### AbbVie and Eli Lilly

AbbVie discloses its U.S. book in full. Its [full-year 2025 release of 4 February 2026](https://www.sec.gov/Archives/edgar/data/1551152/000155115226000004/abbv-20251231xexhibit991.htm) shows U.S. net revenues of USD 46.60 billion, up 8.3%, with Skyrizi at USD 15.20 billion and Rinvoq at USD 5.94 billion in the United States, against USD 3.06 billion for Humira, whose U.S. sales are now eroded by biosimilars. Eli Lilly grew worldwide revenue 45% to USD 65.18 billion in 2025, with Mounjaro at USD 22.97 billion and Zepbound at USD 13.54 billion, and its [fourth-quarter release](https://www.sec.gov/Archives/edgar/data/59478/000005947826000008/q425lillysalesandearningsp.htm) put U.S. revenue at USD 12.9 billion in that quarter alone. Douglas Insights estimates Lilly's 2025 U.S. medicine sales at USD 43.4 billion.

### Johnson & Johnson and Merck

Johnson & Johnson's [2025 Form 10-K](https://www.sec.gov/Archives/edgar/data/200406/000020040626000016/jnj-20251228.htm) says Darzalex and Darzalex Faspro made about 15.0% of company revenue and Stelara about 6.5%, and warns that biosimilar launches will keep cutting Stelara sales. Merck sold USD 65.01 billion worldwide in 2025, USD 58.14 billion of it pharmaceuticals, and Keytruda reached USD 31.68 billion, up 7%, per its [3 February 2026 results](https://www.merck.com/wp-content/uploads/sites/124/2026/02/4Q25-Merck-Earnings-Announcement.pdf). Douglas Insights places Merck's U.S. pharmaceutical drug sales at about USD 29.7 billion.

## Which U.S. pharmaceutical drug segment leads: biologics, branded small molecules, generics, biosimilars or OTC?

Innovative biologics and peptides lead the U.S. pharmaceutical drug market with 40.6% of 2025 value, worth USD 289.23 billion, while biosimilars grow fastest at 13.62% a year. Generics fill 73.2% of standard units but earn only 10.8% of value.

| Segment | Share 2025 | Value 2025 | Growth 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| Innovative biologics and peptides | 40.6% | USD 289.23 billion | 6.84% | USD 560.52 billion |
| Branded innovative small molecules | 39.4% | USD 280.69 billion | 3.21% | USD 384.98 billion |
| Generics | 10.8% | USD 76.94 billion | 1.74% | USD 91.43 billion |
| Over-the-counter (OTC) medicines | 6.3% | USD 44.88 billion | 2.93% | USD 59.91 billion |
| Biosimilars | 2.9% | USD 20.66 billion | 13.62% | USD 74.07 billion |

Innovative biologics and peptides earn USD 289.23 billion because antibodies such as Keytruda, Skyrizi and Darzalex and incretin peptides such as Mounjaro and Ozempic carry the highest prices per dose in the system. Branded innovative small molecules hold USD 280.69 billion; oral oncology drugs, anticoagulants such as Eliquis and Xarelto and SGLT2 tablets keep that share high, but Medicare negotiation and patent expiries slow the segment to 3.21% a year.

Generics are worth USD 76.94 billion from 196.5 billion units at about USD 0.39 each, which is why their value share sits far below their unit share. OTC medicines add USD 44.88 billion, mostly analgesics, cold remedies and antacids sold through drugstores and mass retailers. Biosimilars start at USD 20.66 billion and reach USD 74.07 billion by 2035, as copies of Humira and Stelara take share and more antibodies lose exclusivity; our [Therapeutic Monoclonal Antibodies Market](https://www.douglasinsights.com/therapeutic-monoclonal-antibodies-market/) report follows that class worldwide.

## Why is demand for U.S. pharmaceutical drugs growing 1.62% a year in units?

U.S. pharmaceutical drug volume grows 1.62% a year to 2035, from 268.4 billion to 315.2 billion standard units, and five forces explain all of it. An ageing Medicare population adds 0.58 points and incretin medicines for diabetes and obesity add 0.47 points.

