bioMérieux’s BIOFIRE respiratory panel sales fell 14% in the first nine months of 2026 while its non-respiratory panels grew 12%, its business review of 3 November 2026 shows, a split that sums up where polymerase chain reaction (PCR) testing is heading. The PCR and real-time PCR molecular diagnostics market covers clinical PCR and quantitative real-time PCR (qPCR) instruments, reagents, assay kits, software and service used to diagnose infections, cancers and inherited conditions and to screen donated blood, sold to hospital and reference laboratories, blood banks and point-of-care sites. Douglas Insights sizes it at USD 14.6 billion in 2025, about 1.62 billion tests at an average USD 9.00 each, and at USD 25.2 billion in 2035, a 5.63% compound annual growth rate (CAGR). The study sits in our in vitro diagnostics coverage, beside the Digital PCR Market, Magnetic Bead-based Nucleic Acid Extraction Market and Respiratory Disease Genetic Testing Market reports; every figure follows the Douglas Insights research methodology.

Which PCR diagnostics product earns the most: reagents and assay kits, instruments or software and service?

Reagents and assay kits earn the most, 71.0% of 2025 PCR molecular diagnostics revenue or about USD 10.4 billion, Douglas Insights estimates. Software and service grow fastest, 7.0% a year to USD 3.45 billion by 2035.

Product Share 2025 Value 2025 CAGR 2026-2035 Value 2035
Reagents and assay kits 71.0% USD 10.4 billion 5.8% USD 18.2 billion
Instruments 17.0% USD 2.48 billion 4.2% USD 3.75 billion
Software and service 12.0% USD 1.75 billion 7.0% USD 3.45 billion

Reagents lead because PCR is a razor-and-blade business: a laboratory buys or leases an analyser once and then buys a sealed cartridge, master mix or assay kit for every test it runs. Each of the 1.62 billion tests in our 2025 base consumes reagent, so reagents grow with volume, 5.8% a year to USD 18.2 billion, a Douglas Insights estimate. QIAGEN’s QIAstat-Dx syndromic cartridges sold USD 35 million in the third quarter of 2026, up 11% at constant exchange rates, its results of 4 November 2026 show.

Instruments, USD 2.48 billion, grow 4.2% a year to USD 3.75 billion because many analysers are placed under reagent rental deals rather than sold. bioMérieux added a net 1,800 non-respiratory BIOFIRE systems to its installed base in 2025, after 1,350 in 2024, its results of 27 February 2026 show. Software and service, USD 1.75 billion, grow 7.0% on middleware, connectivity and service contracts. Product values for 2035 sum to USD 25.4 billion, 0.64% above the headline.

Which tests use PCR most: respiratory infections, other infections, oncology, blood screening or genetic testing?

Non-respiratory infectious disease takes the largest share, 30.0% of 2025 PCR diagnostics revenue or about USD 4.38 billion, Douglas Insights estimates. Oncology grows fastest, 8.6% a year to USD 5.33 billion by 2035.

Application Share 2025 Value 2025 CAGR 2026-2035 Value 2035
Infectious disease, non-respiratory 30.0% USD 4.38 billion 7.4% USD 8.94 billion
Infectious disease, respiratory 28.0% USD 4.09 billion 1.5% USD 4.74 billion
Oncology 16.0% USD 2.34 billion 8.6% USD 5.33 billion
Genetic and other testing 15.0% USD 2.19 billion 6.2% USD 4.00 billion
Blood screening and transplant 11.0% USD 1.61 billion 3.4% USD 2.24 billion

Non-respiratory infection testing leads because sexually transmitted infections, hepatitis, HIV viral load, human papillomavirus (HPV), tuberculosis (TB), gastrointestinal, bloodstream and meningitis panels all run on PCR. bioMérieux’s BIOFIRE non-respiratory panels reached EUR 598 million in 2025, up 10% organically, its 27 February 2026 results show. Douglas Insights expects the segment to grow 7.4% a year to USD 8.94 billion.

Respiratory testing, USD 4.09 billion, grows only 1.5% a year because each winter season now sets demand, not a pandemic. Oncology, USD 2.34 billion, grows 8.6% on mutation, fusion and minimal residual disease tests. Genetic and other testing, USD 2.19 billion, grows 6.2% on pharmacogenomic and prenatal tests, and blood screening and transplant, USD 1.61 billion, grow 3.4% as donor volumes stay flat. Application values for 2035 sum to USD 25.3 billion, 0.04% above the headline.

