MegaGen won US clearance for a zygoma implant system in 12 lengths from 30 to 57.5 mm on 18 June 2026, the FDA 510(k) record K251232 shows, two days after BioHorizons launched its Pro Zygoma implant for “severely resorbed maxillae”, its release of 16 June 2026 states. The zygomatic and pterygoid implants market covers the long implants anchored in the cheekbone or the pterygoid plate behind the upper jaw, the angled multi-unit abutments and prosthetic parts sold with them, and the surgical kits, drills and planning tools needed to place them; standard-length implants, bone grafts and dentists’ fees are excluded. Douglas Insights sizes it at USD 165.6 million in 2025, about 240,000 implants at an average USD 690 realised manufacturer price including components, and at USD 394.9 million in 2035, a 9.08% compound annual growth rate (CAGR). The study sits beside our Dental Implants Market, Dental Equipment Market and Zirconia Dental Indirect Restorations Material Market reports; every figure follows the Douglas Insights research methodology.

Which remote anchorage products earn the most: zygomatic fixtures, pterygoid fixtures or angled abutments?

Zygomatic implant fixtures earn the most, 52.0% of 2025 value or about USD 86.1 million, Douglas Insights estimates. Pterygoid fixtures take 21.0%, angled abutments 17.0% and surgical kits 10.0%.

Product type Share 2025 Value 2025 CAGR 2026-2035 Value 2035
Zygomatic implant fixtures 52.0% USD 86.1 million 9.6% USD 215.4 million
Pterygoid implant fixtures 21.0% USD 34.8 million 9.9% USD 89.4 million
Angled multi-unit abutments and prosthetic parts 17.0% USD 28.2 million 8.6% USD 64.2 million
Surgical kits, drills and guided planning 10.0% USD 16.6 million 7.0% USD 32.6 million

Zygomatic fixtures lead because each one is long, machined from titanium alloy and sold in many lengths, so its price sits several times above a standard implant. The Adin Touareg-OS zygomatic implant cleared on 23 March 2026 comes in eight lengths from 35.0 to 55.0 mm at a 4.2 mm diameter, and the S.I.N. Tapered Pro Conical Zygoma system cleared on 7 November 2025 comes in 13 lengths from 30 to 60 mm, the K252031 and K251129 510(k) records show. Zygomatic fixture revenue grows 9.6% a year to USD 215.4 million by 2035, a Douglas Insights estimate.

Pterygoid fixtures, USD 34.8 million, are shorter and cheaper, typically 15 to 25 mm, and often come from a maker’s standard implant line in longer sizes, so a pterygoid case costs the dentist about a third of a zygomatic one. They grow fastest, at 9.9%, because more general implant surgeons are trained to place them than to drill into the cheekbone. Angled multi-unit abutments, USD 28.2 million, grow 8.6%: every remote anchorage implant needs one, and the MegaGen and S.I.N. systems both list 45, 52 and 60 degree versions. Surgical kits, drills and planning tools, USD 16.6 million, grow 7.0% because one kit serves many cases. Product values for 2035 sum to USD 401.6 million, 1.68% above the headline.

Which patients and settings use zygomatic and pterygoid implants?

Patients with a severely resorbed upper jaw who want fixed teeth without bone grafting account for about 85% of 2025 implants, Douglas Insights estimates; cancer resection and trauma cases take about 15%, mostly zygomatic.

The atrophic maxilla group dominates because zygomatic and pterygoid anchorage avoids sinus lifts and block grafts that add months of healing. Most of these patients are older adults who have worn a full upper denture for years and lost bone under it. Within that group, a typical “quad zygoma” case uses four zygomatic fixtures, while a hybrid case uses two zygomatic fixtures with two to four standard anterior implants; pterygoid fixtures are added at the back of the arch to avoid cantilevers. Douglas Insights estimates an average of 2.3 remote anchorage implants per treated arch.

Oncology and trauma cases are fewer but use the most expensive parts. Southern Implants sells a dedicated oncology zygomatic line for patients after maxillectomy, and hospitals buy these through head and neck surgery budgets rather than private dental practices. Specialist oral and maxillofacial surgeons and full-arch clinics place about 70% of remote anchorage implants, Douglas Insights estimates, and hospitals about 15%; the remaining 15% is placed by general dentists with advanced training, mostly pterygoid cases.

What drives demand for zygomatic and pterygoid implants: tooth loss, full-arch clinics or new products?

Remote anchorage implants placed rise 8.00% a year in our base case, from about 240,000 in 2025 to 518,000 in 2035. Four drivers add 9.60 gross points; restraints remove 1.60.

