Coloplast’s Voice and Respiratory Care business, home of Atos Medical, earned DKK 2.42 billion in the year to September 2026, up 8% organically, with laryngectomy products about 70% of it, the company’s results of 2 November 2026 show, while the International Agency for Research on Cancer (IARC) counts 193,937 new laryngeal cancers a year, per its GLOBOCAN 2024 fact sheet. The voice prosthesis devices and equipment market covers the products that let people speak and breathe after a total laryngectomy: indwelling and patient-changed tracheoesophageal voice prostheses, heat and moisture exchangers (HMEs) and their adhesive and tube attachments, electrolarynx devices and voice amplifiers, and insertion and cleaning accessories. Douglas Insights sizes it at USD 520 million in 2025, about 160,000 device users at an average USD 3,250 a year, and at USD 886 million in 2035, a 5.47% compound annual growth rate (CAGR). The study sits beside our Hospital and Healthcare Humidifiers Market, Medical Implants Market and Intraoperative Neuromonitoring Systems Market reports; every figure follows the Douglas Insights research methodology.
Which laryngectomy product earns the most: voice prostheses, HMEs or electrolarynx devices?
Heat and moisture exchangers and their attachments earn the most, 44.0% of 2025 value or about USD 229 million, Douglas Insights estimates. They also grow fastest, 5.9% a year to USD 406 million by 2035.
| Product | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
|---|---|---|---|---|
| HMEs and attachment systems | 44.0% | USD 229 million | 5.9% | USD 406 million |
| Indwelling voice prostheses | 34.0% | USD 177 million | 5.6% | USD 305 million |
| Non-indwelling (patient-changed) voice prostheses | 6.0% | USD 31.2 million | 3.8% | USD 45.3 million |
| Electrolarynx devices and voice amplifiers | 9.0% | USD 46.8 million | 3.5% | USD 66.0 million |
| Insertion, cleaning and other accessories | 7.0% | USD 36.4 million | 5.0% | USD 59.3 million |
HMEs lead because they are consumed every day. A laryngectomee breathes through a neck stoma that bypasses the nose, so a foam or paper cassette over the stoma warms, humidifies and filters each breath, and most users change it 1 to 3 times a day. Medicare pays USD 0.97 for an HME filter and USD 3.56 for an integrated filter and holder at the national ceiling, the October 2026 DMEPOS fee schedule shows, so a year of daily HMEs and adhesives runs to about USD 1,000 to USD 2,000 per user, a Douglas Insights estimate. HME value grows 5.9% a year to USD 406 million.
Indwelling voice prostheses, USD 177 million, grow 5.6%: a silicone one-way valve sits in a surgical puncture between trachea and oesophagus and is changed by a clinician every few months as yeast biofilm makes it leak. Patient-changed prostheses, USD 31.2 million, grow 3.8%. Electrolarynx devices and amplifiers, USD 46.8 million, grow 3.5% as valve speech displaces buzzing handheld units. Accessories, USD 36.4 million, grow 5.0%. Product values for 2035 sum to USD 881 million, 0.47% below the headline.
Where are voice prostheses supplied: hospital and ENT clinic or home and direct-to-patient?
Home and direct-to-patient supply takes the larger share, 58.0% of 2025 value or about USD 302 million, Douglas Insights estimates, and grows faster, 5.9% a year to USD 535 million by 2035.
| Care setting | Share 2025 | Value 2025 | CAGR 2026-2035 | Value 2035 |
|---|---|---|---|---|
| Hospital and ENT clinic | 42.0% | USD 218 million | 4.9% | USD 352 million |
| Home and direct-to-patient | 58.0% | USD 302 million | 5.9% | USD 535 million |
Home supply leads because HMEs, adhesives, brushes and patient-changed devices are shipped to the user each month, often by the maker itself. Medicare separates the two settings: a prosthesis the patient inserts, code L8507, pays up to USD 61.43, while one inserted by a licensed provider, code L8509, pays up to USD 160.17, per the CMS fee file. Home value grows 5.9% as makers sell direct.
The hospital and ENT clinic setting, USD 218 million, grows 4.9%. It covers primary punctures at laryngectomy, clinic exchanges of indwelling devices and the speech and language therapists who fit them. Douglas Insights estimates that an indwelling device exchange takes about 15 to 30 minutes in clinic, so exchange frequency, not surgery count, sets most hospital volume. Setting values for 2035 sum to USD 887 million, 0.21% above the headline.
What drives voice prosthesis demand: new laryngectomies, longer survival, reimbursement or HME adoption?
Device users rise 3.4% a year in our base case, from about 160,000 in 2025 to about 224,000 in 2035. Four drivers add 3.70 points of gross growth; two restraints remove 0.30 points.