### Ageing and chronic disease: 0.58 points

Ageing adds 0.58 points. Older patients take more daily medicines for hypertension, diabetes, atrial fibrillation and high cholesterol, and Medicare Part D enrolment rises every year as the baby-boom cohort turns 65. CMS counts 5.3 million Medicare enrollees who took the 15 second-cycle drugs in 2024, per its [2027 negotiation summary](https://www.cms.gov/files/document/infographic-negotiated-prices-ipay-2027.pdf), a single indicator of how concentrated chronic therapy has become.

### GLP-1 medicines for diabetes and obesity: 0.47 points

Glucagon-like peptide-1 (GLP-1) and dual incretin drugs add 0.47 points. CMS reports that demand for diabetes and weight-loss drugs kept rising in 2024 even as overall use slowed, in its [National Health Expenditure (NHE) highlights](https://www.cms.gov/files/document/highlights.pdf). Zepbound revenue rose 175% in 2025, and the Trump administration put Ozempic and Wegovy at USD 199 a month on TrumpRx.gov at its [5 February 2026 launch](https://www.govinfo.gov/content/pkg/DCPD-202600083/html/DCPD-202600083.htm), a price that pulls new self-pay patients into the unit count.

### New launches: 0.24 points

Launches add 0.24 points. The Food and Drug Administration (FDA) Center for Drug Evaluation and Research (CDER) approved 46 novel drugs in 2025, including the non-opioid pain medicine Journavx on 30 January 2025 and the cardiomyopathy drug Myqorzo on 19 December 2025, per its [novel approvals list](https://www.fda.gov/drugs/novel-drug-approvals-fda/novel-drug-approvals-2025).

### Part D cost cap and adherence: 0.21 points

The Inflation Reduction Act (IRA) redesign of Part D adds 0.21 points. Out-of-pocket spending for people with Medicare drug coverage is capped at USD 2,100 in 2026, per [CMS](https://www.cms.gov/files/document/infographic-negotiated-prices-ipay-2027.pdf), so high-cost patients stop rationing fills late in the year.

### OTC switches and self-care: 0.12 points

Self-care adds the final 0.12 points, as prescription-to-OTC switches and pharmacy-led treatment widen the OTC basket. The five contributions, 0.58 + 0.47 + 0.24 + 0.21 + 0.12, sum to the 1.62% volume leg. Price adds 3.48% a year, mostly mix: Douglas Insights calculates that the shift towards biologics adds 5.90 points to the average unit price, while loss of exclusivity removes 1.09, Medicare negotiation 0.71 and most-favoured-nation (MFN) pricing deals 0.62.

## What holds back U.S. pharmaceutical drug volume: utilization controls, opioids and supply?

Three restraints remove 0.43 points from U.S. pharmaceutical drug volume growth, which would otherwise run at 2.05% a year. Payer utilization management takes 0.17 points, falling opioid prescribing 0.15 points and supply disruption 0.11 points.

Utilization management restrains new brands. Pharmacy benefit managers (PBMs) and health plans use prior authorization, step therapy and exclusion lists, especially for costly obesity drugs, and CMS shows retail prescription drug spending growth slowing from 10.8% in 2023 to 7.9% in 2024, with price growth of only 1.4%, in its [NHE highlights](https://www.cms.gov/files/document/highlights.pdf). Douglas Insights holds that brake at 0.17 points a year.

Opioid prescribing keeps falling under federal and state limits, removing 0.15 points; each lost tablet is a low-value unit, so the value effect is small. Supply disruption removes 0.11 points: the April 2026 [Section 232 proclamation](https://www.whitehouse.gov/presidential-actions/2026/04/adjusting-imports-of-pharmaceuticals-and-pharmaceutical-ingredients-into-the-united-states/) records that about 53% of patented medicines distributed in the United States are made abroad and only 15% of patented active ingredients by volume are made at home, a dependence that sterile generic shortages have already exposed.

