How large is the Europe Hospital Management Software market? USD 8.73 billion in 2025, rising to USD 18.58 billion by 2035 at 7.85% a year. The Europe Hospital Management Software market covers the licences, subscriptions and maintenance that hospitals in Germany, the United Kingdom, France, Italy, Spain, the Nordics, Benelux and the rest of Europe pay for the systems that run admissions, clinical records, billing, departments and resources. Our receipt reads 1.642 million managed hospital beds times about USD 5,317 of yearly software spend per bed, which gives USD 8.73 billion. The trigger for the next buying wave is the European Health Data Space (EHDS) regulation, which entered into force on 26 March 2025 and sets certification duties for electronic health record (EHR) systems. Wider context sits on the healthcare IT hub, and every step follows our research methodology.
Which European hospital systems does this hospital management software estimate count, and which does it leave out?
Hospital management software in this study comprises six module groups worth USD 8.73 billion in 2025. They are EHR and clinical documentation, patient administration and billing, departmental systems, hospital ERP and supply chain, analytics and capacity management, and patient portals and engagement. Hardware, networks and stand-alone medical device software stay outside the count.
The unit is a managed bed: a staffed hospital bed whose admissions and records run on a contracted hospital information system rather than paper or a home-built tool. Douglas Insights counts 1.642 million such beds across Europe in 2025, out of about 2.3 million hospital beds in the European Union alone, as Eurostat reports for 2024. Buyers are university hospitals, general acute hospitals, and specialty and private clinics. Deployment is split across on-premise, private cloud and software as a service. Short version: if a hospital pays a software vendor to run its wards, it is in.
For readers who want the worldwide view, the Hospital Management Software Market report sizes the same category across every region, and this study zooms into European countries only.
How much does a European hospital pay per managed bed for hospital management software?
About USD 5,317 per managed bed a year in 2025 is the European average for hospital management software, covering licences, subscriptions and maintenance. The band runs from USD 2,160 in central and eastern Europe to USD 12,240 in the Nordics, because staff costs and module depth differ.
| Country group | Managed beds 2025 (thousand) | Software spend per bed 2025 (USD) | Spend 2025 |
|---|---|---|---|
| Germany | 412.0 | 4,820 | USD 1.99 billion |
| United Kingdom | 148.0 | 10,650 | USD 1.58 billion |
| France | 238.0 | 5,640 | USD 1.34 billion |
| Italy | 152.0 | 5,380 | USD 817.8 million |
| Spain | 118.0 | 5,860 | USD 691.5 million |
| Nordics | 61.0 | 12,240 | USD 746.6 million |
| Benelux | 92.0 | 7,180 | USD 660.6 million |
| Rest of Europe | 421.0 | 2,160 | USD 909.4 million |
The United Kingdom sits near the top at USD 10,650 per bed because, by our estimate, NHS trusts buy full EPR suites with hosting and long support terms. Germany averages USD 4,820, lower because, by our estimate, many of its 412 thousand managed beds sit on older licence-plus-maintenance contracts. France lands at USD 5,640, Italy at USD 5,380 and Spain at USD 5,860. Douglas Insights expects the European average to reach about USD 7,870 per bed by 2035, a 4.0% yearly price leg, as subscription contracts replace one-off licences.
Price is not the sticker. Charité in Berlin set a financial plan of around EUR 200 million for its Epic programme, covering licences, infrastructure and implementation support, according to the trade press. Licences are the smaller part.
Which hospital management software segment leads, EHR or patient administration?
EHR and clinical documentation leads European hospital management software with a 36.8% share, or USD 3.21 billion in 2025, because nursing and physician records are what funding programmes pay for first. Patient administration and billing follows at 17.4%.
| Segment | Share 2025 | Value 2025 | Growth rate 2026-2035 | Value 2035 |
|---|---|---|---|---|
| EHR and clinical documentation | 36.8% | USD 3.21 billion | 7.9% | USD 6.87 billion |
| Patient administration and billing | 17.4% | USD 1.52 billion | 5.7% | USD 2.64 billion |
| Departmental systems | 16.2% | USD 1.41 billion | 7.0% | USD 2.78 billion |
| Hospital ERP and supply chain | 13.1% | USD 1.14 billion | 5.4% | USD 1.94 billion |
| Analytics and capacity management | 10.2% | USD 890.5 million | 9.8% | USD 2.27 billion |
| Patient portals and engagement | 6.3% | USD 549.9 million | 13.6% | USD 1.97 billion |
EHR and clinical documentation grows at 7.9% to USD 6.87 billion, because electronic documentation of nursing and treatment is a named funding item under Germany’s KHZG. Patient administration and billing, worth USD 1.52 billion, grows slower at 5.7% since most hospitals already bill electronically. Departmental systems for laboratory, radiology and pharmacy hold USD 1.41 billion and grow at 7.0%, carried by digital medication management. Hospital ERP and supply chain is worth USD 1.14 billion and grows at 5.4%, the slowest line, because finance suites change less often than clinical systems. Analytics and capacity management reaches USD 890.5 million today and grows at 9.8% on bed-flow and theatre planning.