What drives PCR diagnostics demand: syndromic panels, point-of-care PCR, oncology or TB testing?

PCR molecular diagnostics revenue rises 5.63% a year in our base case: test volume grows 5.0% a year, from about 1.62 billion to about 2.64 billion tests, and the average price rises 0.6%. Four drivers add 5.90 points; restraints remove 0.90.

Syndromic and non-respiratory panels: 1.80 points

Multiplex panels are the largest driver, worth 1.80 points. bioMérieux’s non-respiratory BIOFIRE panels grew 12% in the first nine months of 2026 and its group sales rose 8% organically to EUR 1.04 billion in the third quarter, its 3 November 2026 review shows. Cepheid won US Food and Drug Administration (FDA) clearance on 20 January 2026 for its Xpert GI Panel, which detects 11 gastrointestinal pathogens in about 74 minutes, its release states. Douglas Insights estimates one panel replaces three to eight single tests and earns USD 120 to USD 250.

Point-of-care and near-patient PCR: 1.50 points

Near-patient testing adds 1.50 points. Roche won FDA 510(k) clearance with a Clinical Laboratory Improvement Amendments (CLIA) waiver, plus CE marking under the In Vitro Diagnostic Regulation (IVDR), for a cobas liat PCR test for whooping cough on 2 December 2025, its release states, so clinics without a laboratory can run it in about 20 minutes or less. QIAGEN’s QIAstat-Dx grew 11% in the third quarter of 2026. Douglas Insights estimates clinics, urgent care and emergency rooms ran about 9% of PCR tests in 2025 and will run about 15% by 2035.

Oncology and precision medicine: 1.40 points

Cancer testing adds 1.40 points. Abbott completed its purchase of Exact Sciences on 23 March 2026 and its Cancer Diagnostics sales reached USD 968 million in the third quarter, up 13.8% on a comparable basis, its results of 21 October 2026 show. Bio-Rad cited strong demand for Droplet Digital PCR instruments, used in oncology monitoring, in its results of 4 November 2026. Douglas Insights estimates PCR-based tumour mutation and monitoring tests grow about 9% a year in volume.

TB and global health programmes: 1.20 points

The last 1.20 points come from TB and other public health testing. Coverage of rapid molecular testing for TB diagnosis rose from 48% in 2023 to 54% in 2024, while 8.3 million people were newly diagnosed out of an estimated 10.7 million who fell ill, the World Health Organization (WHO) said on 12 November 2025. Each further 10 points of coverage adds about 0.8 million TB PCR tests a year, a Douglas Insights estimate. Summed, 1.80 + 1.50 + 1.40 + 1.20 = 5.90 points.

What holds PCR diagnostics growth back: post-pandemic respiratory decline, Medicare fee cuts or price tenders?

Three restraints remove 0.90 points a year from PCR test volume growth: shrinking respiratory testing takes 0.50 points, US laboratory fee cuts 0.25 points and price-led tenders in China and other public systems 0.15 points.

Respiratory decline is the heaviest drag. Danaher’s respiratory testing sales were about USD 350 million in the quarter to 25 September 2026 and it expects about USD 1.50 billion for the full year, its results of 21 October 2026 show, and its diagnostics core growth was only 0.5%, against 7.0% excluding respiratory. bioMérieux’s respiratory panels fell 14% in the first nine months of 2026, the 3 November 2026 review shows.

Fee cuts take 0.25 points. The Centers for Medicare and Medicaid Services (CMS) published preliminary 2027 Clinical Laboratory Fee Schedule rates on 21 September 2026 in which 1,171 of 1,528 codes with private payer data would fall, by about 16% on average, its fact sheet shows, though cuts are capped at 15% a year from 2027. Tenders take the final 0.15 points: volume-based procurement in China and pooled purchasing in Africa push assay prices down 20% to 50%, a Douglas Insights estimate.

Which region buys the most PCR diagnostics, and where does demand grow fastest?

North America buys the most PCR molecular diagnostics, USD 5.99 billion or 41.0% of 2025 revenue, on high test prices and broad syndromic panel use. Asia Pacific grows fastest, 8.0% a year to USD 7.25 billion by 2035.