Edentulous older adults: 3.20 points

Ageing is the largest driver, worth 3.20 points. The World Health Organization (WHO) estimates complete tooth loss at almost 7% of people aged 20 or older and 23% of people aged 60 or older, its oral health fact sheet of 17 March 2025 states, and oral diseases affect nearly 3.7 billion people. Douglas Insights estimates that about 30% of long-term upper denture wearers have too little bone for standard implants without grafting, which makes them the candidate pool for zygomatic and pterygoid anchorage.

Full-arch immediate loading: 2.80 points

The shift to fixed full-arch teeth on the day of surgery adds 2.80 points. Straumann grew 2025 revenue 8.9% organically to CHF 2,605 million, with Europe, Middle East and Africa up 11.2% to CHF 1,084.2 million and Latin America up 18.3% to CHF 233.7 million, its full-year release of 18 February 2026 shows. Full-arch clinics that market “teeth in a day” refer the hardest cases, about 8% of their upper arches by our estimate, to zygomatic protocols instead of turning them away.

New product entries and clinical evidence: 2.10 points

New systems add 2.10 points. The US Food and Drug Administration (FDA) cleared zygoma systems from S.I.N. on 7 November 2025, Adin on 23 March 2026 and MegaGen on 18 June 2026, the 510(k) records above show, and MegaGen cited six studies of about 250 patients and 677 zygomatic implants with survival of 93% to 100%. For pterygoid implants, a systematic review of 38 studies, 2,245 patients and 3,053 pterygoid implants found a 10-year cumulative survival rate of 92.5%, the Journal of Clinical Medicine review of 3 August 2024 reports.

Value systems in Asia and Latin America: 1.50 points

Lower-priced systems add the last 1.50 points. Korean and Brazilian makers price zygomatic fixtures well below Western premium brands, a Douglas Insights estimate, which opens treatment in Brazil, Mexico, India and Southeast Asia. Summed, 3.20 + 2.80 + 2.10 + 1.50 = 9.60 points; after 1.60 points of restraints the 8.00% volume leg multiplied by the 1.00% price leg gives the 9.08% revenue rate.

What holds back zygomatic and pterygoid implant adoption: surgical risk, cost or procurement?

Three restraints remove 1.60 points a year from volume growth: scarce surgical skill and complication risk take 0.70 points, out-of-pocket cost 0.60 points and Chinese procurement 0.30 points.

Surgical skill is the main brake. Zygomatic drilling passes close to the orbit and through or beside the maxillary sinus, and sinusitis, soft tissue problems and fixture loss are the complications surgeons cite most; the Adin clearance limits use to ZAGA-4 extra-maxillary placement, its 510(k) indications state. Douglas Insights estimates that fewer than 6,000 surgeons worldwide place zygomatic implants regularly, so training capacity caps volume growth.

Cost is the second brake. A full upper arch on four zygomatic implants is a cash purchase for most patients because private insurers and public schemes rarely pay for fixed full-arch teeth; Douglas Insights estimates a typical US treatment fee of USD 25,000 to USD 40,000 per arch, of which implants and parts are under 10%. Procurement is the third. Straumann’s Asia Pacific revenue fell 12.8% organically in the fourth quarter of 2025, which the company linked to the pending second round of China’s implant volume based procurement, its full-year release shows; remote anchorage cases in Chinese public hospitals wait for the same price reset.

Which region places the most zygomatic and pterygoid implants, and where does demand grow fastest?

Europe places the most by value, about USD 54.6 million or 33.0% of 2025 sales, led by Spain, Italy, Germany and Portugal. Asia Pacific grows fastest at 10.6% a year to USD 108.8 million by 2035.

Region Value 2025 Share 2025 Value 2026 CAGR 2026-2035 Value 2035
Europe USD 54.6 million 33.0% USD 59.0 million 8.0% USD 118.0 million
North America USD 44.7 million 27.0% USD 48.6 million 8.6% USD 102.0 million
Asia Pacific USD 39.7 million 24.0% USD 44.0 million 10.6% USD 108.8 million
Latin America USD 18.2 million 11.0% USD 20.1 million 10.4% USD 49.0 million
Middle East and Africa USD 8.3 million 5.0% USD 9.1 million 9.8% USD 21.1 million

Europe leads because the zygomatic technique began in Sweden and the ZAGA anatomy-guided approach was developed in Spain, so the densest groups of trained surgeons work in Southern and Northern Europe. European value grows 8.0% as full-arch clinics spread. North America, USD 44.7 million, grows 8.6%, supported by the three new US clearances between November 2025 and June 2026, the K251129, K252031 and K251232 records show, and by dental service organisations that run full-arch centres.

