4/6 sellers: Eli Lilly, Novartis and Pfizer

Eli Lilly leads CDK4/6 inhibitors on revenue with Verzenio at USD 5.72 billion, about 17.0% of the market, built on adjuvant and metastatic use. Novartis ranks next with Kisqali at USD 4.78 billion, or 14.2%, and is the fastest grower of the three ([Novartis](https://www.novartis.com/news/media-releases/novartis-delivered-high-single-digit-sales-growth-achieved-40-core-margin-and-further-advanced-pipeline-2025)). Pfizer holds USD 4.12 billion, 12.3%, with Ibrance, the CDK4/6 brand most exposed to generics ([Pfizer](https://s206.q4cdn.com/795948973/files/doc_financials/2025/q4/Q4-2025-PFE-Earnings-Release.pdf)).

### HER2 and ADC sellers: Roche, AstraZeneca with Daiichi Sankyo, and Gilead

Roche owns the HER2 base: Phesgo at CHF 2.44 billion, Kadcyla at CHF 2.03 billion, Perjeta at CHF 2.97 billion and Herceptin at CHF 1.03 billion. AstraZeneca and Daiichi Sankyo co-market Enhertu, whose combined global sales reached USD 4.98 billion, and Datroway, at USD 218 million; AstraZeneca also sells Truqap, up 68% to USD 728 million ([AstraZeneca](https://www.astrazeneca.com/content/dam/az/PDF/2025/Q4-FY/Full-year-Q4-2025-results-announcement.pdf)). The 15 December 2025 first-line approval for Enhertu with pertuzumab moves AstraZeneca and Daiichi Sankyo directly into the setting where Roche earns most. Gilead sells Trodelvy, at USD 1.4 billion and up 6%, mostly in triple-negative disease.

North America rises from USD 16.24 billion to USD 23.71 billion at only 3.86%, the slowest pace, because generic palbociclib and Medicare negotiation hit there first; Pfizer earned 65.7% of Ibrance revenue in the US ([Pfizer](https://s206.q4cdn.com/795948973/files/doc_financials/2025/q4/Q4-2025-PFE-Earnings-Release.pdf)). Europe grows from USD 8.71 billion to USD 13.15 billion at 4.21%, with Phesgo and biosimilar tenders pulling in opposite directions. Asia Pacific climbs from USD 6.42 billion to USD 13.38 billion at 7.62%, driven in our model by broader Chinese reimbursement for HER2 and CDK4/6 drugs and by Japan, home of Daiichi Sankyo.

Latin America grows from USD 1.38 billion to USD 2.46 billion at 5.97% as public payers in our model add CDK4/6 inhibitors. The Middle East and Africa moves from USD 874.1 million to USD 1.76 billion at 7.23%. The wildcard is the Gulf: our model prices Gulf purchases near European levels, so a few hundred extra patients there shift the regional total visibly.

## Why does the TROP2 antibody-drug conjugate class matter most for triple-negative breast cancer?

TROP2 antibody-drug conjugates gave triple-negative breast cancer its first first-line targeted option in May 2026. Douglas Insights expects the class to grow from USD 1.78 billion to USD 5.96 billion by 2035, the fastest rise of any drug class, because triple-negative disease had no other target.

The FDA approved datopotamab deruxtecan, sold as Datroway, on 22 May 2026 for unresectable or metastatic triple-negative breast cancer in patients who are not candidates for PD-1 or PD-L1 inhibitors ([FDA](https://www.fda.gov/drugs/resources-information-approved-drugs/fda-approves-datopotamab-deruxtecan-dlnk-unresectable-or-metastatic-triple-negative-breast-cancer)). In TROPION-Breast02, which enrolled 644 untreated patients, median overall survival reached 23.7 months against 18.7 months with chemotherapy, and median progression-free survival 10.8 months against 5.6 months. Hazard ratios were 0.79 for survival and 0.57 for progression.

Trodelvy already serves later-line triple-negative and HR-positive disease, and Gilead reported its fourth-quarter sales up 8% to USD 384 million on breast cancer demand. With two TROP2 drugs and a first-line label, Douglas Insights models TROP2 patient-years rising from about 18,600 in 2025 to 57,900 in 2035. Still small. The class stays below 11% of targeted therapy for breast cancer value in 2035.

## What if adjuvant ribociclib and first-line Enhertu move faster than the 2035 forecast assumes?

Faster adjuvant and first-line uptake lifts targeted therapy for breast cancer to USD 69.16 billion in 2035, against USD 54.46 billion in our base case and USD 40.51 billion in a slower case. Volume, not price, drives most of that USD 28.65 billion spread.

The faster case assumes Enhertu with pertuzumab, approved on 15 December 2025 after DESTINY-Breast09 showed 40.7 months of median progression-free survival ([FDA](https://www.fda.gov/drugs/drug-approvals-and-databases/fda-approves-fam-trastuzumab-deruxtecan-nxki-pertuzumab-unresectable-or-metastatic-her2-positive)), becomes the first-line standard in most HER2-positive patients by 2029, and that adjuvant CDK4/6 use reaches half of eligible node-positive women. The slower case assumes more first-line trial misses like ASCENT-07, faster generic erosion and tighter reimbursement in Europe.

One extra point of patient-year growth lifts the 2035 targeted therapy for breast cancer total by USD 5.29 billion. Outside forecasts for breast cancer drugs span 4.37% to 8.5% a year; our 4.94% sits toward the lower end because it nets out the palbociclib and trastuzumab patent losses that broad therapeutics forecasts ignore.

## How do FDA approvals and Medicare negotiation rules shape breast cancer targeted drug launches?

Three FDA breast cancer approvals within nine months and one Medicare negotiated price set the rules for targeted therapy for breast cancer to 2027. Approvals widen the eligible pool, while the Medicare Drug Price Negotiation Program cuts the Ibrance price by 50%.

The FDA approved Enhertu with pertuzumab for first-line HER2-positive disease on 15 December 2025 ([FDA](https://www.fda.gov/drugs/drug-approvals-and-databases/fda-approves-fam-trastuzumab-deruxtecan-nxki-pertuzumab-unresectable-or-metastatic-her2-positive)) and Datroway for first-line triple-negative disease on 22 May 2026 ([FDA](https://www.fda.gov/drugs/resources-information-approved-drugs/fda-approves-datopotamab-deruxtecan-dlnk-unresectable-or-metastatic-triple-negative-breast-cancer)). CMS estimates the negotiated prices would have saved about USD 12 billion, or 44%, had they applied in 2024. Douglas Insights counts 8 growing lines and 3 shrinking ones in the register. The 3 decliners, Ibrance, Perjeta and Herceptin, are the oldest targeted breast cancer brands and together held USD 4.12 billion plus CHF 4.00 billion in sales. The 7 growing US dollar lines add up to USD 17.83 billion.

The finding is a hand-over, not a slowdown. Newer formats such as subcutaneous Phesgo, adjuvant Kisqali and the deruxtecan conjugates are replacing older brands inside the same patient groups, which is why the register shows double-digit growth next to falling sales. Enhertu and Trodelvy totals include some non-breast use.

Targeted therapy for breast cancer includes five drug classes worth USD 33.62 billion in 2025, and excludes endocrine therapy, chemotherapy and immune checkpoint inhibitors. Tamoxifen, aromatase inhibitors, fulvestrant and oral SERDs such as imlunestrant sit outside, even when combined with a targeted drug.

The study covers three disease subtypes: HR-positive HER2-negative, HER2-positive and triple-negative. It follows three lines of therapy, early breast cancer, first-line metastatic and later-line metastatic, and three routes: oral, intravenous and subcutaneous.