Region Value 2025 Share 2025 Value 2026 CAGR 2026-2035 Value 2035
North America USD 5.99 billion 41.0% USD 6.27 billion 4.8% USD 9.57 billion
Europe USD 3.94 billion 27.0% USD 4.12 billion 4.5% USD 6.12 billion
Asia Pacific USD 3.36 billion 23.0% USD 3.63 billion 8.0% USD 7.25 billion
Latin America USD 730 million 5.0% USD 774 million 6.0% USD 1.31 billion
Middle East and Africa USD 584 million 4.0% USD 623 million 6.6% USD 1.11 billion

North America leads because a US syndromic panel can be reimbursed at several hundred dollars and hospitals run panels for respiratory, gastrointestinal, bloodstream and meningitis infections. Hologic’s molecular diagnostics revenue was USD 329.2 million in the quarter to 27 December 2025, and USD 301.2 million excluding COVID-19, up 1.2%, its results of 29 January 2026 show. Regional revenue grows 4.8% a year to USD 9.57 billion, a Douglas Insights estimate, held back by fee pressure.

Europe, USD 3.94 billion, grows 4.5% as IVDR certification costs slow new assay launches. Asia Pacific, USD 3.36 billion, grows 8.0% on Chinese, Indian and Southeast Asian hospital laboratories and TB programmes; Douglas Insights estimates the region runs about 30% of the world’s TB PCR tests. Latin America, USD 730 million, grows 6.0%, and the Middle East and Africa, USD 584 million, grow 6.6%. Regional values sum to USD 15.4 billion in 2026 and USD 25.4 billion in 2035, 0.41% above the headline.

Which companies lead PCR molecular diagnostics?

Roche is the largest PCR diagnostics supplier at about 16% of 2025 revenue, Douglas Insights estimates; Roche, Danaher’s Cepheid, bioMérieux, Hologic and Abbott hold about 58% together, with QIAGEN and Bio-Rad next.

Company Latest reported figure Period and source date
Roche Molecular Lab sales CHF 1.84 billion, up 3% at CER Nine months 2026; 22 October 2026
Danaher (Cepheid) Respiratory sales about USD 350 million Quarter to September 2026; 21 October 2026
bioMérieux Sales EUR 1.04 billion, up 8% organic Quarter to September 2026; 3 November 2026

Abbott Rapid and Molecular Diagnostics USD 899 million Quarter to September 2026; 21 October 2026
QIAGEN Net sales USD 548 million, up 3% Quarter to September 2026; 4 November 2026
Bio-Rad Clinical Diagnostics USD 401.8 million, up 2.7% Quarter to September 2026; 4 November 2026

Roche leads through its cobas analysers, with 2025 Molecular Lab sales of CHF 2.53 billion, up 4% at constant exchange rates, its results of 29 January 2026 show, and nine-month 2026 Molecular Lab sales of CHF 1.84 billion, up 3%, its 22 October 2026 release states; it also launched two real-time PCR tests for a US Cyclospora outbreak. Danaher’s Cepheid leads near-patient PCR with the GeneXpert system and the largest respiratory franchise.

Ownership is shifting fast. Blackstone and TPG completed their purchase of Hologic, valued at up to USD 18.3 billion, on 7 April 2026, the closing release states, and EQT Private Equity announced a EUR 46.50 a share cash offer for QIAGEN on 21 October 2026, with closing expected in mid-2027, QIAGEN’s 4 November 2026 report shows. QIAGEN suspended its 2026 outlook while its PCR and nucleic acid amplification sales fell 5% to USD 71 million.

Abbott now reports rapid, molecular and point-of-care tests together, USD 899 million in the third quarter of 2026, up 0.9% on a comparable basis, its 21 October 2026 results show. Bio-Rad bought digital PCR developer Stilla Technologies and launched its QX Continuum and QX700 instruments on 7 July 2025, its release states. Douglas Insights estimates Chinese makers and smaller assay firms share the remaining 30% or so of revenue.

How much does a PCR test cost, from a blood screening pool to a syndromic panel?

A PCR diagnostic test earns its maker an average USD 9.00 in 2025, Douglas Insights estimates, from under USD 2 for a pooled blood screening or tendered TB test to USD 120 to USD 250 for a syndromic panel cartridge.