## Where in the U.S. do pharmaceutical drug sales grow fastest, by Census region?

The South is the largest U.S. pharmaceutical drug region at USD 277.12 billion, 38.9% of 2025 value, and also the fastest at 5.58% a year. The West is the wildcard, adding USD 112.96 billion by 2035 as specialty drug use climbs.

| Census region | Value 2025 | Share 2025 | CAGR 2026-2035 | Value 2035 |
| --- | --- | --- | --- | --- |
| South | USD 277.12 billion | 38.9% | 5.58% | USD 476.97 billion |
| West | USD 166.70 billion | 23.4% | 5.31% | USD 279.66 billion |
| Midwest | USD 146.75 billion | 20.6% | 4.62% | USD 230.54 billion |
| Northeast | USD 121.82 billion | 17.1% | 4.58% | USD 190.64 billion |

The South reaches USD 476.97 billion by 2035. Texas, Florida, Georgia and the Carolinas add residents and retirees fastest, and the region carries high rates of diabetes and obesity treatment, the categories CMS singles out for rising demand. Douglas Insights estimates the South fills 104.4 billion standard units in 2025.

The West grows 5.31% a year to USD 279.66 billion. California's large Medicaid programme and Arizona's and Nevada's ageing populations lift specialty volumes, and the region hosts many of the biotech companies whose launches enter the market first.

The Midwest, at USD 146.75 billion, grows 4.62% a year to USD 230.54 billion, held back by slower population growth. The Northeast is the most mature at USD 121.82 billion, rising 4.58% a year to USD 190.64 billion, because spending per person is already the highest and academic medical centres in New York, Massachusetts and Pennsylvania prescribe new oncology drugs early. In the data table the row labelled Global holds the U.S. total, USD 712.4 billion in 2025; the world view sits in the [Pharmaceutical Drug Market](https://www.douglasinsights.com/pharmaceutical-drug-market/) study, and the fastest-growing region abroad is covered in our [Asia Pacific Pharmaceutical Drug Market](https://www.douglasinsights.com/asia-pacific-pharmaceutical-drug-market/) report.

## How much does a unit of U.S. pharmaceutical drug cost, from a generic tablet to a biologic dose?

A U.S. pharmaceutical drug unit averages USD 2.654 at manufacturer net prices in 2025, inside a band from about USD 0.02 for an old generic tablet to several thousand dollars for a cancer antibody infusion. Patent status and payer leverage set the price.

| Segment | Units 2025 | Average per unit | Basis |
| --- | --- | --- | --- |
| Generics | 196.5 billion | USD 0.392 | Douglas Insights estimate |
| OTC medicines | 58.6 billion | USD 0.766 | Douglas Insights estimate |
| Branded innovative small molecules | 12.4 billion | USD 22.64 | Douglas Insights estimate |
| Biosimilars | 0.19 billion | USD 108.74 | Douglas Insights estimate |
| Innovative biologics and peptides | 0.71 billion | USD 407.37 | Douglas Insights estimate |

Federal action now resets brand prices directly. The 15 second-cycle Medicare prices, effective in 2027, cut the 30-day list price by 38% for Austedo up to 85% for Janumet, and would have saved USD 12 billion had they applied in 2024, per [CMS](https://www.cms.gov/files/document/infographic-negotiated-prices-ipay-2027.pdf). At the TrumpRx launch, insulin was quoted at USD 25 against USD 200 and a common inhaler at USD 51 against USD 458, per the [presidential transcript](https://www.govinfo.gov/content/pkg/DCPD-202600083/html/DCPD-202600083.htm). Douglas Insights still projects the average unit price to rise 3.48% a year, to USD 3.737 by 2035, because every unit that moves from a USD 0.39 generic to a USD 407 biologic lifts the mix.

## Which therapy areas and channels carry U.S. pharmaceutical drug sales?

Oncology carries 21.4% of 2025 U.S. pharmaceutical drug value, about USD 152.45 billion, and cardiovascular and metabolic medicines 18.6%, about USD 132.51 billion. Retail pharmacy sells 41.3% of value, hospitals and clinics 36.0%.

| Therapy area | Share of 2025 value | Value 2025 |
| --- | --- | --- |
| Oncology | 21.4% | USD 152.45 billion |
| Cardiovascular and metabolic, including obesity | 18.6% | USD 132.51 billion |
| Immunology | 13.2% | USD 94.04 billion |
| Central nervous system | 9.8% | USD 69.82 billion |
| Anti-infectives | 5.1% | USD 36.33 billion |
| Respiratory | 4.9% | USD 34.91 billion |

The six areas hold 73.0% of value; haematology, ophthalmology, rare disease and dermatology make up the rest. Immunology is where AbbVie's Skyrizi and Rinvoq, at USD 21.14 billion of combined U.S. sales, compete with Stelara biosimilars.

