Which companies control liquid nutritional supplement shelves and hospital wards?

Abbott leads liquid nutritional supplements with an estimated 10.6% share in 2025, and the top three, Abbott, Nestle Health Science and Danone, hold 26.5% between them. Their edge is clinical credibility: hospital formularies, dietitian referrals and pharmacy shelf space. The same firms appear in our Nutraceutical Products Market study, where liquids are one format among many.

Douglas Insights estimates Abbott sells about USD 2.60 billion of liquid product, anchored on Adult Nutritionals sales of USD 4.48 billion in 2025, up 3.0% organically, of which roughly 58% we treat as ready-to-drink Ensure and Glucerna volume. The picture turned in 2026. Abbott reported Adult Nutrition sales of USD 1.12 billion for the second quarter, down 4.0% on a comparable basis, with U.S. sales off 6.4% after pricing actions (Abbott second-quarter 2026 results).

Nestle Health Science reported CHF 6.55 billion of 2025 sales with 3.2% organic growth, and its medical nutrition brands Resource and Compleat grew at a mid single-digit pace (Nestle full-year 2025 results). We attribute USD 2.01 billion, a 8.2% share, to liquid formats, since much of the unit sells capsules and powders.

Danone grew Specialized Nutrition 7.4% like-for-like to EUR 9.28 billion in 2025, with Adult Medical Nutrition leading Europe in the fourth quarter and Kate Farms lifting North America (Danone full-year 2025 release). Kate Farms brought plant-based shakes stocked in more than 1,400 U.S. hospitals when Danone took majority control in 2025. Douglas Insights puts Danone at USD 1.88 billion, or 7.7%, before any Huel contribution.

BellRing Brands is the consumer challenger. Premier Protein net sales rose 16.8% in fiscal 2025 on 14.6% higher volume, and group net sales reached USD 2.32 billion (BellRing fiscal 2025 results (SEC)). We count USD 1.79 billion of that as ready-to-drink shakes, a 7.3% share. Fresenius Kabi completes the clinical tier: its nutrition unit booked EUR 602 million in the fourth quarter of 2025, with 5% organic growth, and we place its sip-feed liquids at USD 688.0 million.

Liquid nutritional supplements include every drinkable, shelf-stable or chilled product sold to add nutrients, worth USD 24.52 billion across 11.62 billion servings in 2025. Powders, tablets, gummies, infant formula, sports energy drinks and tube feeds sit outside the scope, because their price per serving and buyers differ.

The model tracks five product groups, four end users (older adults, clinical patients, active adults and children) and four channels: retail pharmacies, hospitals and long-term care, supermarkets and mass merchants, and e-commerce. A serving is the label serving, typically 200 to 330 ml for a shake and 5 to 30 ml for a liquid vitamin. Plant-protein formulas overlap with the Plant Based Supplement Market, and iron syrups with the Iron Supplements Market; we count only the liquid share of each.

Oral nutritional supplement drinks are worth USD 6.72 billion. Disease-related malnutrition protocols, energy-dense 1.5 to 2.4 kcal per ml formulas and reimbursement in several European systems explain the share.

Adult and senior nutrition shakes add USD 5.84 billion, a 23.8% slice built on Ensure, Boost and similar pharmacy brands bought by families for parents over 65. Growth of 4.38% tracks ageing, not innovation.

Protein ready-to-drink shakes bring USD 5.32 billion today and USD 11.77 billion by 2035. Gym users and weight-management buyers want 20 to 42 grams of protein in a sealed bottle, and club stores sell them by the 18-pack.

Liquid vitamins and minerals account for USD 4.00 billion. Cheap per serving, they matter for children and for adults who struggle to swallow tablets, so their 5.05% growth leans on pediatric drops and iron syrups.

Liquid shots and functional concentrates are the smallest group at USD 2.65 billion and 10.8% of value. Collagen, probiotic and electrolyte shots expand at 5.71% a year, mostly online.

Why are older adults and protein shake buyers lifting liquid nutritional supplement volumes?

Liquid nutritional supplement servings grow 4.35% a year to 2035, from 11.62 billion to 17.79 billion. Four forces supply that volume leg: ageing households, protein shakes, clinical feeding protocols and online and Asian demand, with older adults the largest single contributor.

Ageing adds 1.62 points. Douglas Insights counts adult and senior shakes plus oral nutritional supplement drinks at 51.2% of 2025 value, and both track people over 75 who lose muscle and appetite. In our model every additional million people above 80 lifts annual demand by roughly 41 million servings, because about one in five buys a shake at least weekly. Older adults already buy 39.2% of all servings, and their share edges up by about 0.3 points a year as the over-80 cohort expands faster than any younger group.

Protein shakes add 1.48 points. BellRing reported 14.6% higher Premier Protein volume in fiscal 2025 and an 18.4% volume rise in ready-to-drink shakes in the fourth quarter (BellRing results). That rate cools to about 8% a year in our forecast, still double the market. Ready-to-drink protein now sits in coffee aisles and convenience coolers, not only nutrition shelves. A typical bottle carries 30 grams of protein for under USD 2.60, cheaper per gram than many bars, which is why active adults switch formats from powders and bars to bottles they can carry to work or the gym.

