On 1 July 2026 Galderma said the US Food and Drug Administration had issued a complete response letter on its liquid neuromodulator relabotulinumtoxinA, citing observations from a manufacturing site inspection and analytical method optimisation rather than any gap in safety or efficacy, according to the Galderma regulatory update. The product, sold as Relfydess, was already approved in 33 markets and launched in more than 20, so the letter kept the first ready-to-use liquid toxin out of the largest botulinum toxin market for at least another cycle. Four drivers lift botulinum toxin vial volume by 7.0% a year, and therapeutic use is the steadiest of them. AbbVie reported 2025 Botox Therapeutic net revenues of USD 3.77 billion, with the US business up 14.8% to USD 3.15 billion, according to its full-year 2025 results. Chronic migraine, spasticity and bladder injections are paid for by insurers and repeat three to four times a year, so each new patient adds 4 to 6 vials a year for as long as treatment works. Douglas Insights estimates therapeutic botulinum toxin vials grow 7.8% a year to 2035.

New aesthetic patients are the second driver. Galderma reported 2025 neuromodulator net sales of USD 1.47 billion, up 14.3% at constant currency, according to its 2025 results, and credited both Dysport and the Relfydess launch. Evolus sold USD 274.5 million of Jeuveau and Nuceiva in 2025 and reached more than 55% of US aesthetic accounts, according to its fourth-quarter results filing. Douglas Insights estimates aesthetic botulinum toxin vials grow 6.6% a year as first-time patients in their twenties and thirties start preventive treatment and men take a rising share of visits.

Asia Pacific access is the third driver. China approved its first domestic type A toxin decades ago but only a handful of imported brands until the last few years, and Korean makers such as Hugel, Daewoong and Medytox have since won licences across the region at prices 40% to 60% below Western brands. Douglas Insights estimates Asia Pacific botulinum toxin vial volume grows 12.9% a year, almost twice the global rate, because a lower price brings in patients who never paid Western list prices.

Label expansion is the fourth driver. Ipsen grew Dysport sales 9.7% at constant exchange rates to EUR 734.1 million in 2025 and expects pivotal Phase III readouts in chronic and episodic migraine in 2026, according to its 2025 results release. A second migraine brand would widen prescribing in a botulinum toxin indication that Douglas Insights sizes at USD 1.72 billion. Douglas Insights adds about 0.6 points a year to volume growth for approvals expected before 2030 in migraine, masseter reduction and platysma bands.

What holds back botulinum toxin sales after the 2025 US cosmetic dip?

Three restraints trim about 1.4 points a year from botulinum toxin growth, and weak US cosmetic spending is the most visible. AbbVie’s US Botox Cosmetic net revenues fell 4.3% in 2025 to USD 1.50 billion, and its whole aesthetics portfolio fell 6.1% to USD 4.86 billion. Consumers stretched the gap between visits from about 4 months to 5 or 6, which cuts annual vials per patient by up to a third without losing the patient. Douglas Insights holds US aesthetic botulinum toxin volume growth to 4.9% a year on that pattern.

Price competition in the aesthetic channel is the second restraint. Challenger brands enter the US at list prices 20% to 30% below Botox Cosmetic and fund loyalty points and volume rebates on top. Douglas Insights estimates rebates and loyalty programmes absorb 18% to 22% of gross aesthetic botulinum toxin sales in the United States, which limits the aesthetic price leg to 0.4% a year.

Manufacturing and regulatory risk is the third restraint. Botulinum neurotoxins are a Tier 1 select agent under the US Federal Select Agent Program, so every plant needs registration, security and inventory controls on top of biologics licensing, and the two complete response letters for relabotulinumtoxinA show how long a plant finding can delay entry. Douglas Insights removes about 0.3 points a year from botulinum toxin volume growth for launch delays and supply interruptions.
