The US Food and Drug Administration granted traditional approval on 24 February 2026 to encorafenib (Braftovi) with cetuximab and fluorouracil-based chemotherapy for BRAF V600E metastatic colorectal cancer, after the BREAKWATER trial lifted median overall survival to 30.3 months from 15.1 months, the FDA notice states. The colorectal cancer market covers prescription drugs used to treat colon and rectal cancer: chemotherapy, anti-angiogenic agents, anti-EGFR antibodies, immune checkpoint inhibitors and biomarker-targeted drugs, valued at manufacturer net prices. Douglas Insights sizes it at USD 12.4 billion in 2025, about 1.10 million drug-treated patients at an average USD 11,300 each, rising to USD 21.6 billion in 2035, a 5.68% compound annual growth rate (CAGR). The study sits in our pharmaceuticals coverage, beside the Oncology Radiopharmaceuticals Market, Circulating Tumor Cell Diagnostics Market and Hospital Endoscopy Equipment Market reports; every figure follows the Douglas Insights research methodology.

Which colorectal cancer drug class earns most: chemotherapy, anti-angiogenics or immunotherapy?

Chemotherapy still earns the most, 26.0% of 2025 colorectal cancer drug value or about USD 3.23 billion, Douglas Insights estimates. Biomarker-targeted drugs grow fastest, 10.6% a year to USD 3.06 billion by 2035.

Drug class Share 2025 Value 2025 CAGR 2026-2035 Value 2035
Chemotherapy (fluoropyrimidines, oxaliplatin, irinotecan, trifluridine/tipiracil) 26.0% USD 3.23 billion 2.8% USD 4.26 billion
Anti-angiogenic agents (bevacizumab and biosimilars, ramucirumab, aflibercept, fruquintinib, regorafenib) 24.0% USD 2.98 billion 3.2% USD 4.08 billion
Anti-EGFR antibodies (cetuximab, panitumumab) 19.0% USD 2.36 billion 5.2% USD 3.92 billion
Immune checkpoint inhibitors (pembrolizumab, nivolumab, ipilimumab) 15.0% USD 1.86 billion 9.4% USD 4.58 billion
Biomarker-targeted drugs (BRAF, KRAS G12C, HER2) 9.0% USD 1.12 billion 10.6% USD 3.06 billion
Other and newly approved agents 7.0% USD 870 million 5.6% USD 1.50 billion

Chemotherapy leads because almost every treated patient receives it: FOLFOX, CAPOX or FOLFIRI in the adjuvant setting and as the backbone of first- and second-line metastatic regimens. Generic oxaliplatin, irinotecan and capecitabine are cheap, so most of the class value comes from trifluridine/tipiracil (Lonsurf). Otsuka, whose Taiho unit sells it, reported Lonsurf sales of JPY 109.3 billion in 2025, up 4.7%, lifted by US use with bevacizumab, its notice for the 2026 shareholders’ meeting states. The class grows 2.8% a year to USD 4.26 billion.

Anti-angiogenic agents, USD 2.98 billion, grow only 3.2% because bevacizumab biosimilars have cut the price of the largest molecule. Fruquintinib is the growth engine inside the class: HUTCHMED said in-market sales of Fruzaqla reached USD 185.4 million in the first half of 2026, up 14%, in its interim results of 30 July 2026. Anti-EGFR antibodies, USD 2.36 billion, grow 5.2% as they pair with new BRAF and KRAS G12C drugs. Checkpoint inhibitors, USD 1.86 billion, treat the 4% to 5% of metastatic tumours that are microsatellite instability-high (MSI-H) or mismatch repair-deficient (dMMR), a Douglas Insights estimate, and grow 9.4% as use moves earlier. Targeted drugs, USD 1.12 billion, grow 10.6%. Class values for 2035 sum to USD 21.4 billion, 0.90% below the headline.

What drives colorectal cancer drug demand: more patients, earlier onset or longer treatment?

Treated patient volume rises 2.6% a year in our base case, from about 1.10 million in 2025 to about 1.42 million in 2035. Four drivers add 3.40 points of gross growth; restraints remove 0.80 points.