Clinical protocols add 0.81 points. Hospital screening for malnutrition triggers a sip-feed order of two bottles a day for 14 to 30 days in most European and North American pathways. Danone said Adult Medical Nutrition led European growth late in 2025 and that Kate Farms is scaling U.S. medical nutrition (Danone results). Each 1% rise in screened admissions adds about 9 million servings to our count.

Online and Asian demand adds the last 0.44 points. Huel, bought by Danone, sells mostly direct to consumers, which shows how subscription shakes reach buyers outside pharmacies (Danone on Huel). Asia Pacific liquid supplement revenue grows 7.48% a year in our model, led by China, Japan and India. E-commerce already moves 17.3% of category value, and subscription buyers order a 12-bottle case every 2 to 4 weeks, so a single online customer is worth roughly 180 servings a year. The four contributions, 1.62, 1.48, 0.81 and 0.44 points, sum to the 4.35% volume leg.

Which headwinds slow liquid nutritional supplement servings after 2025 price rises?

Three headwinds remove about 0.97 points from liquid nutritional supplement volume growth: price fatigue among U.S. shake buyers, sugar and labelling rules, and reimbursement caps. Without them the volume leg would run near 5.32% rather than 4.35%, and 2035 revenue would sit about USD 4.2 billion higher.

Price fatigue removes 0.42 points. Abbott reported U.S. Adult Nutrition sales down 6.4% in the second quarter of 2026 and blamed lower volumes after earlier pricing (Abbott). Shake buyers switch to store brands when a 6-pack passes USD 13.

Labelling rules remove 0.31 points. The U.S. healthy claim rule sets added-sugar and saturated-fat limits, with compliance due by 25 February 2028 (Federal Register). In our model, standard vanilla and chocolate adult shakes carry 15 to 20 grams of sugar per bottle and need 6 to 9 months of reformulation per product line.

Reimbursement caps remove 0.24 points. Douglas Insights estimates that 38% of oral nutritional supplement drinks are paid by health systems, and tenders in Europe press hospital prices 2 to 4% lower each round.

Where do liquid nutritional supplement sales grow fastest by region?

North America leads liquid nutritional supplements with USD 9.02 billion in 2025, 36.8% of global revenue, while Asia Pacific grows fastest at 7.48% a year. Protein shakes drive the first; ageing in China and Japan and online sales drive the second.

North America reaches USD 14.99 billion by 2035 at 5.21% a year, with BellRing and Abbott fighting for the U.S. shake shelf.

Europe holds USD 6.64 billion, growing 4.62% a year to USD 10.44 billion; hospital sip feeds and tendered reimbursement keep it slower but steady.

Buyers pay an average realised USD 2.11 per serving of liquid nutritional supplement in 2025, rising 1.42% a year to about USD 2.43 by 2035. The spread is wide: clinical sip feeds cost five times a spoon of liquid vitamin.

Oral nutritional supplement drinks fetch USD 2.60 to 4.10 per 200 ml bottle, because they are prescribed and reimbursed. Retail shakes realise USD 1.45 to 2.35 after promotions; BellRing said promotions cut price/mix late in fiscal 2025 even as volume rose 18.4%.

Douglas Insights expects the 1.42% price leg to come from mix, not list prices. Higher-protein and plant-based formulas carry 15 to 25% premiums, while the base vanilla shake stays flat.

Which sales channels move liquid nutritional supplements, from pharmacy counter to care home?

Retail pharmacies sell about 31.4% of liquid nutritional supplement value, the largest channel, because older buyers trust pharmacist advice. Hospitals and long-term care take 24.6%, supermarkets and mass merchants 26.7% and e-commerce 17.3%, the share that grows fastest in our 2035 model.

E-commerce grows fastest, near 9.4% a year in our model, because subscription shakes ship by the case. Hospitals and long-term care buy on annual tenders with 12 to 36 month contracts.

Do older adults, clinical patients, active adults or children drink the most liquid nutritional supplements?

Older adults buy 39.2% of liquid nutritional supplement servings, ahead of active adults at 27.5%, clinical patients at 21.8% and children at 11.5%. Older adults buy for appetite and muscle loss after 75; active adults buy 20 to 42 grams of protein per bottle.

Clinical patients drink fewer servings but at the highest price, so they carry more value than volume suggests. Children use mostly liquid vitamins, drops and growth shakes sold in pharmacies.

What if liquid nutritional supplement volumes run faster or slower to 2035?

The base case reaches USD 43.22 billion in 2035; a slower case lands at USD 36.97 billion and a faster case at USD 50.59 billion. Volume carries most of the swing: legs run from 3.12% to 5.58% a year for servings and 1.04% to 1.83% for price.

Which rules govern liquid nutritional supplement labels, from medical foods to the healthy claim?

Two rulebooks shape liquid nutritional supplements: food for special medical purposes (FSMP) rules in Europe and U.S. food labelling, including the healthy claim due by 25 February 2028 (Federal Register). Together the two rulebooks reach about 51.2% of liquid category value in 2025.

In the European Union, Commission Delegated Regulation (EU) 2016/128 sets composition and information rules for FSMP, which covers most hospital sip feeds. Products must be used under medical supervision, so retail claims stay narrow.

In the United States, adult shakes are regulated as foods and medical foods, not drugs. The healthy rule caps added sugar per serving, and Douglas Insights estimates about 60% of adult shake stock-keeping units (SKUs) would need a sugar cut to qualify.