Patient portals and engagement is the fastest-growing segment at 13.6% a year, from USD 549.9 million to USD 1.97 billion. The reason is regulation: patient portals are a KHZG funding item, and the EHDS gives every patient the right to access their records electronically.
Why are KHZG grants and NHS frontline digitisation pushing hospital software demand across Europe?
Four public programmes add 3.7 points a year to managed-bed growth in European hospital management software: German KHZG grants, NHS England frontline digitisation, EHDS certification and national replacement programmes. Together they lift managed beds from 1.642 million to about 2.36 million by 2035.
Germany’s hospital future fund
Germany’s Krankenhauszukunftsgesetz (KHZG) put up to EUR 4.3 billion behind hospital digitisation, with EUR 3 billion of federal money flowing into the hospital future fund from 1 January 2021 and states or operators covering 30%. Funded items include patient portals, electronic nursing documentation and digital medication management. Douglas Insights models KHZG projects as worth 1.3 points of European managed-bed growth, since German beds are 25.1% of the managed total and many funded projects are still going live.
NHS England frontline digitisation
NHS England’s Frontline Digitisation Programme carries GBP 1.9 billion so that every trust reaches a baseline digital capability, and 95% of trusts are expected to have implemented or upgraded an electronic patient record (EPR) by March 2026, according to NHS England. Epic went live at the Royal National Orthopaedic Hospital on 23 November 2025 under a connect model with University College London Hospitals. We give the NHS programme 0.8 points, because English trusts that hit baseline next buy modules on top.
EHDS certification and replacement cycles
The EHDS adds 1.0 point. From 2029, patient summaries and ePrescriptions must be exchangeable in all member states, and from 2031 images, lab results and discharge reports follow, which forces hospitals running uncertified systems to replace or upgrade them. National replacement programmes add the last 0.6 points: Ireland’s HSE plans to bring all 19 maternity hospitals and units onto its Oracle Health maternity record within five years, as the 31 July 2025 Oracle release states. 1.3 plus 0.8 plus 1.0 plus 0.6 equals the 3.7-point volume leg.
The price leg runs alongside. Douglas Insights tracks a 4.0% yearly rise in hospital management software spend per managed bed, from about USD 5,317 in 2025 to USD 7,870 in 2035, as hospitals add portals, analytics and medication modules to the record they already own. Germany shows the pattern: 634,000 hospital beds in 2024, per Eurostat, and only 412 thousand of them on a managed platform in 2025. Each German bed that moves onto a modern suite adds about USD 4,820 a year of software spend. In England the 95% EPR baseline means new money goes to depth, not coverage.
Which budget and staffing hurdles slow hospital software roll-outs in European hospitals?
Three hurdles remove about 1.4 points a year from what European hospital management software volume would otherwise reach: hospital deficits that delay contracts, scarce implementation staff, and the cost of certification and cyber security. Without them, managed beds would grow nearer 5.1% a year.
Hospital budgets take 0.6 points. Where capital depends on a state grant, as under KHZG with its 30% state or operator share, a signed hospital information system contract often waits for the budget line. Douglas Insights reckons these funding waits cost 0.6 points of managed-bed growth every year to 2035.
Implementation capacity takes 0.5 points. A large EHR go-live needs a project team that can peak near 200 people, as InterSystems reported for the TrakCare roll-out at Gemelli in Rome. Charité in Berlin targets the end of 2029 for full Epic coverage, four years after selection. Cyber security and EHDS certification cost take the last 0.3 points, since smaller vendors face conformity work that larger rivals spread over more customers.
Which vendors win hospital information system tenders, from Dedalus to Epic?