Rising incidence and ageing populations: 1.00 point

Population growth and ageing are worth 1.00 point. The International Agency for Research on Cancer counts 2,041,007 new colorectal cancer cases and 917,895 deaths worldwide in GLOBOCAN 2024, third for incidence and second for mortality, its colorectum fact sheet dated 8 July 2026 shows. That is up from about 1.9 million cases in 2022, the WHO fact sheet of 13 February 2026 states. Asia carries 1,064,396 cases, or 52.2%, and Europe 527,533, or 25.8%, the IARC sheet shows. Douglas Insights estimates that about 54% of new patients, plus a pool of recurrent metastatic patients, receive systemic drugs each year, giving the 1.10 million treated base.

Early-onset disease and screening from age 45: 0.60 points

Younger patients add 0.60 points. The American Cancer Society expects 158,850 new US cases and 55,230 deaths in 2026, with incidence at ages 20 to 49 rising about 3% a year and 45% of new cases now in adults under 65, against 27% in 1995, its release of 2 March 2026 states. Three in four early-onset cases are found at an advanced stage, and 27% at distant stage, its 2026 fast facts show. Younger patients tolerate more lines of therapy and live longer on treatment, so each one is worth about 1.4 times the drug spend of an older patient, a Douglas Insights estimate.

Longer survival and more lines of therapy: 1.00 point

Longer treatment adds 1.00 point. BREAKWATER doubled median overall survival in BRAF V600E disease to 30.3 months, and median progression-free survival rose to 12.8 months from 7.1 months, the FDA notice of 24 February 2026 shows. Exelixis’s zanzalintinib with atezolizumab improved overall survival against regorafenib in the 901-patient STELLAR-303 trial and has a US decision date of 3 December 2026, its release of 2 February 2026 states. Douglas Insights estimates the average metastatic patient now receives 2.6 lines of drug therapy, against about 2.2 a decade ago.

Wider access in China, India and Latin America: 0.80 points

Access gains add 0.80 points. Fruquintinib sales in China, sold as Elunate, rose 41% to USD 60.8 million in the first half of 2026, HUTCHMED’s 30 July 2026 results show, and Merck KGaA’s Erbitux sales in Latin America rose 31.3% to EUR 138 million in 2025, its 2025 annual report shows. Biosimilar antibodies also widen access: Biocon won FDA approval for its bevacizumab biosimilar Jobevne on 10 April 2025, its release states. Summed, 1.00 + 0.60 + 1.00 + 0.80 = 3.40 points, a Douglas Insights estimate.

What holds colorectal cancer drug volume back: screening success or treatment gaps?

Two restraints remove 0.80 points a year from treated volume: falling incidence in older, well-screened adults takes 0.40 points, and untreated or under-treated patients in low-income countries take 0.40 points.

Screening is a restraint on drug volume because it removes polyps before they become cancer and finds tumours at stages cured by surgery alone. Incidence in US adults aged 65 and over is falling by more than 2% a year, the American Cancer Society reported on 2 March 2026. Blood tests widen the funnel: Guardant’s Shield became the first blood test approved by the FDA as a primary screening option on 29 July 2024, its filing states, and Exact Sciences, maker of Cologuard, booked USD 2.53 billion of screening revenue in 2025, its results of 13 February 2026 show. Douglas Insights estimates each 10-point rise in screening uptake in a country cuts its metastatic treated pool by about 4% within a decade.

Treatment gaps are the second restraint. Africa records 77,464 cases a year, 3.8% of the world total, the IARC fact sheet shows, yet we estimate under one in three African patients receives a full chemotherapy course. Toxicity also ends treatment early: neuropathy from oxaliplatin and skin rash from anti-EGFR drugs stop about one patient in five before the planned cycles, a Douglas Insights estimate. Only 37% of US adults aged 45 to 49 are up to date with screening, the same ACS release notes, which slows the screening restraint in younger groups.

Which region spends most on colorectal cancer drugs, and where is growth fastest?

North America spends the most, USD 5.22 billion or 42.0% of 2025 colorectal cancer drug value, because US net prices for antibodies are the world’s highest. Asia Pacific grows fastest, 7.0% a year to USD 6.11 billion.