Asia Pacific, USD 39.7 million, grows fastest at 10.6% as Korean makers such as MegaGen sell domestically and in China, India and Southeast Asia. Latin America, USD 18.2 million, grows 10.4%, helped by Brazil’s large implant industry and by Straumann’s 18.3% organic regional growth in 2025, its full-year release shows. The Middle East and Africa, USD 8.3 million, grow 9.8%, with South Africa home to Southern Implants. Regional values sum to USD 180.7 million in 2026, 0.06% above the headline, and USD 398.9 million in 2035, 1.02% above it.

Which companies lead zygomatic and pterygoid implants, and how concentrated is the field?

Nobel Biocare, part of Envista, leads with about 24% of 2025 value, a Douglas Insights estimate, ahead of the Straumann Group at about 20% and Southern Implants at about 11%. The top three hold about 55%.

Company Base and position Latest filed figure Estimated share 2025
Nobel Biocare (Envista) Zurich; original zygoma system Envista sales USD 2,719.5 million, 2025 24%
Straumann Group (incl. Neodent) Basel; ZAGA zygomatic line Revenue CHF 2,605 million, 2025 20%
Southern Implants Irene, South Africa; zygomatic and oncology lines Private; not published 11%
Noris Medical Nesher, Israel; zygomatic and pterygoid Private; predicate device K151909 6%
MegaGen Daegu, South Korea; zygoma system FDA clearance 18 June 2026 5%
BioHorizons (Henry Schein) Birmingham, Alabama; Pro Zygoma Launched 16 June 2026 3%

Envista grew 2025 sales 8.3% to USD 2,719.5 million with 6.5% core growth and guides 2% to 4% core growth for 2026, its fourth quarter release of 5 February 2026 shows; Nobel Biocare introduced the first commercial dental implant and the first zygoma system, so its share is a Douglas Insights estimate. Straumann sold more than one million iEXCEL implants in 2025 and guides high single-digit organic growth for 2026, its full-year release states.

The challenger tier is growing. The MegaGen and Adin clearances both cite the Noris Medical zygomatic implant as predicate, and the MegaGen file lists the ZAGA system as a reference device, the K251232 record shows. S.I.N. of Brazil, Adin of Israel and a group of Korean, Turkish and Chinese makers share most of the remaining 31%.

What does a zygomatic or pterygoid implant cost the clinic, and how fast does the price rise?

A remote anchorage implant costs the clinic an average USD 690 in 2025 including its abutment and a share of the surgical kit, Douglas Insights estimates, from about USD 380 for a pterygoid fixture with its abutment to about USD 1,050 for a premium zygomatic one.

The average sits between the two because about 45% of 2025 units are zygomatic and 55% pterygoid, but zygomatic fixtures carry 71% of fixture value. Premium Western zygomatic fixtures sell at list prices several times those of a standard implant, while Korean, Brazilian and Israeli systems undercut them by 30% to 50%, Douglas Insights estimates. Angled abutments add about USD 120 per implant, and a full zygomatic surgical kit with long drills, depth gauges and guides lists at several thousand dollars, which clinics recover across many cases.

Two forces set the 1.00% price leg. Mix shift toward zygomatic fixtures and digital planning adds about 2.0% a year; competition from new US-cleared systems and procurement pressure in China take 1.0 point back. The average reaches about USD 697 in 2026 and USD 762 in 2035, a Douglas Insights estimate. By comparison, the average realised price of a standard dental implant with components was USD 290 in 2025 in our Dental Implants Market study, so a remote anchorage implant earns about 2.4 times as much.

Which rules govern zygomatic and pterygoid implants, from FDA 510(k) to EU MDR and China’s tenders?

Three layers govern the market: US 510(k) clearance under product code DZE as Class II endosseous implants, the EU Medical Device Regulation 2017/745, and China’s volume based procurement for implants.

In the US, zygomatic implants are cleared as endosseous dental implants under 21 CFR 872.3640 with product codes DZE and NHA, the K251129 and K252031 records show, and the FDA often limits indications, for example to ZAGA Class 4 cases in the MegaGen clearance. Each new length range needs fatigue testing, and clinical data are optional: the S.I.N. file relied on bench tests alone. Douglas Insights estimates a 510(k) for a zygoma line costs a mid-sized maker under USD 1 million, a modest barrier that explains why three systems were cleared in eight months.