New laryngeal cancer cases: 1.10 points
Incidence is the largest driver, worth 1.10 points. IARC estimates 193,937 new laryngeal cancers and 96,619 deaths worldwide, ranking the disease 21st by cases, in its GLOBOCAN 2024 fact sheet dated 8 July 2026. In the United States the National Cancer Institute projects 12,290 new cases and 3,960 deaths in 2026, per SEER. France’s health technology agency counted about 1,250 laryngectomies in 2024, per its opinion of 11 February 2025. Douglas Insights estimates that about 20,000 total laryngectomies a year worldwide add new device users.
Longer survival and a larger installed base: 0.90 points
Survival adds 0.90 points, because each laryngectomee uses devices for life. Five-year relative survival for laryngeal cancer is 62.5% for 2016 to 2022 diagnoses, and about 87,355 Americans were living with the disease in 2023, SEER shows. France counts at most 10,000 laryngectomees, per the HAS opinion, about 147 per million people, a Douglas Insights calculation. Douglas Insights estimates that each extra year of median survival adds about 4,000 users worldwide.
New reimbursement in emerging markets: 0.90 points
Access adds 0.90 points. Coloplast said emerging markets grew fastest in Voice and Respiratory Care in 2025/26 and that Poland grew double digits after reimbursement opened, in its full-year release; fourth-quarter segment revenue rose 9% organically to DKK 631 million. Where a country starts paying for HMEs, users who made do with gauze or electrolarynx devices move to daily cassettes, a Douglas Insights estimate. We assume about 3,000 such users join each year.
Higher-value devices and HME use: 0.80 points
Product mix adds the last 0.80 points. Coloplast attributed high single-digit laryngectomy growth to more patients and higher patient value from Provox Life, per its 2 November 2026 release. France’s HAS rated Provox Life a minor improvement (ASA IV) with a 5-year listing, per its February 2025 opinion. Summed, 1.10 + 0.90 + 0.90 + 0.80 = 3.70 points, a Douglas Insights estimate.
What holds voice prosthesis demand back: organ preservation or reimbursement ceilings?
Two restraints remove 0.30 points a year from user growth: larynx-sparing treatment takes 0.20 points and low reimbursement ceilings take 0.10 points.
Larynx-sparing treatment is the larger drag. Many early and some advanced tumours are now treated with radiotherapy or chemoradiotherapy instead of removing the larynx, so a rising case count does not raise laryngectomies one for one. The United States counts 12,290 projected cases for 2026 against 3,960 deaths, per SEER, and Douglas Insights estimates that only about 1 in 5 laryngeal cancer patients worldwide has a total laryngectomy. A 1-point fall in that share removes about 1,000 new users a year.
Reimbursement ceilings are the smaller drag. The Medicare floor for a provider-inserted prosthesis is USD 120.13 and for a patient-inserted one USD 46.07, per the October 2026 CMS file, which leaves little room for premium devices in fee-for-service plans. In low-income countries without HME coverage, users buy few consumables, a Douglas Insights estimate. We assume these limits remove about 500 users a year from the paid market.
Which region spends the most on voice prostheses, and where does demand grow fastest?
Europe spends the most on voice prosthesis devices, USD 229 million or 44.0% of 2025 value, and Asia Pacific grows fastest, 7.4% a year, Douglas Insights estimates.
| Region | Value 2025 | Share 2025 | Value 2026 | CAGR 2026-2035 | Value 2035 |
|---|---|---|---|---|---|
| Europe | USD 229 million | 44.0% | USD 240 million | 4.9% | USD 369 million |
| North America | USD 156 million | 30.0% | USD 164 million | 5.3% | USD 262 million |
| Asia Pacific | USD 78.0 million | 15.0% | USD 83.8 million | 7.4% | USD 159 million |
| Latin America | USD 31.2 million | 6.0% | USD 33.2 million | 6.5% | USD 58.6 million |
| Middle East and Africa | USD 26.0 million | 5.0% | USD 27.8 million | 6.8% | USD 50.2 million |
Europe leads because tracheoesophageal speech and daily HME use are standard care there and national systems pay for both. France alone lists Provox Life for up to 10,000 laryngectomees, per HAS, and Poland’s new reimbursement lifted Coloplast’s sales there by double digits, per its 2025/26 release. Regional value grows 4.9% a year to USD 369 million, a Douglas Insights estimate.
North America, USD 156 million, grows 5.3% on Medicare codes L8507 to L8515 and private plans. Asia Pacific, USD 78.0 million, grows 7.4% as Chinese, Indian and Southeast Asian cancer centres adopt valve speech in place of electrolarynx units. Latin America, USD 31.2 million, grows 6.5%, and the Middle East and Africa, USD 26.0 million, grows 6.8%. Regional values sum to USD 549 million in 2026, 0.11% above the headline, and USD 899 million in 2035, 1.48% above it.
Which companies lead the voice prosthesis devices market?