Region Value 2025 Share 2025 Value 2026 CAGR 2026-2035 Value 2035
North America USD 5.22 billion 42.0% USD 5.50 billion 5.3% USD 8.75 billion
Europe USD 3.11 billion 25.0% USD 3.25 billion 4.5% USD 4.83 billion
Asia Pacific USD 3.11 billion 25.0% USD 3.33 billion 7.0% USD 6.11 billion
Latin America USD 621 million 5.0% USD 660 million 6.2% USD 1.13 billion
Middle East and Africa USD 373 million 3.0% USD 398 million 6.6% USD 707 million

North America has only 9.5% of world cases, 194,641 in GLOBOCAN 2024, the IARC sheet shows, but 42% of value. Pfizer’s Braftovi and Mektovi sold USD 672 million in the United States out of USD 716 million worldwide in 2025, its results of 3 February 2026 show, which illustrates how US-weighted new launches are. North American spending grows 5.3% a year to USD 8.75 billion.

Europe, USD 3.11 billion, grows 4.5%, the slowest, because biosimilars and health technology assessment hold prices down; Erbitux sold EUR 483 million in Europe in 2025, Merck KGaA’s annual report shows. Asia Pacific, also USD 3.11 billion, holds more than half of patients but low prices, and grows 7.0% as China and India widen reimbursement. Latin America, USD 621 million, grows 6.2%; the Middle East and Africa, USD 373 million, grow 6.6%. Regional values sum to USD 13.1 billion in 2026 and USD 21.5 billion in 2035, 0.30% below the headline.

Which companies lead colorectal cancer therapy?

Amgen, Merck KGaA, Merck & Co., Bristol Myers Squibb, Pfizer, Roche, Taiho and Takeda with HUTCHMED hold about 60% of colorectal cancer drug value, Douglas Insights estimates; Amgen leads with about 11%.

Company Colorectal product Latest reported figure Source date
Amgen Vectibix, Lumakras, Mvasi Q3 2026 Vectibix USD 335 million, up 18% 3 November 2026

Pfizer Braftovi, Mektovi Nine months 2026 USD 642 million, up 24% 3 November 2026
Merck & Co. Keytruda, Keytruda Qlex 2025 sales USD 31.7 billion, all indications 3 February 2026
Bristol Myers Squibb Opdivo, Yervoy Q3 2026 Opdivo USD 2.37 billion, Yervoy USD 793 million 29 October 2026
Otsuka (Taiho) Lonsurf 2025 sales JPY 109.3 billion, up 4.7% 2026 meeting notice
Takeda, HUTCHMED Fruzaqla, Elunate H1 2026 Fruzaqla USD 185.4 million, up 14% 30 July 2026
Roche Avastin 2025 sales CHF 973 million, down 17% 29 January 2026

Amgen leads on two antibodies. Vectibix sales rose 18% to USD 335 million in the third quarter of 2026, “primarily driven by volume growth”, while its bevacizumab biosimilar Mvasi fell 21% to USD 168 million on an 11% lower net price, its earnings release of 3 November 2026 shows. Merck KGaA’s Erbitux reached EUR 1.18 billion in 2025, up 1.2% reported and 6.6% organic, its annual report shows; Douglas Insights estimates about three-quarters of that is colorectal use. Pfizer’s Braftovi and Mektovi rose 21% to USD 244 million in the third quarter of 2026, its filing of 3 November 2026 shows.

Checkpoint makers earn a small colorectal slice of very large brands. Merck & Co.’s Keytruda sold USD 31.7 billion in 2025, its results of 3 February 2026 show, and the subcutaneous Keytruda Qlex reached USD 830 million in the third quarter of 2026 alone, its third-quarter filing shows. Bristol Myers Squibb’s Opdivo fell 6% to USD 2.37 billion in the same quarter while Yervoy rose 7%, its filing of 29 October 2026 shows. Roche’s Avastin fell 17% at constant exchange rates to CHF 973 million in 2025 “because of biosimilar erosion”, its results of 29 January 2026 state. Servier, Bayer, Exelixis and Biocon complete the field.

How much does a month of colorectal cancer treatment cost, from generics to antibodies?

A month of colorectal cancer treatment ranges from under USD 100 for generic chemotherapy to over USD 15,000 for a US anti-EGFR or checkpoint antibody, Medicare payment limits show; our blended average is USD 11,300 per treated patient a year.