In Europe, implants fall under Regulation (EU) 2017/745, which requires notified body review, clinical evaluation and post-market follow-up, and the higher evidence cost favours makers with long zygomatic records. In China, the National Healthcare Security Administration’s implant tenders cap fixture prices in public hospitals, and the pending second round slowed orders in late 2025, Straumann’s results of 18 February 2026 show. Douglas Insights treats a deeper price cut for long implants in that round as a risk in the slower case, worth about 1.0 point of price growth.

What if training spreads faster or slower: how large is the zygomatic and pterygoid implants market in 2035?

The zygomatic and pterygoid implants market ends 2035 between USD 282.9 million and USD 523.6 million across three cases, around a USD 394.9 million base. Surgeon training and full-arch clinic growth set most of the spread.

Case Volume leg Price leg Revenue rate 2035 value
Slower 5.50% 0.00% 5.50% USD 282.9 million
Base case 8.00% 1.00% 9.08% USD 394.9 million
Faster 10.00% 2.00% 12.20% USD 523.6 million

In the slower case China’s second tender cuts long implant prices, complication reports slow adoption and full-arch clinics consolidate, taking 2.5 points off volume and 1.0 point off price. In the faster case guided and navigated zygomatic surgery cuts learning time, and Asian and Latin American clinics adopt pterygoid protocols widely, adding 2.0 points to volume and 1.0 point to price. One extra point of yearly revenue growth adds about USD 38 million to 2035 value, Douglas Insights estimates.

Company signals bracket the base case. Straumann guides high single-digit organic growth for 2026 and Envista 2% to 4% core growth, the Straumann and Envista releases show; our 9.08% rate sits above both because remote anchorage is a small, fast-growing niche inside their wider implant and dental portfolios.

Douglas Exclusive: the Remote Anchorage Readiness Index

The Remote Anchorage Readiness Index is a Douglas Insights model built from 9 inputs: 4 FDA clearance records, 2 company results releases, 1 product launch, 1 WHO fact sheet and 1 systematic review. It scores how ready each region is to switch graft cases to remote anchorage.

Date Input What it moves
3 August 2024 Pterygoid review: 92.5% 10-year survival Clinical confidence
17 March 2025 WHO: 23% of over-60s edentulous Candidate pool
7 November 2025 S.I.N. zygoma system cleared US supply
5 February 2026 Envista 2025 results and 2026 guidance Leader demand
18 February 2026 Straumann 2025 results, APAC Q4 down 12.8% Procurement risk
23 March 2026 Adin Touareg-OS zygomatic cleared US supply
9 June 2026 MegaGen 510(k) summary: 677-implant evidence Clinical confidence
16 June 2026 BioHorizons Pro Zygoma launch Distribution reach
18 June 2026 MegaGen zygoma system cleared US supply

The finding, a Douglas Insights estimate: in Europe about 9% of severely resorbed upper arches treated with fixed teeth now use zygomatic or pterygoid anchorage instead of grafting, in North America about 7% and in Asia Pacific about 3%, so Asia holds the largest untapped pool. The four US clearances and launches inside eight months lift the North American score most, because supply and distribution, not clinical proof, were its weakest inputs.

The index changes how to read 2027. If China’s second tender sets a separate price band for long implants, Douglas Insights expects Asian volume to grow faster than the 10.6% regional base; if it folds them into the standard implant price, premium makers are likely to hold back zygomatic launches there.

How does the methodology turn 240,000 implants into USD 165.6 million?

How this report is built

  • Every figure carries a confidence grade in the fact sheet above, and the working model ships with every licence.
  • Five regional models sum to the global figure, with country tables in the Excel model.
  • The next scheduled review of this study is May 2027.
  • Licence holders receive it as a maintained tab in the Excel model.

Our methodology multiplies about 240,000 zygomatic and pterygoid implants placed in 2025 by an average USD 690 realised price including components to reach USD 165.6 million, then grows volume 8.00% and price 1.00% a year.

The model covers 4 product types, 3 care settings and 5 regions, using 44 data points, 14 sourced and 30 modelled. Volume comes from the 21.5 million standard implants in our dental implants model, of which remote anchorage takes about 1.1%, cross-checked against the number of trained surgeons and cases per surgeon; all are Douglas Insights estimates. Arithmetic: 240,000 x USD 690 = USD 165.6 million; USD 165.6 million x (1.0800 x 1.0100)^10 = USD 394.9 million. Implants placed reach about 259,000 in 2026 and 518,000 in 2035.

Two cross-checks agree. First, 6,000 regular surgeons placing about 40 remote anchorage implants a year each gives 240,000 implants. Second, our estimated 55% top-three share implies about USD 91 million of revenue for Nobel Biocare, Straumann and Southern Implants, well under 2% of their combined dental implant revenue, consistent with a niche product line. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our editorial standards explain.