Atos Medical, owned by Coloplast, leads voice prosthesis devices with about 46% of 2025 value, Douglas Insights estimates, ahead of InHealth Technologies (Freudenberg Medical) at about 22% and Andreas Fahl Medizintechnik at about 10%; Heimomed and Griffin Laboratories follow.
| Company | Laryngectomy lines | Latest reported figure | Source date |
|---|---|---|---|
| Atos Medical (Coloplast) | Provox Vega, Provox Life HMEs | Voice and Respiratory Care DKK 2.42 billion, organic growth 8% | 2 November 2026 |
| Atos Medical (Coloplast) | Same | 9M 2025/26 segment revenue DKK 1.79 billion, organic growth 7% | 18 August 2026 |
| InHealth Technologies (Freudenberg Medical) | Blom-Singer prostheses | Class 2 recall of 12 mislabelled 16Fr units | Initiated 17 June 2026 |
| Atos Medical | Provox | 35 years since 1990 launch | 7 April 2025 |
Coloplast’s Voice and Respiratory Care revenue rose from DKK 2.28 billion to DKK 2.42 billion in 2025/26, 6% in Danish kroner and 8% organically, about 8% of group revenue of DKK 28.9 billion, and Coloplast finished integrating Atos in the fourth quarter, its release of 2 November 2026 states. Nine-month segment revenue was DKK 1.79 billion, per its 18 August 2026 release. Atos marked 35 years of Provox in April 2025 and said it is working on longer-lasting devices, per its news post.
InHealth Technologies, part of Freudenberg Medical, sells the Blom-Singer range. Its Classic Indwelling Voice Prosthesis 16Fr, 10 mm was the subject of a Class 2 recall of 12 units with mislabelled flange boxes, started on 17 June 2026, the Food and Drug Administration (FDA) recall database shows. The top three hold about 78% of value, because surgeons and speech therapists train on one system and patients stay with it for life, a Douglas Insights estimate.
The rest of the field is regional. Andreas Fahl Medizintechnik and Heimomed, both German, sell prostheses, HMEs, tubes and stoma buttons across Europe; Griffin Laboratories makes the TruTone electrolarynx in the United States; and Servox and Ultravoice serve the electrolarynx niche. Douglas Insights estimates that these suppliers and local distributors share about 22% of 2025 value, about USD 114 million, with electrolarynx makers taking about two-fifths of that.
How much does a voice prosthesis cost, and what does a year of laryngectomy supplies cost?
A laryngectomy device user spends an average USD 3,250 a year in 2025, Douglas Insights estimates, from under USD 1,000 for an electrolarynx user to more than USD 5,000 for a heavy HME and indwelling device user in Europe.
Medicare gives the clearest public prices. At the national ceiling, a provider-inserted prosthesis pays USD 160.17, a patient-inserted one USD 61.43, an indwelling insert USD 106.67, a puncture dilator USD 138.30 and an artificial larynx USD 1,077.27, with floors about 25% lower, the CMS DME26-D schedule shows. A gel cap pays USD 3.18 and a voice amplifier USD 370.62. List prices in Europe for a premium indwelling device run several times the Medicare figure, a Douglas Insights estimate.
Average spend rises 2.0% a year in the base case, to about USD 3,960 in 2035, because users move to adhesive-plus-cassette HME systems and longer-life valves cost more per unit. Device life is the swing factor: a Polish study of 899 exchange intervals found a median device life of 141 days, per Pawlicki and colleagues in Cancers, August 2026, so a user needs about 2.6 devices a year, a Douglas Insights calculation.
What if laryngectomy rates fall or access widens: how large is the voice prosthesis market in 2035?
The voice prosthesis devices market ends 2035 between USD 700 million and USD 1.07 billion across our three cases, around a base of USD 886 million. Surgery rates and reimbursement set most of the spread.
| Case | Volume leg | Price leg | Revenue rate | 2035 value |
|---|---|---|---|---|
| Slower | 2.00% | 1.00% | 3.02% | USD 700 million |
| Base case | 3.40% | 2.00% | 5.47% | USD 886 million |
| Faster | 4.60% | 2.80% | 7.53% | USD 1.07 billion |
In the slower case more tumours are treated without surgery, emerging-market reimbursement stalls and Medicare fees stay flat, taking 1.4 points off volume and 1.0 point off price. In the faster case China and India fund HMEs and voice prostheses, survival keeps improving and premium devices spread, adding 1.2 points of volume and 0.8 points of price. One extra point of yearly revenue growth adds about USD 88 million to 2035 value, a Douglas Insights estimate.
Live signals sit at or above the base case. Coloplast’s 8% organic segment growth and 9% fourth quarter, per its November 2026 release, exceed our 5.47% rate, while flat US incidence near 12,290 cases, per SEER, caps volume. The signal to watch is Coloplast’s first-quarter 2026/27 segment growth, due in early 2027: above 7%, Douglas Insights would raise the volume leg.