US Medicare Part B payment limits for October to December 2026 set the benchmark: USD 183.275 per 10 mg of panitumumab, USD 80.858 per 10 mg of cetuximab, USD 62.253 per mg of intravenous pembrolizumab and USD 0.087 per 0.5 mg of oxaliplatin, the CMS ASP pricing files show. For a 70 kg patient on panitumumab at 6 mg/kg every two weeks, that is about USD 15,400 a month; cetuximab at 500 mg/m² for a 1.9 m² patient is about USD 15,400; a 200 mg pembrolizumab dose costs about USD 12,450, Douglas Insights calculates. The same file pays USD 32.307 per mg for subcutaneous pembrolizumab.

Biosimilars set a lower track for bevacizumab. Branded Avastin is paid USD 75.492 per 10 mg against USD 28.294 for Mvasi and USD 21.139 for Vegzelma in the same CMS file, so a month at 5 mg/kg every two weeks falls from about USD 5,280 to under USD 2,000. Oral drugs carry European list prices: fruquintinib costs GBP 3,950 per 21 capsules of 5 mg in the UK, one 28-day cycle, the NICE appraisal TA1079 shows. Average spend per treated patient rises 3.0% a year in our base case, to about USD 15,200 in 2035, as mix moves to antibodies and targeted drugs while biosimilars cut about 0.8 points, a Douglas Insights estimate.

What if new targeted drugs arrive faster or prices fall: what is the 2035 colorectal cancer forecast?

The colorectal cancer drug market ends 2035 between USD 17.4 billion and USD 26.0 billion across our three cases, around a base of USD 21.6 billion. Launch pace and US pricing set most of the spread.

Case Patient leg Price and mix leg Revenue rate 2035 value
Slower 1.80% 1.60% 3.43% USD 17.4 billion
Base case 2.60% 3.00% 5.68% USD 21.6 billion
Faster 3.30% 4.20% 7.64% USD 26.0 billion

In the slower case, screening cuts metastatic incidence faster, zanzalintinib and other late-line drugs fail to gain wide reimbursement, and US price negotiation reaches oncology antibodies, taking 0.8 points off patients and 1.4 points off price. In the faster case, checkpoint inhibitors win use in earlier, localised MSI-H disease and KRAS G12C and HER2 combinations move into first line, adding 0.7 points of patients and 1.2 points of price. One extra point of yearly revenue growth adds about USD 2.0 billion to 2035 value, a Douglas Insights estimate.

Live signals sit at or slightly above the base case. Braftovi’s 24% nine-month growth and Vectibix’s 18% quarterly growth point up, while Mvasi’s 21% fall and Avastin’s 17% fall show biosimilar pressure. The signal to watch is the FDA decision on zanzalintinib due on 3 December 2026.

Which rules shape colorectal cancer drugs, from FDA approvals to price negotiation?

Three rule sets shape colorectal cancer drugs: regulator approvals of new biomarker combinations, US Medicare price negotiation and Part B inflation rebates, and the European Union’s joint clinical assessments for cancer medicines.

Approvals came quickly in 2025 and 2026. The FDA approved nivolumab with ipilimumab for MSI-H or dMMR colorectal cancer on 8 April 2025, its notice states, and sotorasib with panitumumab for KRAS G12C disease on 16 January 2025, its notice shows, following adagrasib with cetuximab on 21 June 2024. The European Commission approved encorafenib with cetuximab and FOLFOX in first line, Pierre Fabre announced on 19 June 2026, and the FDA approved subcutaneous Keytruda Qlex on 19 September 2025, its notice states.

On price, the US Centers for Medicare & Medicaid Services selected 15 drugs on 27 January 2026 for prices from 1 January 2028, the first round to include Part B drugs, and none is a colorectal cancer drug, its release shows. Inflation rebates already bite: panitumumab carries a reduced coinsurance of 17.864% in the October 2026 ASP file, a sign its price rose faster than inflation. In Europe, the Health Technology Assessment Regulation has applied since 12 January 2025, with joint clinical assessments starting with new cancer medicines, the Danish Medicines Agency explains. Screening rules support earlier diagnosis: the US Preventive Services Task Force lowered the screening start age to 45 in its 18 May 2021 recommendation.

Douglas Exclusive: the Colorectal Biomarker Shift Index

The Colorectal Biomarker Shift Index is a Douglas Insights model built from 12 inputs: 7 regulator and statistics notices and 5 company filings or releases. It measures how much colorectal cancer drug spending goes to therapies chosen by a tumour biomarker.