Douglas Insights puts Dedalus at an estimated 11.6% share of European hospital management software in 2025, ahead of Oracle Health and Epic Systems; the top three hold about 23.7%. The field stays fragmented because hospital software is bought country by country.
| Company | Estimated share 2025 | Position built on |
|---|---|---|
| Dedalus | 11.6% | 1,848 inpatient hospitals outside the US; ORBIS in DACH and France |
| Oracle Health | 6.9% | Ireland’s maternity record covering over half of national births |
| Epic Systems | 5.2% | UCLH, RNOH and Charité selection |
| CompuGroup Medical | 3.9% | Hospital information systems revenue of EUR 319.0 million in 2024 |
| Nexus AG | 2.4% | Revenue of EUR 261.5 million in 2024, sold in 42 countries |
| InterSystems | 2.6% | TrakCare at Gemelli in Rome and Ireland’s National Rehabilitation Hospital |
Dedalus leads on installed base. KLAS data cited in its 17 July 2025 release place Dedalus in 1,848 inpatient hospitals outside the US, and the company signed 24 organisations in 8 countries in 2024 while leading all EHR vendors on newly contracted beds. Oracle Health holds an estimated 6.9%, built on the former Cerner installed base and national programmes such as the Irish maternity record. CompuGroup Medical reported EUR 319.0 million of hospital information systems revenue in 2024, out of group revenue of EUR 1.15 billion, which supports an estimated 3.9% share.
Epic Systems is the challenger. Its European base grew from UCLH in 2019 to the Royal National Orthopaedic Hospital in November 2025, and Charité selected Epic, subject to supervisory board approval, to replace a hospital information system used for about 20 years, as reported in December 2025. Nexus AG, a German specialist, grew revenue 8.3% to EUR 261.5 million in 2024 and sells in 42 countries. InterSystems runs TrakCare at Gemelli, linking about 30 departmental systems for some 3,500 users. Smaller national specialists share the remainder, which Douglas Insights puts at about 67.4% of spending.
Where in Europe does hospital management software spending grow fastest, Germany or the east?
Germany leads European hospital management software with USD 1.99 billion in 2025, a 22.75% share, but the rest of Europe grows fastest at 9.82% a year from the lowest spend per bed. Country growth rates run from 5.9% in the Nordics to 9.82% in the rest of Europe.
Germany grows at 8.4% to USD 4.45 billion as KHZG-funded projects go live. The United Kingdom holds USD 1.58 billion and grows at 7.1% to USD 3.13 billion, slower because most trusts already have an EPR. France is worth USD 1.34 billion, growing at 7.5% to USD 2.77 billion as hospital groups prepare for EHDS exchange duties. Italy holds USD 817.8 million and grows at 8.0%. Spain is worth USD 691.5 million and grows at 8.6% as regional health services prepare for the 2029 EHDS deadline.
The Nordics hold USD 746.6 million, growing at 5.9%, the slowest pace, because Nordic spend per bed, USD 12,240, is already the highest in Europe and leaves the least room to climb. Benelux is worth USD 660.6 million and grows at 6.6%. The rest of Europe, from Poland and Czechia to Portugal and Greece, holds USD 909.4 million and grows at 9.82% to USD 2.32 billion, because spend per bed of USD 2,160 has the furthest to climb. The wildcard is Italy: its 8.0% rate depends on regional health budgets, so it swings more than any other large market.
How do the EHDS regulation and EU certification rules change hospital EHR procurement?
The EHDS regulation, in force since 26 March 2025, makes certification compulsory for EHR systems sold in the EU, with most duties applying from 2029. Douglas Insights calculates that it supplies 1.0 point of the 3.7-point volume leg in hospital management software.
The European Commission records publication in the Official Journal on 5 March 2025 and entry into force on 26 March 2025. Implementing acts follow by 2027. By 2029 patient summaries and ePrescriptions must flow across all member states; by 2031 medical images, lab results and hospital discharge reports join them. For a hospital, that means the EHR it buys in 2026 must pass conformity checks by 2029. Germany layers KHZG rules on top, and NHS England sets its own minimum digital foundations for trusts. Three rulebooks, one procurement file.
Is software as a service replacing on-premise hospital information systems in Europe?
Software as a service holds an estimated 21.7% of European hospital management software spending in 2025, against 52.4% on-premise and 25.9% private cloud, by Douglas Insights estimates. Subscription contracts lift the price leg to 4.0% a year.