Which rules shape voice prosthesis devices, from the EU MDR deadlines to Medicare codes?
Two rule sets shape voice prosthesis devices most: the EU Medical Device Regulation (MDR), whose legacy deadlines run to 2027 and 2028, and Medicare’s HCPCS codes L8500 to L8515, which set US prices.
The European layer sets market access. Regulation (EU) 2023/607 of 15 March 2023 lets legacy class III and implantable class IIb devices stay on sale until 31 December 2027 and other class IIb, class IIa and sterile class I devices until 31 December 2028, per EUR-Lex. On 16 December 2025 the European Commission proposed COM(2025) 1023 to simplify the MDR, with time limits for notified bodies and expected savings of EUR 3.3 billion a year, per its press release.
The US layer sets payment and safety. Medicare’s DMEPOS fee schedule prices each code, from L8507 at USD 61.43 to L8500 at USD 1,077.27, per CMS, and the FDA regulates prostheses as medical devices, recording recalls such as the June 2026 Blom-Singer labelling action in its recall database. France’s HAS adds a health technology layer: Provox Life won a 5-year listing in February 2025, per its opinion.
Douglas Exclusive: the Prosthesis Change Clock
The Prosthesis Change Clock is a Douglas Insights model of 9 inputs: 7 device-life figures from 3 clinical studies, 1 Medicare fee and 1 national laryngectomy count. It converts device life into yearly exchanges and spend per user.
| Date | Input | What it moves |
|---|---|---|
| January 2025 | Żurek trial: 3.03 months scheduled vs 5.37 months on leak | Exchange policy (2 inputs) |
| 11 February 2025 | HAS: about 1,250 French laryngectomies in 2024 | New users |
| 19 February 2026 | Taira study: mean 85 days | Dry-airway users |
| 7 August 2026 | Pawlicki study: median 141, 179, 136 and 84 days | Treatment history (4 inputs) |
| October 2026 | Medicare L8509: USD 160.17 | Price per exchange |
The finding, a Douglas Insights calculation: at the Polish median of 141 days a user needs 2.6 devices a year, worth about USD 415 at the Medicare L8509 ceiling. After chemoradiotherapy, where the median falls to 84 days, the user needs 4.3 devices a year, about USD 696, 68% more, per the device lives reported by Pawlicki and colleagues. Patients whose laryngectomy follows failed radiotherapy therefore buy far more devices than those treated by primary surgery, at 179 days or about 2.0 a year.
Exchange policy matters as much as biology. In a randomised trial of 40 patients and 153 prostheses, scheduled exchange every 3.03 months on average used devices almost twice as fast as exchange only when they leaked, at 5.37 months, per Żurek and colleagues in Frontiers in Oncology, January 2025, and a Japanese series of 24 patients found a mean of 85 days, with tracheal dryness linked to shorter life, per Taira and colleagues, February 2026. Better HME use, which keeps the airway moist, can therefore lengthen device life and partly offset prosthesis volume, a Douglas Insights estimate.
How does the methodology turn 160,000 users into USD 520 million?
How this report is built
- Every figure carries a confidence grade in the fact sheet above, and the working model ships with every licence.
- Five regional models sum to the global figure, with country tables in the Excel model.
- The next scheduled review of this study is May 2027.
- Licence holders receive it as a maintained tab in the Excel model.
Our methodology multiplies about 160,000 laryngectomy device users in 2025 by an average USD 3,250 a year to reach USD 520 million, then grows users 3.4% a year and spend per user 2.0% a year.
The model covers 5 products, 2 care settings and 5 regions, using 68 data points, 22 sourced and 46 modelled. Volume comes from a patient build: about 150 laryngectomees per million people in high-income countries, scaled from the French count of at most 10,000, applied to about 1.1 billion people with full reimbursement, gives about 165,000, less non-users and plus partly covered users elsewhere, for about 160,000, Douglas Insights estimates. Arithmetic: 160,000 users x USD 3,250 = USD 520 million; USD 520 million x (1.034 x 1.020)^10 = USD 886 million. Users reach about 165,400 in 2026 and 223,500 in 2035.
Two cross-checks agree. First, laryngectomy was about 70% of Coloplast’s DKK 2.28 billion Voice and Respiratory Care revenue in 2024/25, about DKK 1.60 billion or USD 240 million at about DKK 6.6 per dollar, a Douglas Insights conversion, which fits an Atos share near 46%. Second, a median device life of 141 days, per Pawlicki and colleagues, matches our assumption of about 2.6 prostheses per indwelling user a year. AI tools help draft the text and automated checks test the arithmetic and links; the Douglas Insights research desk chooses the sources, builds the model and approves publication, as our editorial standards explain.