By channel, retail pharmacy holds 41.3%, or USD 294.22 billion, specialty and mail-order pharmacy 22.7%, or USD 161.71 billion, and hospital and clinic-administered drugs 36.0%, or USD 256.46 billion, on Douglas Insights estimates. Physician-administered infusions such as Keytruda and Darzalex are bought by hospitals and clinics and billed under medical benefits rather than pharmacy benefits.

## Which rules set U.S. pharmaceutical drug prices: Medicare negotiation, MFN deals and Section 232 tariffs?

Three federal tracks now set U.S. pharmaceutical drug prices: Medicare negotiation for 10 drugs since 1 January 2026, MFN deals signed by 16 of the 17 largest drugmakers, and a 100% Section 232 tariff on patented imports. Fifteen more negotiated prices follow in 2027.

Medicare negotiation under the IRA would have cut net spending on the first 10 drugs by about USD 6 billion, or 22%, in 2023, and saves enrollees an estimated USD 1.5 billion out of pocket once prices apply in 2026, per the [CMS fact sheet](https://cms.gov/files/document/fact-sheet-negotiated-prices-initial-price-applicability-year-2026.pdf). Its list includes Eliquis at 56% off, Jardiance and Stelara at 66% and Entresto at 53%. The [second cycle](https://www.cms.gov/files/document/infographic-negotiated-prices-ipay-2027.pdf) adds Ozempic, Rybelsus and Wegovy at 71%, Trelegy Ellipta at 73% and Xtandi at 48%, with USD 685 million of enrollee savings in 2027.

The Department of Health and Human Services (HHS) runs MFN pricing through company agreements and TrumpRx.gov, which opened on 5 February 2026 with more than 40 medicines from AstraZeneca, Eli Lilly, EMD Serono, Novo Nordisk and Pfizer, per the [launch transcript](https://www.govinfo.gov/content/pkg/DCPD-202600083/html/DCPD-202600083.htm).

The [2 April 2026 proclamation](https://www.whitehouse.gov/presidential-actions/2026/04/adjusting-imports-of-pharmaceuticals-and-pharmaceutical-ingredients-into-the-united-states/) sets a 100% duty on patented medicines and their ingredients, 20% for companies with approved onshoring plans and 0% for those that also hold MFN agreements until 20 January 2029, with 15% for the European Union (EU), Japan, South Korea and Switzerland and 10% for the United Kingdom. It applies from 31 July 2026 for 17 listed companies and from 29 September 2026 for all others; generics and biosimilars are exempt. The FDA approves every product through new drug applications (NDAs), biologics licence applications (BLAs) and abbreviated applications for generics.

## What if Medicare negotiation widens or tariffs bite: where does the U.S. pharmaceutical drug forecast land in 2035?

The U.S. pharmaceutical drug market ends 2035 between USD 913.2 billion and USD 1.46 trillion across our three cases, around a base of USD 1.18 trillion. Federal price levers and incretin uptake set most of the spread.

| Case | Volume leg | Price leg | 2035 value |
| --- | --- | --- | --- |
| Slower | 0.90% | 1.60% | USD 913.2 billion |
| Base case | 1.62% | 3.48% | USD 1.18 trillion |
| Faster | 2.30% | 5.00% | USD 1.46 trillion |

In the slower case, Medicare negotiation reaches 20 drugs a year, MFN reference prices spread to commercial plans and payers keep obesity drugs behind prior authorization. In the faster case, obesity coverage widens, the 46 novel approvals of 2025 repeat each year and tariffs mainly move plants rather than prices. One extra point of yearly volume growth adds USD 121.2 billion to the 2035 value; one point less removes USD 110.9 billion.