On-premise installs still dominate in Germany and Italy in our model. Private cloud carries the largest share in the United Kingdom in our model. Software as a service grows fastest among specialty and private clinics, which lack in-house teams. Hosting moves cost from capital budgets to operating budgets, and that matters in hospitals where capital needs a state grant.
Which buyers sign the largest European hospital software contracts, university hospitals or private clinics?
University hospitals sign the largest hospital management software contracts in Europe, with single programmes near EUR 200 million, while general acute hospitals supply most of the 1.642 million managed beds. Specialty and private clinics buy smaller, faster subscriptions.
University hospitals carry research, teaching and complex care in one record, and both Charité and UCLH chose Epic as a single platform. General acute hospitals tend to buy through regional groups or national frameworks. Specialty and private clinics favour modular suites with quick go-live times.
What if KHZG projects stall: how far does the 2035 hospital management software forecast move?
Our base case reaches USD 18.58 billion in 2035 from 3.7% volume and 4.0% price growth; the slower case lands at USD 15.31 billion and the faster case at USD 22.08 billion. A KHZG stall is the main slower-case trigger for European hospital management software.
The slower case uses 2.4% volume and 3.3% price: German projects slip past their go-live dates, and EHDS deadlines drift. The faster case uses 4.9% volume and 4.6% price: the 2029 EHDS duties trigger a replacement rush, and Epic’s Charité selection pulls more German university hospitals onto modern suites. Add one point to volume growth and 2035 spending rises by about USD 1.87 billion. Our 7.85% sits at the low end of published forecasts, which run from about 6.6% to 17.1% a year, because our scope excludes hardware and broad healthcare IT services. The 26 March 2025 EHDS start date anchors the base case timing.
Douglas Exclusive: the European Hospital Software Funding Atlas
The European Hospital Software Funding Atlas is a Douglas Insights model built from 22 inputs. Those are 8 country bed counts, 8 spend-per-bed estimates and 6 sourced public programme figures from KHZG, NHS England, the EHDS and Ireland’s HSE. It maps where public money meets hospital software demand.
| Country group | Public funding anchor | Managed beds 2025 (thousand) | Growth rate 2026-2035 |
|---|---|---|---|
| Germany | KHZG, up to EUR 4.3 billion | 412.0 | 8.4% |
| United Kingdom | Frontline Digitisation, GBP 1.9 billion | 148.0 | 7.1% |
| France | National programmes, not quantified here | 238.0 | 7.5% |
| Italy | National programmes, not quantified here | 152.0 | 8.0% |
| Spain | Regional health services | 118.0 | 8.6% |
| Nordics | No pan-Nordic programme quantified here | 61.0 | 5.9% |
| Benelux | No national programme quantified here | 92.0 | 6.6% |
| Rest of Europe | EHDS deadlines; Ireland HSE maternity record | 421.0 | 9.82% |
The finding: the two country groups with quantified public funds, Germany and the United Kingdom, hold 40.8% of 2025 hospital management software spending but only 34.1% of managed beds. Funded money buys depth per bed, not more beds. The atlas is an estimate, not an official register, and we will refresh it as each country publishes its programme figures.
How the model counts managed beds and prices European hospital management software?
The model covers 8 country groups and about 30 countries, adds 1.642 million managed beds, and multiplies by USD 5,317 per bed to reach USD 8.73 billion. Each group is built separately and summed. In the data table, the row labelled Global means the Europe total of USD 8.73 billion, not the world.
Bed counts start from Eurostat’s 2.3 million EU hospital beds in 2024, including 634,000 in Germany, plus non-EU beds in the United Kingdom, Norway and Switzerland. We then apply a managed share per country, 65.0% in Germany for example. Spend per bed comes from vendor revenue anchors: CompuGroup Medical hospital revenue, Nexus revenue and Dedalus installed base. Cross-check one: CompuGroup Medical’s EUR 319.0 million of hospital revenue, converted to dollars, lands close to the USD 340.5 million, or 3.9%, that our model assigns it, before removing its non-European sales. Cross-check two: our 7.85% sits within 1.25 points of the lowest published rate.
How does the European hospital management software total compare with the global market?
Europe’s USD 8.73 billion hospital management software total grows at 7.85% a year, slower than the 9.4% global rate in our worldwide study, because European hospitals start from a deeper installed base.
Connected-ward spending sits in a separate study, the Smart Hospital Market, which covers monitoring, nurse call and command centres that sit on top of the hospital information system. The software layer described here is what those devices report into.
