Why are screen-and-treat programmes buying hand-held colposcopes faster than clinics retire stand-mounted scopes?

Unit shipments of hand-held colposcopes rise 6.1% a year to 2035. The WHO 90-70-90 elimination targets push screen-and-treat programmes, office gynecology swaps floor stands for portable digital units, and nurse-led telecolposcopy needs a device per outreach team rather than per hospital.

Programme scale-up is the largest pull at 2.6 points of the 6.1-point volume leg. The WHO cervical cancer fact sheet counts around 604,000 new cases and around 280,000 deaths, with the highest incidence and mortality in low- and middle-income countries. Its targets ask for 90% of girls vaccinated by 15, 70% of women screened with a high-performance test by 35 and again by 45, and 90% of women with precancer or cancer treated. Every HPV-positive woman who needs a look at the transformation zone before ablation adds colposcopy demand, and a hand-held colposcope at roughly USD 2,000 fits a district budget where a stand-mounted video system, which Douglas Insights prices near USD 9,000, does not.

Replacement of stand-mounted optics in private gynecology adds 1.9 points. A floor-stand binocular colposcope needs a dedicated room, while a hand-held colposcope with an HD sensor stores images straight into the patient record. Douglas Insights estimates that 34.8 million US dollars, or 46.1% of 2025 hand-held colposcope revenue, already comes from digital video units, the format that clinics pick when they retire analog scopes. Clinics that buy a hand-held unit also cut the time a second clinician spends reviewing images, because a stored still or clip travels by message instead of by appointment.

Task shifting and telecolposcopy add the final 1.6 points. What slows hand-held colposcope volumes when HPV-negative women wait five years between tests?

Three restraints would take about 1.9 points off hand-held colposcope unit growth in the slower case. They are longer screening intervals in rich systems, supplier exits that strand installed bases, and donor-funded procurement that buys in lumps rather than steadily.

Longer intervals remove up to 0.8 points. On 1 July 2025 GOV.UK published the extended screening interval clinical pathway protocol, under which participants aged 24.5 to 49 who test negative on a routine primary high-risk human papillomavirus (hrHPV) test move from a 3-year to a 5-year interval. Fewer routine screens mean fewer borderline referrals, so the replacement cycle for colposcopes in England and in systems that copy it stretches out.

Supplier exits remove 0.6 points in the slower case. DYSIS Medical, maker of the DYSIS View and DYSIS Ultra digital colposcopes with computer-aided cervical mapping, states on its own site that it was acquired by Apgar Danmark A/S effective 4 February 2025 and ceased operations on 24 April 2025. Buyers who watched an installed base lose its service partner now ask hand-held colposcope vendors for longer warranties, such as the three-year cover in the UNICEF specification, and that slows tender closes.

DYSIS Medical, whose DYSISmap quantified acetowhitening, was acquired by Apgar Danmark A/S effective 4 February 2025 and wound down from 24 April 2025, according to the DYSIS Medical site.