Our 5.16% sits below the 7.9% growth CMS recorded for 2024 retail drug spending, because we value at manufacturer net prices, include slower-growing generics and OTC medicines, and let mix, not list-price increases, carry the price leg.

## Douglas Exclusive: the U.S. Federal Drug Price Lever Ledger

The U.S. Federal Drug Price Lever Ledger is a Douglas Insights model built from 9 sourced inputs: two CMS negotiation documents, the CMS NHE release, one White House proclamation, one presidential transcript, the FDA approvals list and three company filings. It dates each federal price lever and estimates its value effect.

| Date | Lever | Effect on U.S. value |
| --- | --- | --- |
| 2024 | Retail drug prices rise only 1.4% | Price growth slows |
| 30 January 2025 | FDA approves Journavx, one of 46 novel drugs that year | Up, launches |
| November 2025 | CMS publishes 15 second-cycle prices | Down from 2027, USD 12 billion |
| 1 January 2026 | First 10 negotiated prices apply | Down, USD 6 billion |
| 1 January 2026 | Part D out-of-pocket cap at USD 2,100 | Up in volume |
| 5 February 2026 | TrumpRx.gov opens with 40-plus drugs | Down per unit, up in self-pay volume |
| 2 April 2026 | Section 232 proclamation signed | Cost risk for importers |
| 31 July 2026 | Tariff applies to 17 listed companies | Neutral for MFN signers |
| 29 September 2026 | Tariff applies to all other importers | Up in cost, down in margin |

The finding, a Douglas Insights estimate: the two negotiation cycles alone remove about USD 18 billion a year on CMS's own base years, 2.53% of our 2025 U.S. pharmaceutical drug value, while mix adds 5.90 points of price growth every year. Federal levers bend the curve but do not reverse it; the larger risk to brand revenue is MFN pricing spreading from Medicaid and cash sales into commercial contracts.

## How does the methodology turn 268.4 billion units into USD 712.4 billion of U.S. pharmaceutical drugs?

Our methodology multiplies 268.4 billion standard units of 2025 U.S. pharmaceutical drug consumption by an average USD 2.654 per unit to reach USD 712.4 billion. Units come from four Census region builds; price comes from five segment bands weighted by units.

The model covers 4 Census regions spanning 50 states and the District of Columbia and uses 74 data points, 22 of them sourced and 52 modelled. Arithmetic: 268.4 billion x USD 2.6542 = USD 712.4 billion; 712.4 x (1.0162 x 1.0348)^10 = 1,177.8, or USD 1.18 trillion. Units reach 315.2 billion in 2035.

Two cross-checks agree. First, CMS put 2024 retail prescription drug spending at USD 467.0 billion, up 7.9%, per its [NHE highlights](https://www.cms.gov/files/document/highlights.pdf); carried forward one year that is about USD 503.9 billion at retail prices, and our USD 411.1 billion of prescription sales through retail, specialty and mail-order pharmacy at manufacturer prices equals 81.6% of it, the gap being pharmacy and wholesaler margin. Second, AbbVie's disclosed USD 46.60 billion of U.S. net revenue equals 6.54% of our total, in line with the share the leading U.S. seller would be expected to hold in a market where no company reaches 10%.

## Which medicines count in the U.S. pharmaceutical drug market, and what is excluded?

The U.S. pharmaceutical drug market counts finished prescription and OTC medicines consumed in the 50 states and the District of Columbia, valued at manufacturer net selling prices after rebates and statutory discounts. Vaccines, bulk active ingredients, dietary supplements, devices and pharmacy margins stay out.

Imported finished medicines count where they are consumed; exports stay out. Contract manufacturing revenue sits in our [Biologics Outsourcing Market](https://www.douglasinsights.com/biologics-outsourcing-market/) study. The by-product split covers innovative biologics and peptides, branded innovative small molecules, generics, biosimilars and OTC medicines; the by-therapy split covers oncology, cardiovascular and metabolic, immunology, central nervous system, anti-infectives and respiratory medicines; and the channel split covers retail pharmacy, specialty and mail-order pharmacy, and hospital and clinic-administered drugs.

## Market by segment