Date Input What it moves
21 June 2024 FDA: adagrasib plus cetuximab, KRAS G12C Targeted share
16 January 2025 FDA: sotorasib plus panitumumab, KRAS G12C Targeted share
8 April 2025 FDA: nivolumab plus ipilimumab, MSI-H/dMMR Checkpoint share
10 April 2025 FDA: Jobevne bevacizumab biosimilar Non-biomarker price
19 September 2025 FDA: Keytruda Qlex subcutaneous Checkpoint delivery
3 February 2026 Pfizer: Braftovi/Mektovi 2025 sales USD 716 million BRAF spend
24 February 2026 FDA: encorafenib traditional approval, BRAF V600E Targeted share
19 June 2026 EU: encorafenib first line Europe targeted share
8 July 2026 IARC GLOBOCAN 2024: 2,041,007 cases Patient base
30 July 2026 HUTCHMED: Fruzaqla up 14% Non-biomarker late line
October 2026 CMS ASP limits for antibodies Price per patient
3 November 2026 Amgen: Vectibix up 18% Anti-EGFR spend

The finding, a Douglas Insights estimate: biomarker-selected drugs, anti-EGFR antibodies for RAS wild-type tumours, checkpoint inhibitors for MSI-H tumours and BRAF, KRAS G12C and HER2 drugs, took 43% of colorectal cancer drug value in 2025 and reach 54% by 2035. Each tested subgroup is small, BRAF V600E about 8% of metastatic tumours and KRAS G12C about 3%, but a targeted regimen costs two to four times a chemotherapy-plus-bevacizumab regimen per month.

The index changes how to read the market. Growth now depends less on how many patients are diagnosed and more on how many tumours are tested for RAS, BRAF, MSI and HER2 at diagnosis. Douglas Insights therefore models testing rates by region, and expects Asia Pacific’s biomarker share to rise fastest, from about 30% to about 45%, as next-generation sequencing becomes routine.

How does the methodology turn 1.10 million patients into USD 12.4 billion?

How this report is built

  • Every figure carries a confidence grade in the fact sheet above, and the working model ships with every licence.
  • Five regional models sum to the global figure, with country tables in the Excel model.
  • The next scheduled review of this study is May 2027.
  • Licence holders receive it as a maintained tab in the Excel model.

Our methodology multiplies about 1.10 million drug-treated colorectal cancer patients in 2025 by an average USD 11,300 of drug spending each to reach USD 12.4 billion, then grows patients 2.6% and spend per patient 3.0% a year.

The model covers 6 drug classes and 5 regions, using 54 data points, 22 sourced and 32 modelled. Patients come from the GLOBOCAN 2024 case count of 2,041,007, multiplied by regional treatment rates of 35% to 75%, plus recurrent metastatic patients; spend per patient comes from regimen costs, line counts and net price discounts by region, Douglas Insights estimates. Arithmetic: 1.10 million x USD 11,300 = USD 12.4 billion; USD 12.4 billion x (1.026 x 1.030)^10 = USD 21.6 billion. Patients reach about 1.42 million in 2035.

Two cross-checks agree. First, the colorectal share of named products, Vectibix, Erbitux, Braftovi, Fruzaqla and Elunate, Lonsurf, Avastin and checkpoint inhibitors, sums to about USD 5.7 billion, about 46% of our value, in line with a market where generics and biosimilars hold the rest. Second, North America’s USD 5.22 billion over about 160,000 treated US and Canadian patients implies about USD 33,000 each, consistent with US antibody prices. Currency conversions use Douglas Insights assumptions of EUR 0.885, CHF 0.83 and JPY 150 per dollar. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our editorial standards explain.

What counts in the colorectal cancer study, and what is excluded?

The colorectal cancer study counts prescription drugs used to treat colon and rectal cancer at all stages, at manufacturer net prices. Screening tests, diagnostics, surgery, radiotherapy equipment and supportive drugs such as antiemetics are excluded.

The study splits value by 6 drug classes and 5 regions: North America, Europe, Asia Pacific, Latin America, and the Middle East and Africa. Molecular tests that select patients appear in our Circulating Tumor Cell Diagnostics Market report, endoscopic diagnosis in the Hospital Endoscopy Equipment Market study and drug manufacturing in the High Potency API Market report.