The average hides a steep price ladder. Douglas Insights puts blood donor screening at USD 1 to USD 4 per donation, tendered TB and HIV viral load tests in low-income countries at USD 4 to USD 15, routine laboratory single-target tests in Europe and the US at USD 8 to USD 30, oncology mutation tests at USD 40 to USD 150 and syndromic panels at USD 120 to USD 250. About 55% of tests sell for under USD 5 but earn only about 15% of revenue, a Douglas Insights estimate.

The average price rises 0.6% a year in the base case, to about USD 9.55 in 2035, because the mix shifts to panels and oncology even as single-target prices fall. Laboratory payment drives what makers can charge. Section 6226 of the Consolidated Appropriations Act, 2026, signed on 3 February 2026, blocked any Medicare fee cut in 2026 and limits cuts to 15% a year from 2027 to 2029, the CMS reporting page states.

Douglas Insights treats the 2027 fee reset as taking about 0.3 points off the US price leg for three years, partly offset by panel and oncology launches that carry new, higher codes. Reagent rental deals, in which a laboratory pays only per test, keep about 60% of instrument value inside reagent prices, a Douglas Insights estimate.

What if respiratory testing falls further or panels spread faster: how large is the PCR diagnostics market in 2035?

The PCR molecular diagnostics market ends 2035 between USD 21.2 billion and USD 30.0 billion across our three cases, around a base of USD 25.2 billion. Respiratory demand and panel uptake set most of the spread.

Case Volume leg Price leg Revenue rate 2035 value
Slower 3.80% 0.00% 3.80% USD 21.2 billion
Base case 5.00% 0.60% 5.63% USD 25.2 billion
Faster 6.20% 1.20% 7.47% USD 30.0 billion

In the slower case respiratory testing keeps falling, US fees drop the full 15% each year from 2027 and donor funding for TB programmes shrinks, taking 1.2 points off volume and 0.6 points off price. In the faster case panels move into most US and European emergency rooms, a new respiratory threat lifts winter testing and oncology monitoring spreads, adding 1.2 points of volume and 0.6 points of price. One extra point of yearly revenue growth adds about USD 2.3 billion to 2035 revenue, a Douglas Insights estimate.

Live signals sit close to the base case. bioMérieux’s non-respiratory panels up 12%, Abbott’s cancer diagnostics up 13.8% and QIAstat-Dx up 11% point up; Danaher’s 0.5% diagnostics core growth and QIAGEN’s 5% fall in PCR sales point down. Our 5.63% rate weighs strong panel and oncology growth against a shrinking respiratory base.

Which rules govern PCR diagnostics, from the vacated FDA LDT rule to the EU IVDR?

Three rule sets govern PCR diagnostics: FDA clearance for kits sold in the United States, with laboratory-developed tests now outside FDA device rules again, the EU In Vitro Diagnostic Regulation (IVDR) and its transition deadlines, and Medicare payment rules under the Protecting Access to Medicare Act (PAMA).

The US test layer changed in 2025. The US District Court for the Eastern District of Texas vacated the FDA’s 2024 laboratory-developed test (LDT) rule on 31 March 2025, and the FDA removed the rule’s text in a Federal Register notice of 19 September 2025, the notice shows, so hospital and reference laboratories can keep running their own PCR assays without device premarket review. Douglas Insights estimates laboratory-developed PCR tests are about 18% of US PCR test volume.

The European layer is Regulation (EU) 2024/1860, which extended IVDR transition to 31 December 2027 for class D devices such as blood screening assays, 31 December 2028 for class C devices such as most infectious disease and cancer tests, and 31 December 2029 for class B devices, provided makers applied to a notified body by set dates, Ireland’s HPRA guidance confirms. The European Commission proposed simpler device rules on 16 December 2025, including a more flexible in-house exemption, a Morgan Lewis summary reports.

The payment layer is PAMA. Medicare fee cuts were blocked for 2026 and capped at 15% a year from 2027 to 2029, and the preliminary 2027 rates of 21 September 2026 would lower 1,171 codes, CMS states. Douglas Insights estimates IVDR certification adds EUR 0.3 million to EUR 1.5 million and 12 to 24 months to each new class C PCR assay launched in Europe.

Douglas Exclusive: the Syndromic Shift Index

The Syndromic Shift Index is a Douglas Insights model built from 12 inputs: 10 company and regulatory releases and 2 agency statistics, listed below. It tracks how much PCR revenue moves from single-target tests to multiplex panels and near-patient cartridges.

Date Input What it moves
7 July 2025 Bio-Rad: Stilla purchase and QX Continuum launch Digital PCR oncology
19 September 2025 FDA: LDT rule vacatur in the Federal Register US laboratory tests
12 November 2025 WHO: rapid TB testing coverage 54% in 2024 Global health volume
2 December 2025 Roche: CLIA-waived cobas liat pertussis test Point-of-care share
16 December 2025 European Commission: device rule simplification proposal EU launch cost
20 January 2026 Cepheid: Xpert GI Panel cleared Panel share
29 January 2026 Roche: 2025 Molecular Lab CHF 2.53 billion Laboratory base
27 February 2026 bioMérieux: non-respiratory panels EUR 598 million Panel share
7 April 2026 Hologic: Blackstone and TPG purchase completed Ownership
21 September 2026 CMS: preliminary 2027 fee schedule US price
21 October 2026 Danaher: respiratory sales about USD 350 million Respiratory base
3 November 2026 bioMérieux: respiratory panels down 14% Respiratory base

The finding, a Douglas Insights estimate: multiplex panels and near-patient cartridges earned about 31% of PCR diagnostics revenue in 2025 from only about 4% of tests, and rise to about 41% of revenue and about 7% of tests by 2035. Non-respiratory panels drive most of the gain, while respiratory panels hold a flat dollar base.

The index changes how to read the market. Because a panel earns 15 to 25 times a single test, the panel shift alone adds about 0.4 points to the yearly price leg, offsetting fee and tender cuts. Douglas Insights therefore weights installed panel systems about three times as heavily as total test counts when forecasting revenue.

How does the methodology turn 1.62 billion tests into USD 14.6 billion?

How this report is built

  • Every figure carries a confidence grade in the fact sheet above, and the working model ships with every licence.
  • Five regional models sum to the global figure, with country tables in the Excel model.
  • The next scheduled review of this study is May 2027.
  • Licence holders receive it as a maintained tab in the Excel model.

Our methodology multiplies about 1.62 billion PCR diagnostic tests run in 2025 by an average USD 9.00 a test, counting reagents, instruments and service, to reach USD 14.6 billion, then grows tests 5.0% and price 0.6% a year.

The model covers 3 product groups, 5 applications and 5 regions, using 58 data points, 23 sourced and 35 modelled. Tests come from disease incidence, testing rates and laboratory counts; prices come from company reports, fee schedules, tender awards and Douglas Insights price ladders. Arithmetic: 1.62 billion x USD 9.00 = USD 14.6 billion; USD 14.6 billion x (1.050 x 1.006)^10 = USD 25.2 billion. Tests reach about 2.64 billion in 2035.

Two cross-checks agree. First, Roche’s CHF 2.53 billion of 2025 Molecular Lab sales, about USD 2.9 billion before non-PCR products, is consistent with a leader near 16% of the market once sequencing and other methods are removed. Second, bioMérieux’s 2025 sales of EUR 4.07 billion, of which BIOFIRE non-respiratory panels were EUR 598 million, fit our panel revenue base. Currency conversions use Douglas Insights assumptions of USD 1.13 per euro and USD 1.15 per Swiss franc. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our editorial standards explain.

What counts in the PCR and real-time PCR molecular diagnostics study, and what is excluded?

The PCR and real-time PCR molecular diagnostics study counts clinical PCR, qPCR and digital PCR instruments, reagents, assay kits, software and service used for diagnosis and blood screening worldwide. Research-use qPCR, sequencing, isothermal tests and laboratory testing service fees are excluded.

Values are at the manufacturer’s first sale to a laboratory, hospital or distributor, so laboratory service charges and distributor margin are excluded. Reagents for laboratory-developed PCR tests are counted when they are sold for clinical use, and nucleic acid extraction kits sold with a PCR assay are counted; standalone extraction products are not, and appear in our Magnetic Bead-based Nucleic Acid Extraction Market report.

The study splits revenue by 3 product groups, 5 applications and 5 regions: North America, Europe, Asia Pacific, Latin America, and the Middle East and Africa. Research digital PCR appears in our Digital PCR Market report, research analysers in the Biotechnology Instruments Market report and inherited respiratory tests in the Respiratory Disease Genetic Testing Market report.
